Atriyum fibrilasyonda kardiyoversiyonun hemostaz üzerine olan
etkileri arastırılmıs olmakla birlikte monofazik ve bifazik kardiyoversiyon
yöntemlerini karsılastıran bir çalısma yoktur. Çalısmaya elektriki kardiyoversiyon
endikasyonu konmus ve yas ortalaması 64,3 ± 9,7 olan 14’ü erkek ve 26’sı bayan
olmak üzere toplam 40 hasta dahil edildi. Kardiyoversiyon islemi transtorasik
anterolateral yöntemle monofazik ve bifazik olarak yapıldı. EKV’un koagulasyon ve
inflamasyona etkisini arastırmak için islemden hemen önce, islemden 5 dakika
sonrası ve islemden 30 dakika sonrası olmak üzere 3 kere fibrinojen, D-dimer, sPSelectin,
aktive trombositlerin (CD41 FITC+ ve CD62P PE+ ) trombositler içindeki
degerleri, lenfosit + trombosit aggregatı olarak degerlendirilen (CD41 FITC+ ve
CD45 PerCP+ ) lenfositler içindeki degerleri, Protrombin fragment 1+2 degerleri,
monosit + trombosit aggregatı olarak degerlendirilen (CD41 FITC+ ve CD45
PerCP+ ) monositler içindeki degerleri, nötrofil + trombosit aggregatı olarak
degerlendirilen (CD41 FITC+ ve CD45 PerCP+ ) nötrofil içindeki degerleri ve hs-
CRP ise EKV islem öncesi ve islemden 1 hafta sonra olmak üzere 2 kere çalısıldı.
Monofazik ve bifazik dalga formlarıyla EKV yapılan hastaların çalısılan bu degerleri
karsılastırıldıgında her iki yöntem ile hs-CRP dısındaki markerlarda artıs olmakla
birlikte iki yöntem arasında istatistikî olarak belirgin fark bulunmamıstır. hs-CRP
düzeylerinde her iki grupta da birinci haftada islem öncesine göre düsüs
gözlenmistir. Bunun nedeni olarak hs-CRP’nin bir hafta sonra tekrarlanmıs olmasını
düsünmekteyiz. Sonuçta iki EKV yöntemi arasında olası tromboembolik
komplikasyon açısından birbirlerine üstünlük saglayamadıgını ve bu yönden
birbirlerine tercih sebebi olamayacaklarını düsündürmektedir.
Previous research has studied the effect of cardioversion on
hemostasis in the atrial fibrillation, however a comparative study between
monophasic and biphasic cardioversion methods does not exist. In this study there
are total of 40 patients who underwent electrical cardioversion (ECV). Fourteen of
the patients are male and 26 of them are female, and the average age is 64,3 ± 9,7.
Monophasic and biphasic cardioversion procedures have been applied with
transthorasic anterolateral method. To explore the effect of ECV on coagulation and
inflammation, fibrinogen, D-dimer, sP-Selectin, the percentage of active
thrombocites (CD41 FITC+ ve CD62P PE+) among all thrombocites, the percentage
of aggregate of lenfocyte + thrombocite (CD41 FITC+ ve CD45 PerCP+) among all
lenfocytes, Prothrombin fragment 1+2, the percentage of aggregate of monocyte +
thrombocite (CD41 FITC+ ve CD45 PerCP+) among all monocytes, the percentage
of aggregate of neutrophil + thrombocite (CD41 FITC+ ve CD45 PerCP+) among all
neutrophils are studied total of 3 times; just before, 5 minutes after, and 30 minutes
after the procedure. In addition, hs-CRP is studied total of 2 times; just before and 1
week after the procedure. When we compared these values for patients who
underwent monophasic and biphasic waveform ECV, all the markers except hs-CRP
increased, however the differences were not statistically significant. We observed
that hs-CRP levels decreased in the first week when compared with before procedure
levels. We believe the reason for this is the repeated hs-CRP one week after the
procedure. As a result, neither of these two ECV methods is superior to each other
from a thromboembolic complication point of view, and therefore there is no
motivation to prefer one against the other.
