Bu ĢalıĢmanın amacı akut koroner sendrom tanısıyla yatırılan hastalarda görülen aritmilerin sıklığını saptamaktır. ÇalĢımaya hastanemize baĢvurusunda akut koroner sendrom tanısıyla koroner yoğun bakım ünitesine kabul edilen 700 hasta tarandı, bu hastaların içinden ilk 48 saat içinde aritmi geliĢen 155 hasta dahil edildi. ÇalıĢmaya alınan hastalara çalıĢma hakkında bilgi verilerek onam formları alındı. Gebeler, malignite tanısı alanlar ve koroner anjiyografi yapılmayan hastalar çalımaya alınmadı. Akut koroner sendrom ve akut miyokard infarktüsü tanısıyla koroner yoğun bakım ünitesinde yatırılan hastaları Nihon kohden BSM 2301 cihaziyla 48 saat EKG kayıtları izlendi. ÇalıĢmaya dahil edilen hastaların ortalama yaĢı 65.6±12.8 idi. ÇalıĢmaya alınan hastaların %70.3‟ü erkek geri kalan %29.7‟si kadın idi. Cinsiyete göre incelendiğinde ise kadın hastaların yaĢ ortalamasının erkeklerden belirgin olarak daha yüksek olduğu görüldü (74.1±11.5 karĢılık 61.9±11.5, p<0.001). ÇalıĢmaya dahil edilen hastalardan STEMĠ tanısı alan hastaların 50 (%46.7)‟sinde, NSTEMĠ tanısı alanların 10 (%23.8)‟unda VEV görüldü, VEV STEMĠ‟larda NSTEMĠ‟lara göre anlamlı oranda daha fazla görüldü (%46.7 karĢılık %23.8, p<0.05). Hastalardan STEMĠ tanısı alanların 3(%2.8)‟ünde ve NSTEMĠ tanısı alanların 8(%19)‟inde atriyal flatter gözlendi. Atriyal flatter NSTEMĠ‟larda STEMĠ‟lara göre anlamlı derece daha yükseti ( %19‟a karĢılık %2.8, p<0.05). cinsiyete göre incelendiğinde diğer aritmilerde cinsiyetler arası anlamlı farklılık görülmezken AF erkeklere göre kadınlarda anlamlı düzeyde daha fazlaydı(%32.6 karĢılık %18.3). Ventriküler fibrilasyon yaĢayanlara göre ölenlerde anlamlı derecede daha fazla ( %31.8 karĢılık %9.0, p<0.05) idi. Üçüncü derece AV blok da ölenlerde yaĢayanlara göre anlamlı derece de daha fazla idi ( % 36.6 karĢılık %11.3, p< 0.05).
The aim of this study is to determine the frequency of arrhythmias in patients who have been diagnosed with Acute Coronary Syndrome. 700 patients who were admitted to the coronary care unit of our hospital, diagnosed with Acute Coronary Syndrome, were all screened, of these patients 155 of them developed arrhythmias within the first 48 hours and they were included in study. The patients enrolled in the study were given information regarding the study and consent forms were taken as well. Pregnant women, patients diagnosed with malignancy and undone coronary angiography patients were not scheduled for the study. With Nihon Kohden BSM 2301 device, the 48 hours ECG records of patients, who were diagnosed with Acute Coronary Syndrome and Acute Miyocardial infarction and they were sleeping in intensive care unit, were observed. The average age of the patients who participated in the study was 65.6±12.8. Patients included in the study, %70.3 of them were male and the remaining, %29.7, were female. When analyzed by gender, the average age of female patients were found significantly higher than the men (74.1 ± 11.5, versus 61.9 ± 11.5, p <0.001). VEV was seen in 50 (%46.7) of patients who attended the study and were diagnosed with STEMI and 10 (%23.8) of NSTEMI patients. VEV was seen considerably higher in STEMI patients that in NSTEMI patients. Atrial Flutter was seen in 3(%2.8) of the patients diagnosed with STEMI and 8(%19) of those diagnosed with NSTEMI. Atrial Flutter was remarkably higher in NSTEMI patient that in STEMI pateits (19% versus 2.8%, p <0.05). The survival rate of devoloping arrhythmias was analyzed, considering the statistics this the rate of the death was significantly high in patients with Ventricular Fibrilation ( %31.8 versus %9.0, p<0.05). Patients died as a result of third-degree AV block was much higher than the living ones ( % 36.6 versus %11.3, p< 0.05).
