Bipolar bozukluk (BB) ve majör depresyon
(MD)’un remisyon döneminde bilişsel işlev bozukluklarının devam ettiğini gösteren
birçok çalışma mevcuttur. İntihar girişiminin de bilişsel işlevlerde bozulmaya yol açan
faktörlerden biri olduğu düşünülmektedir. Bu çalışmanın amacı, intihar girişimi olan
ve olmayan remisyondaki BB ve MD hastalarının bilişsel işlevlerini karşılaştırmak;
hastalıkların ve intiharın bilişsel işlevler üzerindeki etkisini değerlendirmektir. Bu
amaçla 01.06.2020-01.03.2021 Eskişehir Osmangazi Üniversitesi Psikiyatri Anabilim
Dalı polikliniklerine başvuran, klinik görüşme ile DSM-5 tanı kriterlerine göre BB ve
MD tanısı konan, remisyonda olan ve araştırmaya dahil edilme kritlerlerini karşılayan
19 intihar girişimi olan BB, 21 intihar girişimi olmayan BB, 21 intihar girişimi olan
MD, 19 intihar girişimi olmayan MD ve 20 sağlıklı kontrole Cambridge
Neuropsychological Test Automated Battery’ye ait 6 nörobilişsel test uygulanmıştır.
Çalışmamızda BB ve MD gruplarının yürütücü işlevler, işlem belleği ve duygu tanıma
becerilerini içeren bilişsel işlevlerinde sağlıklı kontrollere göre daha düşük performans
gösterdiği, intihar girişiminde bulunan katılımcıların yürütücü işlevler ve duygu
tanıma yetisinde kayıp yaşadığı saptanmıştır. Atak periyodlarının kısa olması bilişsel
işlevlerdeki kayıpla ilişkili görünmektedir. İntihar girişiminde bulunan grubun korku
ve mutluluk duygusunu tanımakta zorlandığı, intihar girişimi olmayan grubun ise
tiksinme ve öfke duygularını tanımakta zorlandığı saptanmıştır. Çalışmamızın
sonuçları, intihar girişiminin nörobilişsel performansta daha fazla performans kaybına
yol açtığını göstermektedir
In the up-to-date literature, there are many studies showing that cognitive
dysfunctions continue in the remission period of bipolar disorder (BD) and major
depression (MD). Suicide attempt is thought to be one of the factors that lead to
deterioration in cognitive functions. The aim of this study was to compare the cognitive
functions of BD and MD patients in remission with and without suicide attempt; to
evaluate the effect of diseases and suicide on cognitive functions. For this aim,
01.06.2020- 01.03.2021, who applied to the outpatient clinics of Eskişehir Osmangazi
University Psychiatry Department, were diagnosed with BD and MD according to the
DSM-5 diagnostic criteria by clinical interview, were in remission and met the criteria
for inclusion in the study, 19 BD patients who attempted suicide, 21 BD patients
without suicide attempt, 21 MD patients who attempted suicide, 19 MD patients
without suicide attempt, and 20 healthy controls were included in the study. 6
neurocognitive tests of Cambridge Neuropsychological Test Automated Battery were
applied. In our study, it was determined that the BD and MD groups performed lower
than the healthy controls in cognitive functions including executive functions, working
memory and emotion recognition skills, and the participants who attempted suicide
had more impairment, especially in executive functions and emotion recognition. The
short attack periods seem to be related to the impairment in cognitive functions. It was
determined that the group that attempted suicide had difficulty in recognizing the
feelings of fear and happiness, while the group that did not attempt suicide had
difficulty in recognizing the feelings of disgust and anger. The results of our study
show that suicide attempt causes more performance loss in neurocognitive
performance
