Brakiyal pleksus bloğu kolda yapılacak birçok
cerrahi islem için oldukça iyi analjezi sağlayan ve güvenilir bir yöntemdir. Rejyonel
anestezide kullanılan lokal anestezik maddelerin etkilerini arttırmak ve blok süresini
uzatmak amacıyla pek çok adjuvan ajan kullanılabilir. Biz bu çalısmada el ve ön kol
cerrahisi geçirecek hastalarda aksiller brakiyal pleksus blokajında prilokain,
prilokain’e eklenen deksametazon ve levobupivakain’in lokal anestezik etkilerini
klinik olarak değerlendirmeyi amaçladık. Fakülte etik kurul onayı alındıktan sonra
çalısmaya ASAI-II grubu, 18-60 yas arası el ve ön kol cerrahisi uygulanacak 45 hasta
alındı. Hastalar rastgele 3 gruba ayrılıp 1.gruba 5 mg/kg %2 ‘lik prilokain % 0.9
NaCl ile toplam 40 ml hacimde olacak sekilde tamamlanarak, 2.gruba 5 mg/kg
%2’lik prilokain+8 mg deksametazon (2 ml) % 0.9 NaCl ile toplam 40 ml hacimde
olacak sekilde tamamlanarak, 3. gruba 1.5 mg/kg %0.5’lik levobupivakain % 0.9
NaCl ile toplam 40 ml hacimde olacak sekilde tamamlanarak verildi. Blok süresince
hastalara yüz maskesi ile oksijen verilip, kalp atım hızı, sistolik, diastolik, ortalama
arteriyel kan basınçları, oksijen saturasyon değerleri kontrol, 5, 10, 15, 30, 60.
dakikalarda takip edilerek kayıt edildi. Hastaların sensoriyal ve motor blok baslama
süreleri ve blok süreleri değerlendirilerek takip edilip kayıt edildi. Hastalar yan
etkiler açısından takip edildi. Elde edilen veriler istatistiksel olarak değerlendirildi.
Gruplar arasında hemodinamik parametreler bakımından istatistiksel olarak anlamlı
fark saptanmadı (p>0.05). Sensoriyal ve motor blok baslama süreleri grup 3’te grup 1
ve grup 2’ye göre daha uzundu. Sensoriyal ve motor blok süreleri bakımından grup 3
hem grup 1 hem de grup 2’ye göre, grup 2 de grup 1’ e göre daha uzundu. Fark
istatistiksel olarak anlamlıydı (p≤0.01). Elde edilen veriler aksiller pleksus bloğunda
deksametazonun uygun hastalarda etkili ve güvenli bir adjuvan ajan olarak
kullanılabileceğini, levobupivakainin ise uzun postoperatif analjezi süresi ile ve uzun
sürecek cerrahi girisimlerde çok etkili ve hemodinamik bakımdan güvenilir bir lokal
anestezik olduğunu ortaya koymustur.
Brachial plexus blocakade is a good method for many
surgeries to provide a good analgesia conducted by hand and forearm. Many
adjuvant agents were used to enhance the quality of regional blockage and make
longer duration of anesthesia. Đn this study, we compared the local anesthetics
efficacy in clinically of the prilocaine, dexamethasone added to prilocaine and
levobupivacaine in axillary brachial plexus on hand and forearm surgery. After
approval of the Faculty Ethical Committee, this study was performed on 45 patients
of ASA I-II, 18-60 years old who posses no any other health problems. The patients
were divided into randomized three groups. 5 mg/kg 2 % prilocaine in 40 ml volume
was given to the first group, 5 mg/kg 2 % prilocaine+8 mg dexamethasone ( 2 ml) in
40 ml volume was given the second group and 1.5 mg/kg 0.5 % levobupivacaine in
40 ml volume eas given the third group. In each group, patients’ heart rate, systolic,
diastolic, average blood pressure, O2 saturation were monitored and recorded at
kontrol, 5th , 10th , 15th, 30th, 60th minutes. Data relating to the onset time of sensory
and motor blockade of patients as well as the duration of blockade of those were
saved. All patients were constantly monitored in terms of the side effects. Obtained
data were evaluated statically The findings revealed that there was no statistically
significant difference between the groups in terms of hemodinamic parameters
(p>0.05). The onset time of sensorial and motor blockade in group 3 was longer than
that in both group 1 and group 2. The duration of sensorial and motor blockade in
group 3 was longer than that in other two groups; additionally it was longer in group
2 than the one in group 1. A statistically significant difference was found in this case
(p≤0.01). Based on the obtained data, dexamethasone in axillary blockage can be
effectively and reliably used for appropriate patients as an adjuvant agent.
Furthermore, the findings showed that levobupivacaine that has a long time of
postoperative analgesia can be considered as a reliable local anesthetics based upon
hemodinamic responses and an effective one even in attempts to long surgeries.
