Bu çalışma; Eskişehir Osmangazi Üniversitesi Tıp Fakültesi (ESOGÜ),
Eskişehir Yunusemre Devlet Hastanesi (EYDH) ve Eskişehir Devlet Hastanesi (EDH)
ameliyathanelerinde görev yapan hemşirelerin delici/kesici aletlerle yaralanma
(DKAY) sıklığını ve DKAY’ı etkileyen faktörler ile DKAY’a karşı hemşirelerin
aldıkları önlemleri belirlemek amacıyla yapılmıştır.
ESOGÜ, EYDH, EDH ameliyathanelerinde görev yapan 89 hemşireyle bire bir
görüşülerek, sosyo demografik özellikleri, DKAY durumları, DKAY’dan korunmak
için aldıkları önlemler gibi değişkenleri içeren 24 maddelik anket uygulanmıştır.
Elde edilen verilerin SPSS 11,5 paket programıyla istatiksel analizleri
yapılmıştır. Tüm analizler için iki yönlü p değeri kullanılmış ve p değeri 0,05’in
altında ise istatiksel açıdan anlamlı kabul edilmiştir.
Elde edilen verilere göre 89 hemşirenin 77’si (%86,5) son bir yıl içinde en az bir
kez yaralanma yaşamıştır. En fazla yaralanma Genel Cerrahi ve Nöroşirurji
bölümünün ameliyatlarında yaşanmıştır. 0-10 yıldır ameliyathanede çalışan
hemşirelerde DKAY oranı en yüksektir (%59,6). Hemşirelerin %51,9’u sol elinden
yaralanmıştır. Bistüri ve sütur iğnesi en fazla yaralanmaya sebep olan aletlerdir.
08:00-16:00 saatleri arasındaki yaralanmalar daha fazladır (%64). Ameliyatın 3.
saatinden sonra yaralanma oranı artmaktadır (%84,3). Hemşirelerin %92,2’si cerrahi
operasyona yardımcı olurken yaralanma yaşamıştır ve hızlı hareket etmeyi yaralanma
nedeni olarak göstermişlerdir (%65,2). Hemşirelerin %83,1’i Hepatit B (HBV) aşısı
yaptırmıştır.
Bu çalışma sonucunda çalışma süresi 0-10 yıl olan yani deneyimin daha yetersiz
olduğu dönemlerde DKAY en fazladır. Bu sonuç doğrultusunda işe yeni başlayan
hemşirelere sürekli hizmetiçi eğitimin yanında oryantasyon programlarının daha
uygun zamana yayılmasının yararlı olacağı düşünülmüştür. Geçmiş yıllarda yapılan
çalışmalara göre hemşirelerin HBV’ye karşı aşılanma oranı artsada (%83,1) istenen
düzeyde değildir.
This study was carried out to determine the frequency of injury caused by
needlestick and sharp injury (NSI) of the nurses working in the operating theatres of
Eskişehir Osmangazi University (ESOGÜ), Eskişehir Yunusemre state hospital
(EYDH) and Eskişehir state hospital (EDH) and the factors affecting NSI and the
precautions taken by the nurses against NSI.
While collection research data, used to by interviews by one by with 89 nurses
working at the operating theatres of ESOGÜ EYDH and EDH, we carried out a 24-
item survey including such variables as socio demographic features, NSI incidences
and precautions taken against NSI.
The statistical analysis of the data gathered was done through the SPSS 11,5
package program. For all analysis, two-way p value was used and if p value is below
0,05, it is considered as statistically significant.
According to the data collected, 77 of 89 nurses (%86,5) injured at least one
time in the past 1 year. The injuries mostly occurred during general and neurosurgery
surgeries. The NSI rates were highest for the nurses working in the operating theatres
for 0 to 10 years (%59,6). %51,9 of the nurses were injured in their left hands.
Bistouries and suture needles were the most common causes of the injuries. The
injuries were more common the hours between 8 and 4 pm (%64). After the 3rd hour
of the surgery, the injuries increased (%84,3). %92,2 of the nurses were injuries while
assisting surgery and they argued that fast movements were the cause of the injuries
(%65,2). %83,1 of the nurses were vaccinated against Hepatitis B (HBV).
Considering this study, it can be concluded that NSI is more common if the staff
have an experience of 0 to 10 years, which is insufficient. In view of these results, it
was thought that new comer, inexperienced, nurses should be given more in-service
training and the orientation programs should be given in the in the length of time.
Although the rate of vaccination of nurses against HBV has increased compared to the
studies conducted earlier than this study, it is not at the level that is desired.
