Bu çalışma, nazal salmon kalsitoninin (sCT) tedavisinin diz OA’ inde ağrı,
fonksiyonel durum, kartilaj dejenerasyonu ile ilişkili serumda bulunan Nitrik Oksit (NO),
Malondialdehit (MDA), İnterlökin 1 Beta (İL-1β), Matriks Metalloproteinaz-3 (MMP-3),
idrarda bulunan C-terminal telopeptid tip II kollojen (CTX-II) düzeylerine ve MRG ile
değerlendirilen kartilaj kalınlığı üzerine etkilerini değerlendirmek amacıyla yapılmıştır. Diz
OA ve postmenapozal osteoporoz tanısı alan 50 bayan hasta, randomize olarak iki gruba
ayrıldı. 30 kişiden oluşan 1. gruba 200IU/gün nazal sCT, 1200mg/gün kalsiyum ve 800IÜ/gün
vitD3 tedavisi ve ev egzersiz programı, 20 kişiden oluşan 2. gruba ise 1200mg/gün kalsiyum
ve 800IÜ/gün vitD3 tedavisi ve ev egzersiz programı 6 ay boyunca uygulandı. Tedavi öncesi
ve sonrası VAS (istirahat, aktivasyon, yürüme), WOMAC (ağrı, tutukluk, toplam)
parametreleri, 20 m yürüme süresi, serum İL-1 Beta, NO, MDA, MMP-3, 24 saatlik idrarda
CTX-II ve MRG ile diz eklemi kartilaj kalınlığı değerlendirildi. Tedavi sonrası 1. grupta VAS
(p<0.001), WOMAC ağrı, total (p<0.001) ve WOMAC tutukluk (p<0.05), 2. gruptaki
hastalarda VAS istirahat, yürüme (p<0.05), WOMAC total (p<0.05) değerlerinde anlamlı
iyileşme bulundu. Gruplar arası karşılaştırmada, sadece 1. grupta VAS aktivasyonda anlamlı
farklılık saptandı (p<0,05). Yirmi metre yürüme süresinde ise TS’ de 1. grupta anlamlı
iyileşme vardı (p<0.001). Gruplar arasında ise anlamlı fark yoktu. NO ve CTX-II
düzeylerinde, TS’ de 1. grupta anlamlı düzeyde düşme vardı (p<0,001). Gruplar arasında ise
TS’ de serum NO düzeylerinde 1. grupta anlamlı fark saptandı (p<0,001). Tedavi sonrası grup
1 deki hastalarda MFK alanında anlamlı iyileşme vardı (p<0,05). Çalışmamızın sonuçları,
nazal sCT tedavisinin diz OA’ lı hastaların semptomatik tedavisinde etkili olduğu ve aynı
zamanda anti-inflamatuar ve kondroprotektif etkilerinin de olduğunu göstermiştir.
This is study was to evaluate effects of nasal salmon calcitonin (sCT)
treatment in knee OA on the functional condition, NO, MDA, İL-1β, MMP-3, CTX-II levels
in the serum with regard to cartliage degeneration and thickness, which is evalueted by MRI.
50 patients diagnosed with knee OA and postmenopausal osteoporosis were randomly
separated into two groups. The 1st group that included 30 persons was given 200IU/day nasal
sCT, 1200mg/day calcium, 800IU/day vitD3 treatment and home excercise program, and the

nd group that comprised of 20 persons was given 1200mg/day calcium, 800IU/day vitD3
treatment and home excercise program for 6 months. The patients were assessed before and
after the treatment VAS (rest, activation walking), WOMAC (pain, stiffness, total) with, 20 m
walking time, serum levels İL-1β, NO, MDA, MMP-3, CTX-II in 24-hour urine, and knee
joint cartilage thickness results by MRI. Following the treatment, significant improvement
was observed in VAS (p<0.001), WOMAC pain, total (p<0.001) and WOMAC stiffness
(p<0.05) in group 1, and VAS resting, walking (p<0.05), WOMAC total (p<0.05) values of
group 2. In the comparison between the groups, significant difference was found in VAS
activation in the group 1 (p<0.05). İn group 1 demonstrated significant improvement after the
treatment in 20-meter walking time parameters (p<0.001). Difference was not established
between the groups. İn NO and CTX-II levels significant decrease was observed in group 1
following the treatment (p<0.001). Between these groups, significant difference was found
only in serum NO levels in group 1 (p<0.001). Significant improvement was established in
MFK of the patients in group 1 after the treatment (p<0.05). The results of our study
demonstrated that nasal sCT treatment is effective in the symptomatic treatment of patients
with knee OA, and it contains anti-inflammatory and chondroprotective effects as well.
