Kronik karaciğer parankim
hastalığı ile ilişkili siroz HSK gelişimi için en önemli risk faktörüdür. Kılavuzlar HSK
taraması için AFP ve görüntüleme yöntemlerini önermektedir. AFP yüksek veya düşük
olmasının HSK fenotipindeki değişiklikler ile ilişkili olabileceği düşünülmüştür. Bu
çalışmada AFP yüksek ve düşük hastalarda görüntüleme bulgularının farklı olup
olmadığını değerlendirmeyi amaçladık. Ocak 2015- Şubat 2021 tarihleri arasında
çalışma kriterlerine uyan 108 hasta çalışmaya dahil edildi. Hastaların görüntüleri 2
radyolog tarafından değerlendirildi. Hastalar AFP değeri 20 IU/ml göre yüksek ve
düşük olarak iki gruba ayrıldı. Hastaların 51 (%47,2)’inde AFP düşük, 57
(%52,8)’sinde yüksekti. 54 (%50) hastada tek, 38 (%35,2)’inde çok, 16 (%14,8)’sında
infiltratif HSK vardı. Tek lezyonu olan hastalarda AFP değeri düşük, çok ve infiltratif
lezyonu olanlarda yüksek olma eğilimi göstermekteydi (χ
2=16,08, P<0,001). LI RADS sınıflamasına göre 3 (%2,8) hastada LI-RADS 3, 16 (%14,8)’sında LI-RADS
4, 66 (%61,1)’sında LI-RADS 5, 23 (%21,3)’ünde LI-RADS TIV lezyon mevcuttu.
AFP yüksek grupta LI-RADS-TIV hasta daha çoktu (χ
2=20,00; P<0,001). Hastaların
96 (%88,9)’sında patolojik lenf nodu yoktu, 12 (%11,1)’sinde patolojik lenf nodu
vardı ve gruplar arasında farklılık saptanmadı. (χ
2=2,67 P=0,102). İndeks lezyonun
AFP yüksek grupta boyut ortalaması 62,99±34,57 mm, düşük grupta 36,41±26,8
mm’di (P<0,001). AFP değerinin boyut ile arasında orta kuvvette pozitif korelasyon
vardı (R=0,529, P<0,001). AFP yüksek ve düşük grup arasında lezyon/dalak ADC
oranlarında farklılık saptanmamıştı. (sırasıyla; 1,13±0,31; 1,19±0,26, P=0,319). AFP
için kesim noktası 42 kabul edildiğinde tümör trombüsü varlığını ortaya koymadaki
tanısal performasında sensivite %87, spesifite %65 hesaplandı. Sonuç olarak, HSK
tanısı için MRG bulguları AFP değerinden bağımsızdır. Ancak yüksek değerler,
multipl ya da infiltratif HSK morfolojisi, büyük tümör boyutu ve eşlik edebilecek
venöz tümör trombüsü açısından uyarıcı olmalıdır
Cirrhosis associated with chronic liver
parenchymal disease is the most important risk factor for the development of HCC.
Guidelines recommend AFP and imaging modalities for HCC screening. High or low
AFP levels were thought to be associated with changes in the HCC phenotype. In this
study, we aimed to evaluate whether imaging findings are different in patients with
high and low AFP levels. Between January 2015 and February 2021, 108 patients who
met the study criteria were included in the study. The images were evaluated by 2
radiologists. Patients were divided into two groups as high and low AFP with the cut off point of 20 IU/ml. AFP was low in 51 (47.2%) patients and high in 57 (52.8%)
patients. There was a single lesion in 54 (50%) patients, multiple lesion in 38 (35.2%)
and infiltrative HCC in 16 (14.8%) patients. AFP value tends to be low in patients with
a single lesion and higher in patients with multiple and infiltrative lesions (χ2=16.08,
P<0.001). The images were LI-RADS 3 in 3 (2.8%) patients, LI-RADS 4 in 16 (14.8%)
patients, LI-RADS 5 in 66 (61.1%) patients and LI-RADS-TIV in 23 (21.3%) patients.
There were more LI-RADS-TIV patients in the high AFP group (χ2=20.00; P<0.001).
96 (88.9%) of the patients had no pathological lymph nodes, 12 (11.1%) of them had,
no significant difference was found between AFP groups (χ2=2.67 P=0.102). The
mean size of the index lesion was 62.99±34.57 mm in the high AFP group and
36.41±26.8 mm in the low group (P<0.001). There was a moderately strong positive
correlation between AFP value and size (R=0.529, P<0.001). There was no difference
in lesion/spleen ADC ratios between the AFP groups (respectively; 1.13±0.31;
1.19±0.26, P=0.319). With the cut-off point 42 for AFP, the diagnostic performance
in detecting the presence of tumor thrombus is calculated as 87% sensitivity and 65%
specificity. In conclusion, MRI findings for the diagnosis of HCC are independent of
AFP value. However, high values should be indicative of multiple or infiltrative HCC
morphology, large tumor size, and accompanying venous tumor thrombus
