Çalışmanın amacı sensörinöral işitme kaybı olan ve Kulak Burun boğaz(KBB) servisi tarafından temporal kemik BT ve MRG tetkikleri istenen hastalarda iç kulak oluşumlarını ve malformasyonlarını değerlendirmek, MRG ile internal akustik kanal(İAK) içerisindeki sinirlerin varlığını saptamak ve kohlear implantasyon için kontrendike olabilecek durumları belirlemekti. Şubat 2006-Nisan 2008 tarihleri arasında KBB bölümü tarafından konjenital sensorinöral işitme kaybı (SNİK) ön tanısıyla refere edilen 60’ı erkek, 33’ü bayan toplam 93 hastaya 64 dedektörlü BT ve 1,5 tesla MRG çalışıldı. Toplam 186 kulak çalışmaya alındı. 2
olguda tek tarafta ortak kavite malformasyonu, 1 olguda her iki tarafta incomplet
partition-I(IP-I), 5 olguda her iki tarafta Mondini malformasyonu(incomplet
partition-I, IP-II), 1 olguda her iki tarafta X-linked malformasyon, 3 olguda her iki
tarafta lateral semisirküler kanal-vestibül displazisi(LSVD) saptandı. Ayrıca 3 olguda her iki tarafta vestibüler akuadakt(VA) genişliği ve bu olgulardan birinde her iki tarafta geniş endolenfatik kese izlendi. 11 olguda bilateral lateral semisirküler kanal(LSSK) hipoplazikti. LSSK hipoplazisi çalışmamızda en sık saptanan anomalidir. 2 hasta 3 kulakta ise LSSK aplazik izlendi. 2 olgunun her iki kulağında internal akustik kanal(İAK) içerisinde kalın 8.sinire ait olabilecek bir yapı görüldü. Ayrıca fasiyal siniri mevcuttu. 1 olguda her iki tarafta İAK’da tek bir sinir izlendi. Bu olguda kohlear sinir hipoplazik olarak değerlendirildi. Radyolojik değerlendirmeler sonucunda; olgu popülasyonunda iç kulak anomalileri tanımlandı, kohlear sinir varlığı ve kohlear implantasyon açısından kontrendike olabilecek durumlar belirtildi. Sonuç olarak; kohlear implantasyon öncesi iç kulağın detaylı
bilgilendirilmesi için mutlaka çok dedektörlü temporal kemik BT ile malformasyonların değerlendirilmesi ve ek olarak İAK içerisindeki sinir yapıların ve
patolojilerin ortaya konabilmesi için temporal kemik MRG’nin kombine edilmesi
gerekmektedir.
The aim of the study is to evaluate the patients, having
sensorineural hearing lose who are requested temporal bone CT and MRI by the Ear
Nose-Throat branch for their inner ear formation and malformations, to define the
exsistanse of nerve in the internal acoustical canal (IAC) with MRI and determine the
cochlear implantation for controdict occurances. Between May 2006 and April 2008.
Totaly 93 patients, 60 of men, 33 of women who are refered by the Ear-Noise-Throat
service due to congenital sensorineural hearing lose (SNHL) are studied by 64 slice
dedector CT and MRI on them. Totaly 186 ears were taken under evaluation in 2
cases, on-side common cavite malformation, in 1 case in both sides incomplet
partitition-I (IP-I), in 5 cases in both sides Mondini malformation (incomplete
partitition II, IP-II), in 1 case X-linked malformation on both sides, in 3 cases on
both sides lateral semisircular canal-vestibular dysplasia are diagnozed. Besides, in 3
cases on both sides vestibular aqueduct (VA) widening and one of these 3 cases on
both sides wide endolymphatic sac is observed. In 11 cases bilateral lateral
semicircular canal (LSSC) was observed as hypoplasic. LSSC aplasic was observed. İn 2 patients on 3 ears LSSC aplasic was observed. In 2 cases on both ears in the
internal acoustic canal a structure may belong to 8.th nevre is observed. Also there is
a facial nerve. In 1 case on both sides, only one nerve was observed in (IAC). In that
case cochlear nerve is determinated as hypoplasic. As a result radiologic evaluation;
inner ear anomaly was defined in cases, exsistance of cochlear nerve and same cases
which can be controdict is determined. As a result, before cochlear implantation, it is
neccessary that inner ear must be evaluated by the multislice temporal bone CT for
malformation evaluaiton and also it’s essential to combine temporal bone
MRI to deterninate the patology for structure of the nerve inside the nerve inside the IAC.
