Metoklopramid’ in AS’ ye basvuran renal kolikli hastalarda bulantı, kusma ve ağrı
semptomlarını gidermeye yönelik klinik etkinliğinin arastırılması amacı ile,
21.04.2008- 25.02.2009 tarihleri arasında klinik olarak renal kolik ön tanısı alan 240
(128 erkek, 112 kadın, ortalama yas 36.76) hasta ile yapılmıstır. Çalısmanın gücü 1-
β =0.80 olarak alınmıstır. Üç ilaç grubu; 1.grup 2 mL NSAĐĐ (20 mg Tenoksikam IV)
+ 100 mL SF, 2.grup 2 mL NSAĐĐ (20 mg Tenoksikam IV) + 100 mL SF içinde 10
mg Metoklopramid, 3.grup 2mL SF + 100 mL SF içinde 10 mg Metoklopramid
olacak sekilde hastalara kura usulü ile randomize edilmistir. Tedavi uygulaması çift
kör yapılmıstır. Hastalarda 0, 10, 20 ve 30. dakikalarda sorgulanan bulantı ve ağrı
düzeyleri için VAS, kusma için var- yok tanımlamaları kullanılmıstır. 30 dakika
sonunda üç grup arasındaki bulantı, kusma ve ağrı değisiklikleri ile ek tedavi
gereksinimleri karsılastırılmıstır. Gruplar arasındaki fark değerlendirmesi; varyans
analiz, ki- kare testi, bağımsız örneklerde t testi, Kolmogorov- Smirnov testi, bağımlı
örneklerde t testi (paired t testi), Mc Nemar χ2 testi, Wilcoxon Signed Ranks Test
kullanılarak yapılmıstır. Gruplar arasında tanımlayıcı veriler ile 0. dakika ağrı ve
bulantı düzeyleri arasında fark saptanmamıstır. Çalısmada 10, 20, 30. dakikalarda
ağrı ve bulantı düzeyleri ile kusmada anlamlı azalma saptanmıstır. 0 ile 30. dakika
arası ağrı düzeyi farkı ortalamaları 1. grup: 36.11±34.75 mm, 2. grup: 45.42±30.63
mm, 3. grup: 37.95±32.39 mm olarak bulunmustur (p = NS). 30. dk sonunda ek ağrı
ve bulantı giderici gereksinimi varlığı için bakıldığında gruplar arasında fark anlamlı
olarak değerlendirilmistir. Veriler Đleri X² analizi ile de test edilerek farklılığın 1.
gruptan kaynaklandığı, bu grupta daha fazla ek ağrı ve bulantı giderici ihtiyacı
olduğu belirlenmistir. 2. ve 3. grup arasında ise anlamlı fark saptanmamıstır. Sonuç
olarak bulantı gidermek amacı ile kullanılmakta olan Metoklopramid’ in renal kolikli
hastalarda analjezik etkinliğinin de en az NSAĐĐ’ ler kadar olduğunu ve analjezik
etkisi için de güvenle kullanılabileceğini düsünmekteyiz.
In this study, the efficacy of Metoclopramide (MTC) is evaluated
for the treatment of nausea, vomiting and pain in the patients who came to
Emergency Department with renal colic. The study was conducted between
04.21.2008 and 02.25.2009 with 240 patients (128 male, 112 female, average age of
36.76 ). The power of study was 1- β =0.80. Three groups of medicine were given to
the patients with random selection method: in the 1st group 2 mL (20 mg) Tenoxicam
IV + 100 mL Normal Salin (NS), in the 2nd group 2 mL (20 mg) Tenoxicam IV + 10
mg MTC in 100 mL NS and in the 3rd group 2 mL NS + 10 mg MTC in 100 mL NS
was given. Double blind method was used for treatment. The levels of nausea and
pain were interrogated with VAS and vomiting was ‘existing-not existing’ at the 0,
10th, 20th and 30th minutes (min). At the end of 30 min the differences in nausea,
vomiting, pain and the necessity of additional treatment were compared in three
groups. The analysis of differences within the groups: Variation analysis, chi-squared
test, t test for independent samples and Kolmogorov- Smirnov test, t test for
dependent samples, Mc Nemar χ2 test and Wilcoxon Signed Ranks Test. There was
no differences found between baseline characteristics and the levels of pain and
nausea at 0 min. A significant decrease in the levels of pain, nausea and vomiting
was determined at the 10th, 20th and 30th min. The average level of pain differentiated
between 0 and 30th min was assessed as 11±34.75 mm for the 1st group, 45.42±30.63
mm for the 2nd group and 37.95±32.39 mm for the 3rd group (p = NS). In terms of the
necessity for additional treatment of nausea and pain, the difference among the
groups was evaluated as significant. 1st group was detected as the source of the
difference and the necessity for additional treatment was stated for the same group.
There was no significant difference between 2nd and 3rd group. In conclusion, MTC
which is used for the treatment of nausea, has an analgesic affect in patients with
renal colic as effective as those with NSAII and it can also be used securely.
