Trombositten zengin plazma (TZP) trombosit içeriği normal kan seviyesinden daha yüksek olan ve özel tekniklerle hazırlanan otolog plazma olup, pek çok biyoaktif maddeyi yoğun şekilde içerir. Özellikle yara iyileşmesi üzerine etkileri iyi bilinen TZP’nın, bu çalışmada sinir rejenerasyonu üzerine etkisinin araştırılması planlandı. 40 adet Wistar cinsi sıçan 4 gruba ayrıldı. Uygun anestezi altında siyatik sinirde 54 newton sabit basınç uygulayan klemp ile ezilme tarzı hasar oluşturuldu. Grup 1’de (n=10), sadece sinir diseksiyonu yapıldı, hasar oluşturulmadı. Grup 2’de (n=10), sinir diseksiyonunu takiben ezilme hasarı yapılıp, ezilme zonuna bir işlem yapılmadı. Grup 3’te (n=10), sinir diseksiyonunu takiben ezilme hasarı yapılıp, ezilme zonuna TZP uygulandı. Grup 4’te (n=10), sinir diseksiyonunu takiben ezilme hasarı yapılıp, ezilme zonuna trombositten fakir plazma uygulandı. TZP ve TZP’yı aktive etmek için kullanılan trombin solusyonu FDA onaylı Harvest SmartPReP–2 cihazı, otolog trombin kiti ve TZP işlem kiti kullanılarak hazırlandı. Sinir rejenerasyonunu değerlendirmek için, deney sonrası 7, 14, 21 ve 30. günlerde siyatik fonksiyonel indeks (SFİ) ve ayak geri çekme refleksleri (WRL) ölçüldü. Deney sonrası 30. günde anestezi altında elektrofizyolojik testler yapılıp aksiyon potansiyeli eğrilerinde latans değerleri, tepetepe amplitüdü ve maksimal pozitif yöndeki pik değerleri ölçüldü. Histomorfometrik olarak hasar öncesi ve sonrası bölgelerde ortalama sinir lifi çapı ve dejenere sinir lifi yüzdesi ile birim alanda bulunan toplam Schwann hücre çekirdek sayısı ve dejenere Schwann hücre çekirdek sayısı değerlendirildi. Verilerin istatistiksel analizi ile TZP uygulanmış olan grup 3’te SFİ ve WRL değerlerinin daha hızlı düzeldiği ve bu düzelmenin elektrofizyolojik ve histomorfometrik bulgularla uyum içinde olduğu saptandı. Sonuç olarak ezilme tarzı sinir hasarında, diğer gruplara göre, tek doz uygulanan TZP ile daha hızlı ve kaliteli bir rejenerasyon elde edildi.
OZBAYOGLU A.C. The Effects of Platelet Rich Plasma on Sciatic Nerve Regeneration in Experimental Rat Crushed Sciatic Nerve Injury Model. Platelet rich plasma (PRP) is an autologous plasma that contains higher platelet levels than normal blood. Due to the high platelet concantration it contains high levels of bioactive molecules. In this study it is planned to show the effects of PRP on nerve regeneration. Forty Wistar rats divided into 4 groups. Under general anesthesia sciatic nerve crush injury was performed with a clamp which had a constant pressure of 54 newton. Groups were planned as follows: In group I (n=10) only nerve dissection was performed and no crush injury was produced; in group 2 (n=10) after nerve dissection only crush injury was performed and no other treatment was given; in group 3 (n=10) after the dissection of nevre, crush injury was performed and PR-P was given over the crush zone; in group 4 (n=10) after the dissection of nevre, crush injury was performed and platelet poor plasma (PPP) was given over the crush zone. PR-P and the thrombin solution (which was used to activate the PRP) were prepared by Harvest SmartPReP system, which has FDA permission. Before the beginning of experiment and after 7th, 14th, 21 th and 30th days sciatic functional index (SFÎ) and withdrawal refleks (WRL) were measured for each group. At 30th day under general anesthesia electrophysiologic tests were performed. Latency, peak to peak amplitude and maximum positive peaks were measured. For histomorphometric analysis, samples were taken before and after the crushed segment seperately, and nerve fiber diameter, mean degenerated nerve fiber percentage, Schwann cell nucleus number and Schwann cell degenerated nucleus number percentages were calculated. After the statistical analyses of the data, it is found out that in group 3 by using PPO, the SFI and WRL values were better than the other groups and this regenerative data were supported by electrophysiologic and histomorphometric data. In conclusion, single application of PRP can improve nerve healing in rats due to the growth factors that PR-P contains.
