Bu çalışma propofol ve fentanil indüksiyonuna
eklenen farklı dozlardaki lidokainin LMA yerleştirilmesi üzerine etkilerinin
araştırılması amacıyla Mart2009-Ağustos 2009 tarihleri arasında, farklı tanılarla,
genel anestezi verilerek elektif cerrahi girişim planlanan, ASA I-II grubu, 18-65 yaş
arası 45 erişkin hasta üzerinde gerçekleştirildi. Hastalar rastgele, Grup I (0,5mg/kg
lidokain), Grup II (1 mg/kg lidokain) ve Grup III (1,5mg/kg lidokain) olarak 15’er
kişilik üç gruba ayrıldı. Premedikasyon yapılmaksızın, 3 dakikalık
preoksijenizasyonu takiben, tüm hastalara 1mcg/kg i.v fentanil ve ardından Grup
1’e 0,5mg/kg lidokain , Grup II’ye 1mg/kg lidokain ve grup III’e 1,5mg/kg lidokain
2 dakika içerisinde iv olarak uygulandı ve sonrasında anestezi indüksiyonu için 2.5
mg/kg propofol verildi.İdamede, %50 oksijen , %50 N2O ve %2 sevofluran
kullanıldı. Propofol enjeksiyonundan 90 saniye sonra LMA yerleştirilmesi için ilk
deneme yapıldı. Hastaların sistolik, diastolik ve ortalama kan basınçları
(SKB,DKB,OKB), oksijen saturasyonları (spO2), kalp atım hızları(KAH); bazal,
LMA öncesi, LMA sonrası 1dakika (dk), 3dk., 5dk, 7dk, 10dk, 30dk. ve 60.dk
kaydedildi. Ayrıca maske ile ventilasyon, çene gevşemesi, LMA yerleştirilmesine ve
kafın şişirilmesine yanıt 3 puanlı skala ile mükemmel, iyi, zayıf olarak skorlanmış ve
bunların gruplar arasında karşılaştırılması planlanmıştır. LMA uygulamalarında
optimal koşulları sağlamak amacıyla propofol ve fentanil indüksiyonuna eklenen
lidokainin farklı dozlarıyla yapılan karşılaştırmada, her 3 grupta; KAH, SKB, DKB,
KAH ve SpO2 değerlerinde anlamlı bir farklılık saptanmadı. Ancak 1mg/kg ve
1,5mg/kg lidokain uygulamasının optimal koşulları sağladığı, 0,5mg/kg lidokainin
ise bu konuda yetersiz olduğu görüldü. Sonuç olarak propofol ve fentanil
indüksiyonu ile yapılan LMA uygulamalarında optimal koşulları sağlamak için
1mg/kg iv. lidokainin yeterli doz olduğu kanısındayız.
This study was performed to investigate the
effects of different lidocain doses which added to propofol and fentanyl induction on
LMA insertion in 45 adult patients aged 18-65 years within ASA I-II group and
planned elective surgery due to different diagnosis during March 2009 – August
2009. The patients were randomly divided three different groups included 15 patients
as Group I (0.5 mg/kg lidocaine), Grup II (1 mg/kg lidocaine) and Group III (1.5
mg/kg lidocaine). Without premedication, Group I received 0.5 mg/kg, Group II
received 1 mg/kg and Group III received 1.5 mg/kg intravenous lidocaine within 2
minutes following preoxygenation over 3 minutes and 1 mcm/kg i.v. fentanyl
introduction and then 2.5 mg/kg propofol was given to anesthesia induction. For
anesthesia maintenance, 50% oxygen, 50% N2O and 2% sevoflurane were used. 90
second after the injection of propofol, initial LMA insertion was tried. The systolic,
diastolic and mean blood pressures (SBP, DBP, MBP), oxygen saturations (SpO2),
heart rate (HR) were recorded at baseline, before LMA insertion and at 1, 3, 5, 7, 10,
30 and 60 min after LMA insertion. Additionally, ventilation with mask, jaw
relaxation, response to LMA insertion and cuff puffing (coughing, gagging,
swallowing, laryngospasm) was scored by 3-point scale as excellent, good and poor
and these parameters was planned to compare between groups. When different doses
of lidocaine which added to propofol and fentanyl induction in order to elicit the
optimal conditions during LMA insertions were compared, there was no significant
difference for SBP, DBP, MBP, HR and SpO2 values between groups. However, it
was seen that 1 mg/kg and 1.5 mg/kg of lidocaine introductions provide the optimal
conditions for LMA insertion but 0.5 mg/kg of lidocaine is inadequate. As a
conclusion, we decided that 1 mg/kg i.v. lidocaine is an adequate dose to elicit
optimal conditions during LMA insertions with propofol and fentanyl induction.
