Preterm doğum, gebeliğin yaĢayabilirlik sınırından sonra ve 37 hafta veya 259
günden önce doğumun gerçekleĢmesi demektir. Preterm doğumlar perinatal mortalite
ve morbiditenin en önemli sebebidir. ÇalıĢmamıza, EskiĢehir Osmangazi
Üniversitesi Tıp Fakültesi Kadın Hastalıkları ve Doğum Anabilim Dalı, Perinataloji
Ünitesi’nde Ocak 2008 ile Ocak 2010 tarihleri arasında, 20 hafta ile 36 hafta 6 gün
arasında preterm doğum yapmıĢ, 213 hastadan dosyasına ulaĢılabilen 185’i ve bu
gebelikler sonucu doğmuĢ olan yenidoğanlar dahil edildi. Veriler hasta dosyalarından
retrospektif olarak sağlandı. ÇalıĢmaya dahil edilen hastalardan, 3’ü (%1.6) 18 yaĢ
ve altı, 156’sı (%84.3) 19-34 yaĢ arası, 26’sı (%14.1) 35 yaĢın üzerindeydi. Ġlk
gebeliğinde baĢvuran 87 (%47), ikinci gebeliğinde baĢvuran 41 (%22.2), üçüncü
gebeliğinde baĢvuran 26 (%14.1), dördüncü gebeliğinde baĢvuran 17 (%9.2), beĢinci
ve daha sonraki gebeliğinde baĢvuran 14 (%7.5) olgu mevcuttu. ÇalıĢmamızda
preterm doğum oranı %19 idi. Preterm doğumların %51.9’u preterm eylem, %7’si
EMR, %38.9’u ise maternal-fetal problemler nedeniyle gerçekleĢmiĢti. Doğum
haftası ile yenidoğan dönemine ait komplikasyonlar değerlendirildiğinde,
gestasyonel yaĢ azaldıkça intrakranial kanama (p<0.01), konvülziyon (p<0.001),
RDS (p<0.001), entübe edilme (p<0.001), mortalite (p<0.001), surfaktan ihtiyacı
(p<0.001), yenidoğan retinopatisi (p<0.001) gibi komplikasyonların arttığı görüldü.
Preterm eylemde olan ve erken membran rüptürü olan hastalarda baĢvuruda servikal
açıklık arttıkça, antenatal kalıĢ süresinde yani doğuma kadar geçen sürede anlamlı
azalma olduğu gözlendi. Tokolitik tedavi baĢlanan 11 hastada ya tokolitik ajan
değiĢtirilmek zorunda kalındı ya da tedaviye yeni bir tokolitik eklendi. Çoğul
preterm eylemlerde ortalama baĢvuru haftası 31.2±3.8 hafta, ortalama doğum haftası
31.9±4.1 hafta iken, çoğul olmayan preterm eylemlerde ortalama baĢvuru haftası
32.9±3.5 ve ortalama doğum haftası 33.5±3.4 hafta idi.
Preterm birth (PTB) refers to a birth that occurs before 37
completed weeks or less than 259 days of gestation. Preterm birth is the major cause
of perinatal morbidity and mortality. Hundred eighty-five of 213 patients who had
preterm birth between 20 weeks and 36 weeks and 6 days between the period of
January 2008 and January 2010 in Department of Obstetrics and Gynecology of
Osmangazi University School of Medicine and their newborns were included into the
study. Three of the patients (1.6%) were younger then 18, 156 (84.3) were between
the ages of 19 and 34, 26 ( 14.1%) were older than 35 years. Eighty-seven ( 47%)
patients were in their first pregnancy, 41 patients ( 22.2%) were in their second
pregnancy, 26 (14.1%) patients were in their third pregnancy, 17( 9.2%) patients
were in their fourth pregnancy and 14 ( 7.5) patients were in their fifth or over
pregnancies. Nineteen per cent of births in our study was preterm. The etiological
factors were preterm labor (51.9%), preterm rupture of the membranes (7%) and
maternal-fetal indication (38.9%). When the gestational age at birth and
complications of newborn were evaluated; it was observed that complications like
intracranial hemorrhage (p<0.01), convulsions (p<0.001), RDS (p<0.001), the need
of intubation (p<0.001), mortality(p<0.001), surfactan requirement (p<0.001),
retinopathy of newborn (p<0.001) were increased with decreasing gestational age.
The length of antenatal hospital stay of the patients with preterm labor and preterm
rupture of membranes was significantly less when the cervical dilatation at the time
of application was more. Tocolytic treatments of 11 patients were changed or another
tocolytic was added to the treatment. In the multiple pregnancies mean application
time to hospital was 31.2±3.8 gestational weeks and mean gestational age at the
birth was 31.9±4.1 but it was 32.9±3.5 and 33.5±3.4 respectively for the single
pregnancies.
