Hipertansiyon, dünya nüfusunun önemli bir bölümünü etkileyen önemli bir sağlık
sorunudur. Hipertansiyon tanısında en sık kullanılan yöntemler muayenehanede veya
klinikte hemĢire veya doktor tarafından ya da hasta veya yakını tarafından evde veya
24 saatlik ambulatuar kan basıncı (KB) ölçümüdür. Ancak eforla oluĢan hipertansif
cevap, ofiste KB ölçümü ve ambulatuar KB ölçümüne kıyasla özellikle hedef organ
hasarını göstermede daha değerli bulunmuĢtur. Ayrıca antihipertansif ilaçların
yalnızca istirahatte değil, fiziksel aktivite sırasında da kan basıncını kontrol altına
alması gerektiği gösterilmiĢtir. Eforla oluĢan hipertansif cevabın
değerlendirilmesinde, fiziksel ya da mental stresi ve yeniden toparlanmayı içeren
kısa süreli, kolay uygulanabilir ve takip edilebilir bir test olan efor testi
uygulanabilir. Beta blokerlerin egzersiz sırasında kan basıncı cevabı üzerine olumlu
etkileri konusunda yapılmıĢ çeĢitli çalıĢmalar bulunmaktadır. Fakat beta bloker
ajanların kendi aralarında bu faydalı etki açısından fark olup olmadığı konusu ise
yeterince irdelenmemiĢtir. Bizim çalıĢmamızda her ikisi de beta-1 selektif beta
bloker olan metoprolol ile ek olarak nitrit oksit (NO) salınımı özelliği ile vazodilatör
etkiye sahip olan nebivololün düĢük riskli hipertansif hastalarda efor testi sırasında
sistolik ve diastolik kan basıncı ve kalp hızı üzerine etkileri karĢılaĢtırıldı. DüĢük
riskli hipertansif 50 hasta (38 kadın ve 12 erkek) çalıĢmaya alındı. Ortalama yaĢ
54.2±9.5 idi. Metoprolol ya da nebivolol tedavisi altında olan bu hastalara treadmill
egzersiz testi uygulandı. Egzersize hemodinamik cevap, sistolik ve diyastolik kan
basıncı ve kalp hızındaki değiĢikliklerle değerlendirildi. Metoprolol ve nebivololün
hem istirahat hem de egzersiz sırasında iyi kan basıncı kontrolü sağladığı ve egzersiz
sırasında kan basıncı ve kalp hızı üzerine benzer etkileri olduğu bulundu.
Hypertension is an important health
problem which affects a significant portion of the world population. Most frequent
methods on hypertension diagnosis are blood pressure measurement at doctor’s
office or clinic or at home by the patient or by relatives or 24 hours ambulatory blood
pressure measurement. However; hypertensive response that occurs by exercise have
been indicated to be more valuable than blood pressure measurement at office or 24
hours ambulatory blood pressure measurement at displaying target organ damages.
Additionally, it has been indicated that antihypertensive treatment is required for
blood pressure control not only on rest but also on exercise. Exercise stress testing
can be performed in evaluation of hypertensive response that occurs by exercise
because it is short-term, easy applicable and can be trace and includes physical or
mental stress and recovery stage. There are many studies about positive effects of
beta blokers on blood pressure response. However, whether there is difference in
beta blocker agents among themselves about this beneficial effects has not been
investigated sufficiently. In our study, affects of metoprolol which is B1 selective
and nebivolol is B1 selective which also has vasodilator affects, for low risk
hypertension patients during exercise test, maksimal sytolic, diastolic blood pressure
and blood velocity are compared. Fifty hypertensive patients (38 female and 12
male) with low risk included in the study. Mean age was 54.2±9.5. These patients
under antihypertensive therapy with metoprolol or nebivolol were studied during
treadmill exercise testing. The hemodynamic responses to exercise were evaluated by
changes in systolic and diastolic blood pressure and heart rate. It is concluded that
metoprolol and nebivolol provide blood pressure control both at rest and exercise
succesfully and show similar effects on blood pressure and heart rate during exercise.
