Kemoterapötik tedavi alan çocuklarda T ve B
lenfositlerin, nötrofillerin sayı ve fonksiyonlarında azalmayla birlikte immün sistem
fonksiyonlarında bozulma oluşmaktadır. Çalışmamızda tedavinin değişik
aşamalarındaki akut lenfoblastik lösemili çocuklarda immün sistem değişikliklerini
ve alerjik hastalık sıklığını irdelemeyi amaçladık. ALL tanısı konup, remisyon
indüksiyon ve konsolidasyon tedavisi yapılıp idame tedavisine geçilen 21 hasta ve
idame tedavisi biten 29 hasta çalışmaya alındı. Kontrol grubu olarak da aynı yaş ve
cinsiyette, enfeksiyonu olmayan 39 sağlıklı çocuk çalışmada değerlendirildi.
Olgularda beyaz küre, absolü nötrofil, absolü lenfosit sayıları, lenfosit alt grupları
(CD3+CD4+
, CD3+CD8+
, CD19+
, CD16+
56+ ve TH/TS oranı) ve Ig düzeyleri (Ig G,
Ig M ve Ig A) çalışıldı. Alerjik hastalık sıklığını değerlendirmek için ise her hastaya
anket formu dolduruldu ve absolü eozinofil sayısı, total Ig E, spesifik Ig E alerjenleri
ve phadiatop testi çalışıldı. Çalışmamızda, idame tedavisi alan çocuklarda beyaz
küre, absolü nötrofil ve lenfosit sayısının azaldığı, CD3+CD4+
, CD3+CD8+
,CD19+
ve
CD16+
56+ sayısının düşük olduğu, Th/Ts oranının azaldığı, Ig G, Ig A ve Ig M
düzeylerinin anlamlı olarak düştüğü saptandı. Đdame tedavisi bittikten sonra immün
sistemin yeniden yapılandığı, beyaz küre, absolü nötrofil ve lenfosit sayısı,
CD3+CD4+
, CD3+CD8+
, CD19+
ve CD16+
56+ sayıları ve Ig G, Ig A ve Ig M
değerleri tedavi yoğunluğuna bağlı olarak zamanla artarak normal düzeye ulaştığı
gözlendi. ALL’li hastalarda alerjik hastalık semptomu olabilecek bulgularda kontrol
grubuna göre artış olduğu saptandı. Absolü eozinofil sayısı, total Ig E ve spesifik Ig
E düzeyleri ve phadiotop sonuçları incelendiğinde hasta ve kontrol grupları arasında
istatistiksel fark bulunmadı. Sonuç olarak; lösemili hastalarda tedavi sürecinde
immünsupresyon oluşmakta, idame tedavisi bittikten sonra immün sistem yeniden
yapılanmaktadır. Ayrıca lösemili hastalarda alerjik semptomlarda artış
gözlenmektedir.
Chemotherapy decrease the number
and functions of T, B lymphocytes and neutrophiles and cause dysfunction in the
immun system of children. The aim of our study is to rule out the changes in immune
system and the insidance of allergic disease in children with acute lymphoblastic
leukemia (ALL) in different stages of the chemotherapy. We involved 21 children
who was having maintenance theraphy, and 29 children who had finished
maintenance theraphy after remission, induction and consoludation theraphy for ALL
in the study group. We involved 39 healthy children with the same age and gender in
the control group. We searched white blood cells, absolute number of neutrophiles
and lymphocytes, subgroups of lymphocytes (CD3+CD4+
, CD3+CD8+
, CD19+
,
CD16+
56+ and TH/TS ) and level of immunglobulins (Ig G, Ig M, Ig A) in both
study and control groups. Survey forms were filled by every child for the insidance
of allergic disease. Absolute eosinophil number, total IgE and spesific IgE allergens
and phadiatop test were searched for every child. In our study, we have found out
the decrease in white blood cells, the number of absolute neutrophiles and
lymphocytes, the level of subgroups of lymphocytes and the level of Ig G, Ig A and
Ig M in children who had maintenance theraphy. After maintenance theraphy has
finished , immun system reconstituded, WBC, number of absolute neutrophile, and
lymphocyte, subgroups of lymphocytes, Ig G, Ig A ve Ig M levels increased with
time and reached to normal levels In the children with ALL, the symptoms according
to the allergic disease increased in number, when compared with the control group.
When we searched for the number of absolute eosinophile, the level of total IgE and
specific IgE and the results of phadiotop, we found out no statistical difference
between the children with ALL and the control group. In conclusion,
immunsupression occured in ALL children during the chemotherapy and immun
system reconstitued again after maintenance therapy has finished. In addition to that,
allergic symptoms increased in children with ALL.
