Bu tez
çalışması PTE tanılı hastalarda klinik belirleyicilerin araştırılması amacı ile
Eylül 2009 ve Eylül 2011 tarihleri arasında gerçekleştirildi. PTE tanılı 132
hasta çalışmaya alındı. Yoğun bakım yatışı, vasopressör gerektiren
hipotansiyon, entübasyon, nüks gelişmesi, trombolitik uygulanması ve mortal
seyir durumlarından bir veya birden fazlasının olması kötü klinik sonuç olarak
kaydedildi. Eşlik eden hastalıklar, risk faktörleri ve akciğer grafi bulguları ile
PTE prognozu arasında ilişki gösterilemedi. Artmış D-dimer, pro-BNP, Tn I
düzeyleri ve dinamik toraks BT bulgularından RV/LV oranının > 1,1, PVOS >
11 olması ve BT’ de IVS’ un sola deviye izlenmesi kötü klinik sonuçlarla ilişkili
bulundu. Başvuru esnasında hipotansiyon varlığı, EKG’ de sağdal bloğu ve
EKO’ da İVS’ un sola deviye izlenmesi kötü prognoz için bağımsız risk
faktörleri olarak bulundu. Bağımsız risk faktörlerine eşlik eden Tn I yüksekliği
varlığında kötü klinik sonuçlar için duyarlılığın ve negatif kestirim değerinin
anlamlı oranda arttığı görüldü. Sonuç olarak akut PTE tanılı bir hastada
prognozu tahmin etmede klinik, laboratuar ve radyolojik bulguların tek tek
değerlendirilmesi yerine bu bulguların kombinasyonunun daha güçlü bir
prognostik değerlendirmeye olanak sağladığı görüldü.
This specialty
thesis is studied in order to search for the clinical determinants of patients
diagnosed with PTE between september 2009 and september 2011. 132
patients with the diagnose of PTE were included in the study. Death within 30
days after diagnosis of PTE, admission to the intensive care unit,
cardiopulmonary resuscitation, endotracheal intubation, vasopressor therapy,
relapse and thrombolysis were recorded as adverse clinical outcomes. No
correlation between the prognosis of PTE and the factors including
comorbidities, risk factors and chest radiography findings was found. It was
found that there was a correlation between adverse clinical outcomes and the
factors including elevated D-dimer, pro-BNP, Tn I levels, RV/LV ratio > 1,1,
PVOS > 11, deviation of IVS to the left on the thorax CT. It was found that
hypotension on the admission, right bundle branch block on the EKG and
deviation of IVS toward the left ventricle on the EKO were independent risk
factors for adverse clinical outcomes. Also, it was seen that there was a
significant increase in sensitivity and negative predictive value of adverse
clinical outcomes in the existence of elevated Tn I levels concominant with
independent risk factors. In conclusion, while deciding about prognosis in the
patients with acute PTE, it was seen that taking into account the combination
of clinical, laboratorial and radiological findings all together instead of
evaluating these findings seperately provided a stronger prognostic
assessment of patients with PTE.
