elektif kulak burun boğaz cerrahi girişim planlanan genel anestezi verilecek ASA I II, 18-65 yaş arası 40 erişkin hasta alındı. Hastalar rastgele, Grup I
(premedikasyonsuz Düşük Akımlı SevofluranAnestezi (DASA) Grubu) ve Grup II (
deksmedetomidinle premedikasyonlu DASA Grubu) olarak 20’şer kişilik iki gruba
ayrıldı. Hastalara operasyondan 45 dakika önce premedikasyon amacıyla deltoid
kastan intramuskuler olarak Grup I’e 2cc volümde serum fizyolojik, Grup II’e ise 2,5
μg/kg deksmedetomidin uygulandı. Her iki gruptaki hastaların premedikasyondan
önce ve premedikasyondan 45 dk sonra olmak üzere, Ramsey Sedasyon Skalası ve
hemodinamik verileri kaydedildi. Grup I ve Grup II 10 L/dk %100 O2 ile 2 dk
preoksijenizasyon sonrası, intravenöz yoldan sırası ile 5-7 mg/kg pentotal, kas
gevşemesi için 0,1 mg/kg veküronyum ve 1 μg/kg fentanil uygulanması sonrasında 3
dk beklenerek endotrakeal entübasyon uygulandı. Entübasyonu takiben her iki gruba
usulüne uygun olarak düşük akım anestezi tekniği uygulandı. İndüksiyondan önce ve
sonra, entübasyondan sonraki 1, 15, 30, 45 ve 60. dk’.larda hemodinamik veriler, Et
Sev, EtCO2, FiO2, FetO2 değerleri kaydedildi. Operasyonun bitimine 15 dakika kala
normal akıma geçildi. Operasyonun bitiminde her iki grupta da anestezikler kesilip,
taze gaz akışı 6 L.dk-1%100 O2’e çıkartılarak hastalar ekstübe edildi.
Ekstübasyondan önce ve sonra hemodinamik veriler kaydedildi. Ekstübasyon
zamanı, göz açma, sözel emirlere yanıt, hava yolu hassasiyeti, kirpik refleksi,
öksürük refleksi süreleri de kaydedilerek hastalar uyanma odasına gönderildi. Grup
II’deki olguların premedikasyondan 45 dakika sonra RSS değerleri, Grup I’deki
olgulara göre anlamlı olarak yüksek bulundu. Grup II’nin preoperatif, indüksiyon
sonrası ve entübasyon sonrası 1. Dk ve ekstübasyon sonrası SKB, DKB ve OKB
değerleri Grup I’den daha düşüktü. Sonuç olarak çalışmamızda ekonomik ve ekolojik
açıdan sağladığı avantajlardan faydalanmak adına, çözünürlüğü ve anestezik gücü
düşük olan sevofluranı düşük akım anestezi tekniği ile kullandık. Deksmedetomidin
ile premedikasyon uygulanan düşük akım anestezi grubunda özellikle indüksiyon ve
ekstübasyon döneminde daha stabil bir hemodinami olduğunu gördük.
Totally 40 patients aged 18-65 years old ASA I-II in which planned elective
otorhinolaryngology surgery under general anaesthesia were included the study. The patients
were divided randomly 2 groups equally as Group I (Low flow sevoflurane anaesthesia
(LFSA) without premedication) and Group II (LFSA with dexmedetomidine as
premedication). Group I received 2 cc saline and Group II received 2,5 μg/kg
dexmedetomidine for premedication as intramusculer in deltoid muscle 45 min before
surgery. Ramsey Sedation Score and hemodynamic parameters were recorded before
and after 45 min the premedication in both groups. Both groups were
preoxygenationed with 10 L/min 100% O2 during 2 min and received 5-7 mg/kg
pentothal and 1 μg/kg fentanyl as intravenous. Endotracheal intubation was
performed after 3 min following 0,1 mg/kg vecuronium and for muscle relaxation.
Then, low flow anaesthesia was formally introduced. Hemodynamic parameters, Et
Sev, EtCO2, FiO2 and FetO2 values were recorded before and after induction and 1,
15, 30, 45 and 60 min after intubation. Normal flow was taken place 15 min to the
end of the surgery. At the end of the surgery, anaesthetics were interrupted and
patients extubated maintaining the fresh gas flow to 6 L.min-1100% O2.
Hemodinamic parameters were recorded after and before the extubation. The
extubation time, eye opening, response to verbal commands, sensitization of airway,
eyelash reflex, cough reflex durations were recorded and the patients were taken to
the recovery room. Ramsey Sedation Score values 45 min after premedication were
significantly higher in Group II compared to Group I (p<0,01). Preoperative, after
induction, 1 min after intubation and after extubation, SBP, DBP and MBP values of
Group II were found lower than those in Group I. As a conclusion, for profit the
advantages of economical and ecological, we used sevoflurane which has low
solubility and anesthesic potency by low flow anaesthesia technique. More stabile
hemodynamia was observed in low flow anaesthesia group introduced premedication
with dexmedetomidine, especially during induction and extubation periods.
