Metabolik Sendrom dünyada ve Türkiye’de giderek daha fazla sayıda insanı
etkileyen önemli bir morbidite nedenidir. Metabolik sendrom, birden fazla
kardiyovasküler risk faktörünün kümelendiği hastalıklar grubudur ve metabolik
sendromun başlıca bileşenleri hiperglisemi, hipertansiyon, viseral obezite,
hipertrigliseridemi ve düşük HDL kolesterol olarak sıralanır. İnflamasyon ve
Oksidatif stres; ateroskleroz, diyabet, hipertrigliseridemi, obezite gibi metabolik
sendromda tanımlanan risk faktörlerinin patogenezinde önemli bir rol oynamaktadır.
Biz bu tez çalısmasında oksidatif hasarın ve inflamasyonun metabolik sendrom
patogenezindeki olası rolünü incelemeyi amaçladık. Çalışma grubu olarak G1: Tip II
diyabeti olan metabolik sendromlu hastalar (n=20), G2: Diyabeti olmayan metabolik
sendromlu hastalar (n=13), G3: Tip II Diyabet hastaları (n=19), G4: Kontrol grubu:
20 sağlıklı erişkin seçildi. Hasta ve kontrol grubu serumlarında glukoz, HDL-K,
LDL-K, TG, malondialdehit , nitrik oksit, hs-CRP ve plazma fibrinojen düzeyleri
ölçüldü. Hasta gruplarında malondialdehit,fibrinojen ve hs-CRP düzeyleri kontrol
grubuna göre anlamlı düzeyde yüksek bulundu. Hasta grubu serum NO düzeyleri ise
kontrol grubuna göre anlamlı derecede düşüktü. Fibrinojen düzeyleri kontrol
grubuyla karşılaştırıldığında hasta grubunda anlamlı düzeyde yüksekti fakat hasta
grupları arasında anlamlı fark bulunmadı. hs-CRP düzeyi kontrol grubunda düşük,
hasta gruplarında yüksek bulundu, en yüksek değerler G2’de tespit edilirken risk
sınıflaması yapıldığında orta ve yüksek riskli hastalar G3’te yoğunlaştı . NO düzeyi
kontrol grubunda daha yüksek bulunurken MDA düzeyleri ise kontrol grubunda
düşük bulundu. Bu sonuçlar metabolik sendrom patogenezinde inflamasyon ve
oksidatif stres ile lipid.peroksidasyonunun rolü olabileceğini ve buna bağlı olarak
NO düzeylerinde de anlamlı değişiklikler oluştuğunu ve aynı zamanda koagülasyon
eğiliminde artış nedeniyle fibrinojen düzeylerinin de arttığını düşündürmektedir.
The metabolic syndrome, which affects an increasing number of
people worldwide as well as in Turkey, is a major cause of death. The metabolic
syndrome comprises a cluster of cardiovascular risk factors, among which
hyperglycemia, hypertension, visceral obesity, hypertriglyceridemia, and low HDL
cholesterol are primary. Inflammation and oxidative stress have major roles in the
pathogenesis of risk factors that define the metabolic syndrome such as
atherosclerosis, diabetes, hypertriglyceridemia, and obesity. In this study, we aim to
explain the potential role of the oxidative damage and inflammation in the
pathogenesis of the metabolic syndrome. The experimental groups are G1: Consists
of patients with diabetes and with the metabolic syndrome (n=20), G2: Consists of
patients without diabetes but with the metabolic syndrome (n=13), G3: Consists of
patients with type 2 diabetes (n=19); and the control group consists of 20 healthy
adults. We measured the levels of glucoses, HDL-C, LDL-C, TG, malondialdehyde,
nitric oxide (NO), hs-CRP in the serum, and plasma fibrinogen of both the
experimental and the control groups. We find that the levels of malondialdehyde,
fibrinogen, and hs-CRP in the experimental groups are significantly higher than
those in the control group. On the other hand, the NO levels in the experimental
groups are significantly lower than those in the control group. The levels of
fibrinogen in the experimental groups are significantly higher than those in the
control group however, there is not a significant difference in the levels of fibrinogen
among the experimental groups. We find that the levels of hs-CRP are low in the
control group and high in the experimental groups; while G2 has the highest levels,
after the risk order, the patients with high and medium risk are concentrated in G3.
We find high levels of NO, and low levels of MDA in the control group. Under the
light of these findings, we believe that the oxidative stress, inflammation, and lipid
peroxidation have a role in the pathogenesis of the metabolic syndrome.
Consequently, the level of NO changes significantly. Meanwhile, because of the
increase in the coagulation tendency, the level of fibrinogen increases as well.
