Günümüzde, diabetes mellitus(DM) giderek
artan insidans ve prevalans ile önemli bir sağlık sorunu oluĢturmaktadır. DM koroner
arter hastalığı eĢdeğeri kabul edilmektedir. Hiperlipidemi de koroner arter hastalığı
için sigara, hipertansiyon, obesite ve diyabet gibi değiĢtirilebilir bir majör risk
faktörüdür. Bu yüzden tüm kılavuzlar diyabetik hastalarda tedavi hedeflerini
belirlerken glisemik hedefler yanında lipid hedeflerine de ulaĢmanın önemine dikkat
çekmiĢlerdir. Bu çalıĢma, endokrinoloji polikliniğine kontrole gelen Tip 2 diyabet
hastalarında kan lipid düzeylerini değerlendirmeyi, glisemik ve lipid hedeflerine
ulaĢmama nedenlerini, diyabet hastalığı takiplerinde üniversiteye baĢvurmadan
önceki baĢvurulan doktor sayısının, yaĢam tarzı değiĢikliklerinin önemini, hastalara
diyabet bilinçinin yerleĢtirilmesini hedeflemiĢtir. ÇalıĢmada Ekim 2013-Mart 2014
tarihleri arasındaki 6 aylık süreçte gönüllü 400 hastaya, yapılan çalıĢma konusunda
bilgilendirildikten ve onayları alındıktan sonra 28 soru içeren bir anket uygulanmıĢ
ve diyabet takiplerinde rutin istenilen kan değerlerine bakılmıĢtır. Hastaların
%34.5'inde LDL-K, % 41'inde TG, % 38.5'inde HDL-K, % 84.8'inde TK hedef
değerlerde bulunmuĢ; % 43.3'ünde HbA1C ≤ % 6.5, % 50.8'inde HbA1C ≤ 7.0
gözlemlenmiĢtir. ÇalıĢmamızın sonuçlarına bakıldığında; doktor sayısının 3 ve
üzerinde olması hedef parametrelere ulaĢma oranlarında ciddi azalmaya neden
olmaktadır. Bu sonuçlara göre LDL-K < 100 olarak alındığında bir doktor tarafından
takip edilen hastalarda hedef değere ulaşma oranı % 26.6, 2 farklı doktor tarafından
takip edilenlerde % 71.7 iken, 3 ve 4 doktor tarafından takip edilenlerde bu oran
sırasıyla % 1.4 ve % 7.0 olarak anlamlı olarak daha düşüktü. Diğer parametrelere
ulaşma oranında yine 3 ve 4 doktor tarafından takip edilenler 1-2 doktor tarafından
takip edilenlere göre anlamlı olarak daha düşüktü.
Department of Family Medicine Diabetes Mellitus (DM) is
presently becoming an important health problem with its growing incidence and
prevalance. DM is considered to be as risky as the coronary artery disease.
Hiperlipidemia, is also a major modifiable risk factor for coronary artery disease
along with smoking, hipertension, obesity and diabetes are. The refore all guides
have pointed out the importance of achieving lipid and glycemic targets when
defining the objectives for treatment. This study aims to evaluate blood lipid levels,
reasons for failing to achieve glicemic and lipidemic goals, total number of doctors
consulted, the importance of life style changes, as well as to raise the awareness for
diabetes in patients. The study involves 400 voluntary patients, who have been
admitted to the Endocrinology Clinic at the Practice and Research Hospital of the
EskiĢehir Osmangazi University Faculty of Medicine during a 6-month time frame
from October 2013 to March 2014. After informing the patients about this research
and obtaining their approvals, a survey of 28 questions and routine blood tests for
diabetes follow-ups have been conducted on the patients. Target levels of LDL-C in
34.5% of the patients, TG in %41.0 of the patients, HDL-K in 38.5% of the patients,
TC in 84.8% of the patients have been achieved. In 43,3% of thepatients; HbA1C ≤
6.5 and in 50.8% of the patients HbA1C ≤ 7.0 were observed. This study shows that
more doctors are being in volved, more reductions in target parameters are achieved.
According to the results of this study, when LDL-C limit value is accepted as 100,
theratio of achieving target levels were 26.6% in patients under one doctor’s
supervision, 71.7% in patients followed up by 2 different doctors, 1.4% in patients
under 3 different doctor’ supervision and 7.0% in patients followed up by 4 different
doctors. The ratio of achieving other parameters for the patients followed up by 3 or
4 doctors was also lower than those of the patients followed up by 1 or 2 doctors
