Bu çalışmada kolorektal
kanserli hastalarda primer tümör, lenf nodu tutulumu ve uzak metastazların
saptanmasında F-18 FDG PET/BT tanı ve evrelemedeki rolünün belirlenmesi
amaçlandı. Gereç ve Yöntem: Kolonoskopik biopsi ile kanıtlanmış kolorektal
kanser tanısı olan 44 cerrahi öncesi hastada cerrahi sonrası histopatolojik bulgular,
biyopsi ve yaygın uzak metastaz nedeniyle cerrahi yapılmayan hastalarda klinik ve
radyolojik bulgular altın standart kabul edilerek kontrastlı rutin abdominopelvik BT
ve F-18 PET/BT bulguları karşılaştırıldı. Bulgular: Primer tümöral lezyonların
tümünde (%100) F-18 FDG PET/BT ile artmış metabolik aktivite izlendi. Primer
tümörlerin ortalama ortalama SUVmax 14.83±1.20 (range 5.85-45.7) olarak ölçüldü.
Lenf nodu metastazını saptamada F-18 FDG PET/BT’nin duyarlılığı BT’ye göre
daha düşük (%35’e karşılık %60) oranda ancak özgüllüğü ve PPD’i BT’ye göre
oldukça yüksek (%100’e karşılık %70.8 ve %100’e karşılık %63.2) olarak saptandı.
Yedi hepatik metastazlı hastanın hepsinde PET/BT ile lezyonlar hipermetabolik
olarak görüldü. BT’nin ise karaciğer metastazlarını saptamada duyarlılığı %71,
özgüllüğü %74, doğruluğu %63.6, NPD’i %92, PPD’i %38 olarak saptanmıştır.
Toplam 44 hastanın 17’inde (%38.6) ekstrahepatik uzak metastaz (akciğer, kemik,
sürrenaller ve supraklavikuler lenf nodu) saptandı. Metastazların eşik SUVmax
değerlerini belirlemek amacıyla yapılan ROC analizinde hepatik metastazlar için
SUVmax 3.64, ekstrahepatik metastazlar için ise 1.99 olarak bulundu. Sonuç olarak;
kolorektal kanserli hastaların preoperatif evrelemesinde primer tümör ve lenf nodu
tutulumunun saptanmasında F-18 FDG PET/BT’nin tanısal BT’ye üstünlük
göstermediği, ilk evrelemede sınırlı bir tanısal değere sahip olduğu görüldü. F-18
FDG PET/BT’nin major rolünün hepatik ve ekstrahepatik metastazların saptanması
olduğunu ve tanı anında potansiyel ekstrahepatik uzak metastazı olan ileri evre
hastalarda F-18 FDG PET/BT’nin en üstün görüntüleme yöntemi olduğunu
düşünmekteyiz.
The objective of the
study was setting the role of F-18 FDG PET/BT in detection of the primary tumour
and lymph node and distant metastases in patients with colorectal cancer. Material
Method: Preoperative Abdominopelvic BT and PET/BT findings of 44 patients
proved to have colorectal cancer by colonoscopic biopsy were compared by
accepting postoperative histopathologic findings, biopsy (hepatic metastases) and
clinical and radiological follow-up of lesion progression in patients that were not
operated as gold standart. Results: All lesions were detected with F-18 FDG PET/BT
(%100) as all had incresed metabolic activity in F-18 FDG PET/BT images. Mean
SUV Max value of the primary tumour was 14.83±1.20 (range 5.85-45.7). Regarding
detection of lymph node metastases, sensitivity of PET/BT was lower(%35 vs %60)
but specivity (%100 vs %70.8) and PPD (%100 vs%63.2) were substantially higher
than BT sensitivity. In all of the eeven patients with hepatic metastases, lesions were
hipermetabolic on PET/BT images. Sensitivity of BT in detecting hepatic metastases
was measured as %71, spesifity is %74, accuracy is %63.6, NPD is %92, PPD is
%38 .17 of 44 patients (%38.6) had extrahepatic distant metastases (lung, bone,
surrenal glands and supraclaviculer lymph node). ROC analyse that was done to
define the optimal SUV max value to define the hepatic and extrahepatic lesions seen
as metastatic on PET/BT images was found as 3.64 for hepatic metastases and 1.99
for extrahepatic metastases. Conclusion: As a result, in the preoperative staging of
colorectal cancers, F-18 FDG PET/BT was not found to be superior to BT in the
detection of the primary tumour and lymph node involvement and it was seen that it
had limited diagnostic value at initial staging. We think that the major role of
PET/BT is detection of hepatic and extrahepatic metastases and for advenced stage
patients that have potential to have extrahepatic metastases, PET/BT is the best
imaging modality.
