Günümüzde alerjik rinit tedavisinde; allerjenden
korunma, farmakoterapi, immünoterapi ve cerrahi tedavi hastanın durumuna göre tek
başına veya farklı kombinasyonlarda uygulanabilen tedavi seçenekleridir. Bu tedavi
yöntemleri ile yeterli iyileşmenin sağlanamadığı olgularda kullanılabilecek yeni yöntemler
ile ilgili araştırmalar devam etmektedir. Son yıllarda teknolojik gelişmeler sayesinde alerjik
cilt hastalıklarında kullanılarak iyi sonuçlar elde edilen fototerapi, nazal mukozaya da
uygulanabilecek duruma getirilmiştir. Bu çalışmada allerjik rinitli hastalarımıza uygulanan
intranazal fototerapinin, allerjik rinit semptomları ve muayene bulguları ve bu hastaların
yaşam kalitesi üzerine etkileri araştırılmıştır. Polikliniğimize burun akıntısı, burun kaşıntısı,
burun tıkanıklığı şikayeti ile başvuran, kulak burun boğaz fizik muayeneleri, spesifik IgE
düzeyleri ve deri testleri sonucunda allerjik rinit tanısı konan ve rinofototerapi
uyguladığımız ilk 100 hasta ile gerçekleştirildi. Hastaların fizik muayenelerindeki bulguları
ve nazal akıntı, hapşırma, nazal kaşıntı gibi şikayetleri şiddetlerine göre skorlanarak
kaydedildi. 100 olgunun tedavi öncesi ve bitiminden 1 ve 3 ay sonraki yaşam kalitelerinin
belirlenmesinde RQLQ testi kullanıldı. Tedavi öncesi ve sonrasındaki sonuçlar istatistiksel
olarak karşılaştırıldı. Değerlendirme sonucunda tüm semptomlarda tedaviden önceki ve
sonraki skorlar arasında anlamlı fark bulundu(p<0,001***). Çalışmamız sonucunda
fototerapinin alerjik rinit semptom ve muayene bulgularında düzelme sağladığı, hastaların
yaşam kalitesi skorlarını olumlu yönde etkileyen bir tedavi yöntemi olduğu saptandı. Bu
uygulamanın alerjik rinitli olguların tedavilerinde seçilebilecek yeni bir yöntem olabileceği
kanaatine varılmıştır.
Nowadays, avoidance of allergens
and environmental controls, drug therapy, immuno-therapy and surgical theraphy are the
treatment options in allergic rhinitis. They can be administirated alone or in combination
with others depending on the patient situation. The researches for providing better
treatment options are being conducted about allergic rhinitis, in which conventional
therapies fail. In recent years, phototheraphy has been successfully used in allergic
dermatologic diseases and now it is able to be applied to the nasal mucosa, thanks to the
technical improvements. In this study, we investigated the effects of intra-nasal
phototeraphy on symptoms of allergic rhinitis patients. The study included 100 patients
who admitted to the policlinic with symptoms of sneezing, nasal discharge, nasal
congestion and itching and diagnosed as allergic rhinitis according to the findings of their
physical examinations, spesific IgE levels ,skin tests. None of these patients improved
with conventional drug theraphies and immunotheraphy methods Physical examination
findings, complaints like nasal discharge, itching, and sneezing were all recorded by
scoring them according to their intensities. Pre-treatment and post-treatment results were
compared statistically. RQLQ test was used for displaying the difference of the scores.
These scores were obtained using symptoms and physical examination findings including
the ones before treatment and the ones 3 months after the phototherapy. We found that
there was a statistically significant difference between pre and post treatment
scores(p<0,001***). According to our results, we think that phototeraphy can be used, not
only for dermatologic diseases but also for mucosal inflamations related to immune
system. We conclude that this method is a new therapy for treatment of allergic rhinitis.
