Legionellozis, asemptomatik enfeksiyondan, hayatı tehdit eden ölümcül
hastalıklara varabilen önemli bir halk sağlığı sorunudur ve hastanelerin su
sistemleri primer kaynaklardır. Su sistemlerinde kolonize olan bakteriler,
immündüşkün ve risk faktörleri bulunan duyarlı kişilerde enfeksiyona yol
açabilmekte iken etkenin eradikasyonu ile enfeksiyonlar önlenebilmektedir.
Bu çalışmada hastanelerdeki su sistemleri ile pnömoni tanısıyla izlenen
hastalarda etken olarak Legionella araştırılması amaçlandı. Bu amaçla
hastanelerdeki 100 musluktan toplam 200 su ve sürüntü örneği alınıp,
doğrudan ve asitle muamele sonrasında 400 ekim yapıldı. MWY
besiyerindeki koloni morfolojileri, mikroskobik görünümleri, kanlı agarda
ürememesi ve Legionella lateks aglütinasyon testi ile 23 (%23) musluktan
alınan 29 örnekte Legionella spp. izole edildi. İzolatların 23’ü L.pneumophila
serogrup 2-14, 6’sı serogrup 1 idi. İzolatların 17’si immündüşkün hastaların
bulunduğu ünitelerde üredi. Sürüntü örneklerinde Legionella daha fazla izole
edildi ve anlamlı bulundu. Su örneklerinde direkt ekim ile sürüntü
örneklerinde ise asitle muamele sonrası Legionella daha fazla izole edildi ve
anlamlı bulundu. Ayrıca pnömonili 50 hastada kültürde Legionella aranırken,
idrarlarında L.pneumophila antijeni ve serumlarında L.pneumophila total
antikoru bakıldı. Hastaların hiçbirisinde kültürde üreme olmadı. 50 hastadan
1’inde idrar antijeni pozitif iken, bu hasta dahil 5 (%10) hastada
L.pneumophila serogrup 1-6’ya karşı seropozitiflik saptandı. Bu 5 hastada
lejyonellozis için belirlenen risk faktörleri mevcuttu. Risk faktörlerinin
varlığında pnömoni etiyolojisinde Legionella mutlaka araştırılmalıdır.
Legionella tür ve serogruplarının dağılımına uygun, duyarlılığı ve özgüllüğü
daha yüksek tanı testlerinin geliştirilmesine ve daha fazla çalışma
yapılmasına gereksinim vardır.
Legionellosis is a public health problem
ranging from asymptomatic to life-threatening infections and water
distribution systems of the hospitals are the primary reservoirs. The bacteria
colonizing in the water systems cause infection in immuncompromised or
susceptible persons who have risk factors. We can prevent the infections
with eradicating the causative agent. We aimed to investigate the presence
of Legionella in the etiology of pneumonia and in the water systems of
hospitals. 400 inoculations from 200 water and swab samples were done
directly and after acid treatment from 100 faucets of hospitals. Legionella
spp. were isolated in 29 samples taken from 23 (23%) faucets according to
colony morphology in MWY agar, appearance with microscopy, not growing
on blood agar and using Legionella latex agglutination method. Among
isolates, 23 were L.pneumophila serogroup 2-14 and 6 were serogroup 1. 17
of 29 isolates were from departments where the immuncompromised patients
are hospitalized. Legionella recovery rate in swab samples was higher and
statisticaly significant. Legionella was recovered more frequently when
inoculated directly in water samples and after acid treatment in swab
samples with statistical significance. In 50 patients with pneumonia, culture
was performed for Legionella and Legionella urinary antigen and total
antibody were studied. None of the samples were culture positive. Legionella
urinary antigen was positive in 1 of 50 patients and 5 (10%) patients including
that patient were seropositive for L.pneumophila. Risk factors for legionellosis
were detected in all of 5 patients. Legionella should be researched in the
etiology of pneumonia when risk factors are present. Further studies are
needed to develop more sensitive and specific tests appropriate for the
distribution of Legionella species and serogroups. Key Words: Hospital, Legionella, pneumonia, water
