Bu çalışma Ocak 2010- Nisan 2010 tarihleri arasında elektif cerrahi planlanan
ve genel anestezi uygulanacak 80 hastada yapıldı. Çalışmamızda, anestezi
indüksiyonu sırasında kullanılan 2,5 mg/kg propofol ile oluşan kan basıncı
düşüşünün önlenmesinde farklı dozlardaki ketaminin etkinliklerini karşılaştırmak
amaçlandı. Etik Kurul onayı ve hasta onamı alındıktan sonra operasyon öncesi 8 saat
aç bırakılan hastalara premedikasyon uygulanmadı. ASA I-II grubu 80 olgu rastgele
4 gruba ayrıldı. İndüksiyon öncesinde grup I’e 0.3 mg/kg ketamin, takiben 1 dk sonra
2.5 mg/kg propofol; Grup II’e 0.5 mg/kg ketamin, takiben 1 dk sonra 2.5 mg/kg
propofol; Grup III’e 0.7 mg/kg ketamin, takiben 1 dk sonra 2.5 mg/kg propofol;
Grup IV’e 2 cc SF, takiben 1 dk sonra 2.5 mg/kg propofol verildi. Hastalar
indüksiyondan sonra 0.1 mg/kg dozunda vekuronyum ile gevşetilerek entübe edildi.
Anestezi idamesi % 1-2 isofluron, dakikada 6 lt olacak şekilde %50 O2 ve %50
N2O ile sağlandı. İndüksiyon öncesi, ketamin sonrası, propofol indüksiyonu sonrası,
entübasyon sonrası, operasyonun 3. ,5. ,10. ,15.,30.dk’sında ve operasyon devam
ettiği sürece 15 dk da bir ve ekstübasyon sonrası noninvaziv olarak sistolik arter
basıncı (SAB), diyastolik arter basıncı (DAB),ortalama arter basıncı (OAB), kalp
atım hızı (KAH) ve periferik oksijen saturasyonları (SpO2) kaydedildi. Bütün
gruplarda propofol indüksiyonu sonrası anlamlı düşüş bulundu. Gruplar arası
değerlendirmede; Grup IV olgularında sistolik kan basıncı düşüşü diğer gruplara
göre istatistiksel olarak daha düşük bulundu. Sonuç olarak ketamin;0.3, 0.5 ve 0.7
mg/kg dozunda propofol indüksiyonu sonrasında sistolik kan basıncındaki düşmenin
şiddetini azaltmada etkin olmuş, ancak bu etkinlik tüm hemodinamik parametrelerde
ve cerrahi süreç boyunca devam etmediği için tatminkar bulunmamıştır.
This study was carried out with 80 patients performing elective surgery
between January 2010 and April 2010. In our study, we aimed to determine the
effectiveness of differrent doses of ketamine in prevention of hemodynamic
depression due to propofol injection used in anesthesia induction. No premedication
was performed on the patients who were famished for 8 hours prior to the surgery.
ASA I-II group of 80 patients were randomly divided into four groups. Group I was
applied 0.3 mg/kg of ketamine and 1 min later 2.5 mg/kg propofol, Group II 0.5
mg/kg of ketamine and 1 min later 2.5 mg/kg propofol , Group III 0.7 mg/kg of
ketamine and 1 min later 2.5 mg/kg propofol and Group IV 2 ml 0.9 % SF and 1
min later 2.5 mg/kg propofol . Vecuronium was given 0.1 mg/kg as neuromuscular
blocking agent, and maintance of anesthesia was carried on with 1-2 % of
isofluorane, O2 %50 and N2O %50 . Prior to ketamine induction,after ketamine
induction,after propofol induction, after intubation, noninvasive systolic arterial
pressures (SAP), diastolic arterial pressures (DAP), mean arterial pressures (MAP),
heart beat rates (HBR) and peripheral oxygen saturations (SpO2) at the third ,fifth,
tenth and fifteenth minutes and every fifteen minutes during operation was recorded.
We found that hemodynamic depression due to Propofol induction in Group IV
patients compared to other groups. Ketamine 0,3, 0,5 and 0,7 mg/kg prior to
propofol induction prevents decrease in systolic blood pressure due to propofol. This
effect is not satisfactory in other heamodynamic parameters and does not prevent
heamodynamic changes during opereation period. Key Words: propofol, ketamine,
