. Esas olarak yağ hücrelerinde
üretilen Adiponektin (AN) glukoz metabolizmasında rol oynar ve insülin
duyarlılığını arttırır. AN’nin antidiabetik, antiatherojenik ve antiinflamatuvar etkileri
olduğu gösterilmiştir. Bu çalışmada nonspesifik akut enfeksiyon geçiren çocuklarda
serum AN düzeyleri ve metabolik parametreler, ateş ve akut faz reaktanları ile ilişkisi
araştırıldı. Çalışmaya Eskişehir Osmangazi Üniversitesi Tıp Fakültesi Pediatri
Poliklinikleri ve Pediatri Acil Polikliniği’ne başvuran ve ≥38°C ateş saptanan, yaşları
14-163 ay arasında değişen, üst solunum yolu enfeksiyonu, otit, idrar yolu
enfeksiyonu gibi nonspesifik enfeksiyon tanısı alan, prepubertal dönemde ve boya
göre ideal vücut ağırlığı (BGVA) %100-110 olan 18 erkek, 24 kız olmak üzere
toplam 42 çocuk alındı. Hastalardan başvuru sırasında ve ateş düştükten üç gün sonra
(başvurudan 4-7 gün sonra) venöz kan örnekleri alınarak serum AN düzeyleri ve tam
kan sayımı, sedimentasyon, C-reaktif protein (CRP), fibrinojen, ferritin, high sensitif
CRP (HsCRP), kan glukoz, insülin, total kolesterol, trigliserid, LDL kolesterol
(LDL-C), HDL kolesterol (HDL-C) düzeyleri çalışıldı. Glukoz/insülin oranları,
Homeostasis model assessment of insulin resistance (HOMA-IR) ve vücut kitle
indeksleri (VKİ) hesaplandı. Bu hastaların başvuru sırasında ve kontrole
geldiklerinde serum AN düzeyleri arasında belirgin fark saptandı (p<0.0001).
Başvuru sırasında serum AN düzeyleri (8.8 ± 1.7 µg/ml), kontroldeki AN
düzeylerinden (12.04 ± 2.38 µg/ml) daha düşük idi. Başvuru sırasında serum AN
düzeyleri ile glukoz/insülin oranları arasında pozitif korelasyon saptandı (r=0.338,
p<0.05). Başvuru sırasında serum AN düzeyleri ile ateşin derecesi, vücut ağırlığı
(VA), BGVA, VKİ ve diğer parametreler arasında herhangi bir korelasyon
saptanmadı (p>0.05). Kontrolde ise sadece serum AN düzeyleri ile BGVA arasında
negatif korelasyon saptandı (r=-0.307, p<0.05). Başvuru sırasında kontrole göre
lökosit ve nötrofil sayıları ve CRP düzeyleri daha yüksek (sırasıyla p<0.001, p<0.001
ve p<0.01) ve lenfosit sayıları daha düşüktü (p<0.0001). Başvuru sırasında kontrole
göre serum glukoz düzeyleri, glukoz/insulin oranları ve HDL-C düzeyleri daha
yüksek (sırasıyla p<0.01, p<0.05 ve p<0.001) ve trigliserid ve LDL-C düzeyleri daha
düşük saptandı (sırasıyla p<0.001, p<0.01). Sonuç olarak akut enfeksiyonlu
çocuklarda serum AN düzeyleri değişmektedir. Bu değişikliğin ne anlama geldiği
farklı klinik modellerin oluşturulduğu çalışmalarla araştırılmalıdır.
Adiponectin (AN) plays a role mainly
in glucose metabolism and increases insulin sensitivity. It has been shown that AN
has antidiabetic, antiatherosclerotic and antiinflammatory activities. We analyzed
serum AN levels and relationship with metabolic parameters and fever and acute
phase reactants in children with nonspecific infections. Forty-two children (18 boys,
24 girls, age range: 14-163 months) were included in the study. These children were
admitted to our pediatric emergency unit and policlinics with fever ≥ 38°C and other
infection symptoms (upper respiratory tract infections, otitis, urine tract infections..).
They were all prepubertal children and their ideal body weights for height (IBW)
were within 100-110%. Two venous blood samples were taken from them during the
period of the fever at the admission and three days later after the fever dropped
(control) for analyzing serum AN levels and complete blood count, erythrocyte
sedimentation rate and CRP, fibrinogen, ferritin, HsCRP, glucose, insulin, total
cholesterol, triglycerides, LDL-C, HDL-C levels. Glucose/insulin rate and
Homeostasis model assessment of insulin resistance (HOMA-IR) and body mass
index (BMI) were calculated. There was significantly difference between the serum
AN levels analyzed at the admission and the control visit (p<0.0001). Serum AN
levels at the admission (8.8±1.7 µg/ml) were lower than at the control visit
(12.04±2.38 µg/ml). Serum AN levels were positively correlated with glucose/insulin
at the admission (p<0.05). Serum AN levels were not correlated with the degree of
fever, BW, IBW, BMI and the all other parameters analyzed at the admission
(p>0.05). Serum AN levels were negatively correlated only with IBW at the control
(r=-0.307, p<0.05). Only, leukocyte and neutrophil counts and CRP levels were
higher (p<0.001, p<0.001 and p<0.01, respectively), lymphocyte counts were lower
(p<0.001) at the control than at the admission. Serum glucose levels, glucose/insulin
and HDL-C levels were higher (p<0.01, p<0.05 and p<0.001, respectively) and
triglycerides and LDL-C levels were lower (p<0.001 and p<0.01, respectively) at the
admission than at the control. As a conclusion; serum AN levels are lower during the
acute phase with fever than the during recovery phase of acute infections in children.
It is needed further detailed studies to explain what the meaning of this finding is.
