Koroner arter hastalığının tedavisinde yaygın olarak kullanılan stentlerin takibinde en
büyük problem erken tromboz ve restenozdur. Şimdiye kadar hiçbir girişim erken
stent trombozu (ST) ve stent-içi restenozu (SİR) tamamıyla engelleyememiştir.
Klinik çalısmalarda intravasküler ultrasonun (IVUS) stent yerleştirme stratejilerinde
anahtar rol oynayabileceği iddia edilmiştir. Bu çalışmada; kliniğimizde Ekim 2006
ve Nisan 2010 tarihleri arasında, takibinde kontrol koroner anjiyografileri yapılmış,
IVUS rehberliğinde ilaç kaplı stent implantasyonu yapılan 23 hastayla (Grup I),
Eylül 2008 ve Nisan 2010 tarihleri arasında IVUS kullanılmaksızın implante edilen
23 hasta (Grup II) olmak suretiyle toplam 46 hastanın dosya kayıtları retrospektif
olarak incelendi. Grup II hastaları; Grup I’deki hastaların morfolojik ve angiyografik
özellikleri, risk faktörleri, ilaç kullanım rejimleri ve kullanılan stent özellikleri
açısından fark olmayacak şekilde belirlendi. Hastaların ortalama yaşı Grup I’de
62±11 yıl, Grup II’de 58.1±8.5 yıl olarak tespit edildi. Her iki grupta 23 hasta olup,
Grup I’de 17 (%73.9), Grup II’de 15 (%65.2) erkek vardı. Tüm olgularda lezyon
uzunluğu, darlık yüzdesi, işlem öncesi minimal lümen çapı, işlem sonrası stent içi
minimal lümen çapı, kontrol anjiyografide stent içi minimal lümen çapı ölçüldü ve
geç lümen kaybı (GLK) değerleri hesaplandı. Gruplar arasında GLK değerleri, ST ve
SİR gelişen olgu sayıları karşılaştırıldı. Grup I’de GLK 0.4±0.6 mm iken, Grup II ‘de
bu değer 0.53±0.7 mm idi. Grup I’de 1 olguda SİR tespit edildi, akut ST gözlenmedi.
Grup II ‘de ise 3 hastada SİR, 1 hastada akut ST gözlendi. Her 3 parametrede IVUS
grubunda daha düşük değerler saptandı, fakat gruplar arasında istatistiksel olarak
anlamlı fark yoktu. Bunun nedeni muhtemelen calışmamızdaki en önemli kısıtlılık
olan yetersiz hasta sayısı idi. Sonuç olarak, ilaç kaplı stent implantasyonunda
IVUS’un akut tıkanma ve restenozu azaltmada diğer koroner girişimlerle beraber
tamamlayıcı bir yöntem olarak düşünülmesi gerekir.
Acute thrombosis and restenosis is
the most important problem related to stents, which are widely used in the treatment
of coronary artery disease. Currently, no procedure can prevent early stent
thrombosis (ST) and in-stent restenosis (ISR) completely. Clinical trials suggest that
ıntravascular ultrasound (IVUS) has a key role in stent implantation. In this study;
the records of 46 patients in total were analyzed retrospectively which are controlled
by follow-up coronary angiography and which consisting of two groups: 23 patients
with IVUS guidance drug-eluting stent implantation between October 2006 and April
2010 (Group I) and 23 patients without IVUS guidance implantation between
September 2008 and April 2010 (Group II). Group II patients were identified as will
be no difference in the morphologic and angiographic features, risk factors, drug
regimens and stent features of patients in Group I. The mean age of patients were
determined as 62±11 year in Group I and 58.1±8.5 year in Group II. There were 23
patients in each group, 17 (%73.9) male in Group I and 15 (%65.2) male in Group II.
In all cases were measured lesion length, percentage of stenosis, minimal lumen
diameter before operation, in-stent minimal lumen diameter after operation, in-stent
minimal lumen diameter in the control angiography and was calculated late lumen
loss (LLL) value. The LLL values, amount of patients with ST and ISR compared
between groups. The LLL value were 0.4±0.6 mm in Group I and 0.53±0.7 mm in
Group II. In Group I, ISR was detected in 1 case and acute ST not observed. In
Group II ISR was detected in 3 cases and acute ST in 1 case. In all three parameters
were determined lower values in IVUS group, but the differences were not
statistically significant. The reason for this probably was insufficient amount of
patients which were in our study. As a result, IVUS should be considered a
complementary method with other coronary interventions to decrease acute
thrombosis and restenosis in drug-eluting stent implantation.
