Nefrotik sendromda tromboembolik olaylar sık görülür. Bu durum
nefrotik sendroma bağlı edinsel hiperkoagülabilite ile ilişkili olabilir. Bununla
birlikte herediter trombofilinin de eşlik etmesi, özellikle de kombine herediter
trombofilinin bulunması trombotik olayların gelişiminde rol oynar. Renal ven
trombozu nefrotik sendromda sık karşılaşılan bir tromboembolik hadisedir.
Çalışmaya yeni tanı almış 51 idiyopatik nefrotik sendromlu hasta ve yaş, cinsiyet
uyumlu 20 sağlıklı kontrol grubu dahil edilmiştir. Hasta grubu ve kontrol grubu
biyokimyasal özellikler, trombofili ile ilişkili koagülasyon testleri ve herediter
trombofili ile ilişkili genetik mutasyonlar olan Faktör V Leiden mutasyonu,
protrombin gen mutasyonu ve metilen tetrahidrofolat redüktaz gen mutasyonu
açısından karşılaştırmalı olarak değerlendirilmiştir. Hasta grubu ve kontrol grubunda
subklinik veya klinik renal ven trombozu sıklığını araştırmak için renal ven doppler
ultrasonografisi yapılmıştır. 51 hastadan 6’sında (% 11.8) (4’ü semptomatik, 2’si
subklinik olmak üzere) tanı anında tromboembolik komplikasyon saptanmıştır, RVT
insidansı % 3.9 bulunmuştur. Tromboembolik komplikasyon izlenen hastaların
tamamında herediter trombofiliyi destekleyen genetik mutasyonlar saptanmıştır.
Semptomatik olan 4 hastanın 3’ünde ve asemptomatik olan 2 hastanın 1’inde birden
fazla genetik mutasyon bulunmuştur. Tromboembolik komplikasyon gelişen ve
gelişmeyen hastalar arasında protein C aktivitesi, serbest protein S düzeyi,
antitrombin III düzeyi, plazma fibrinojen, serum albümin, lipid profili ve 24 saatlik
idrarda protein atılımları bakımından farklılık tespit edilmemiştir. Nefrotik
sendromda edinsel faktörlerin yanı sıra kalıtsal trombofili de tromboembolik
olayların ortaya çıkışında rol oynar, trombüsün nedeni edinsel faktörlerle
açıklanamadığında kalıtsal trombofili risk faktörlerinin araştırılması faydalı olacaktır.
Thromboembolic phenomena are
common in nephrotic syndrome. This situation may be associated with the acquired
hypercoagulability due to nephrotic syndrome. The coexistence of inherited
thrombophilia, especially the presence of combined hereditary thrombophilia, plays
an important role in thrombotic events. Renal vein thrombosis is also common in
nephrotic syndrome. In this study newly diagnosed 51 patients with idiopathic
nephrotic syndrome and 20 healthy individuals as control group were included.
Patients and control group were compared in terms of biochemical features, the
coagulation tests related with thrombophilia, and the genetic mutations related with
hereditary thrombophilia such as Factor V Leiden, prothrombin gene mutation and
methylen tetrahydrofolate reductase gene mutation. To search for clinical or
subclinical renal vein thrombosis, doppler ultrasound was performed in both patients
and control group. In 6 of 51 patients (% 11.8) (4 symptomatic and 2 subclinical)
thromboembolic complications were present at the time of diagnosis of nephrotic
syndrome. The incidence of RVT was % 3.9. In all patients with thromboembolic
complications, the genetic mutations of hereditary thrombophilia were present. In 3
of 4 patients with symptomatic thromboembolic event and in 1 of 2 patients with
asymptomatic thrombosis more than one genetic mutation had found. Among
patients with and without thromboembolic phenomena, protein C activity, free
protein S, antithrombin III, plasma fibrinogen, serum albumin, lipid profile and 24
hour urine protein were found to be similar. The coexistence of inherited
thrombophilia in addition to acquired factors in nephrotic syndrome may facilitate
thromboembolic complications, if the cause of thrombosis cannot be explained by
the acquired factors, screening for hereditary thrombophilia may be beneficial.
