Kronik böbrek hastalarında tanı yöntemlerinin gelişmesi ile erken tanının daha sık konulması ve diyaliz tedavisindeki gelişmeler bu hastalarda ortalama yaşam beklentisinin daha uzun olmasını sağlamıştır. Bunun sonucunda diyaliz hastalarında kalça ve vertebra kırıkları ile ilişkili yaşam kalitesini etkileyen çeşitli sorunlar daha sık yaşanmaktadır. Diyaliz hastalarında osteoporotik kırık insidansının genel populasyona göre çok daha yüksek olması sonucu gelişen morbidite ve mortalite artışını azaltmak için, kırık riskinin önceden belirlemesi ve önlemler alınması gereklidir. Çalışmaya yaş ortalaması 56.7±12.5 olan 145 kronik böbrek hastası ve 52.5±82 olan 45 kontrol hastası alınmıştır. Hastaların demogrofik özelliklerinin kaydedilmesi ve biyokimyasal tetkiklerinin çalışılmasının ardından DEXA ile lomber vertebra, femur boynu, proximal radius, mid-radius ve distal radius bölgelerinden KMD ölçümü yapılmıştır. Hasta grubunda PTH ile femur boynu, mid-radius ve distal radius T skor ölçümleri arasında negatif korelasyon bulunmuş (sırasıyla r=-0.248, p<0.05; r=0.177, p<0.05; r=-0.264, p<0.05), lomber vertebra ve proximal radius ölçümleri arasında ilişki bulunmamıştır. Ayrıca evre 1-5 KBH ve kontrol grubu arasında KMD ölçümleri karşılaştırıldığında, gruplar arasında sadece distal radius T ve Z skor açısından farklılık olduğu görülmüştür. Evre 5 KBH ında distal radius T skoru 2.08±1.41 olup evre 1 ve 3 KBH ından ve kontrol grubundan düşüktür, distal radius Z skoru -1.24±1.22 olup evre 3 KBH ve kontrol grubundan düşüktür. KBH seyrinde gelişen sekonder hiperparatiroidizm kortikal kemik dansitesinde azalma ve trabeküler kemikte artış meydana getirmektedir. Bu nedenle KBH da DEXA ile KMD değerlendirilmesinde distal radius ölçümlerinin alınması kemik yoğunluğunun değerlendirilmesinde daha güvenilir bir yöntem olarak durmaktadır. Böylece osteoporoza yönelik gerekli önlemlerin alınması ile kırık insidansında oluşacak azalma, hemodializ hastalarında kırığa bağlı oluşan morbidite ve mortalitenin azalmasını sağlayacaktır.

The incidence of osteoporotic fractures in dialysis patients is much higher than the general population caused by the increase in morbidity and mortality, to reduce fracture risk and identify measures to be taken in advance. This study includes 145 patients with chronic kidney disease at the mean age of 56.7±12.5 and 45 control group at the mean age of 52.5±82. Developing in the course of secondary hyperparathyroidism in CKD gives rise reduction in cortical bone density and increase trabecular bone. Therefore, the evaluation of distal radius BMD measurements by DEXA in CKD to be a more reliable method of assessing bone density remains. Thus, taking the necessary measures for osteoporosis will occur with a reduction in the incidence of fractures, fracture-induced morbidity and mortality in hemodialysis patients would be decreased.
