Eskisehir’de Kursunla Çalısan syerlerinde Kursun Maruziyeti. Eskisehir
Osmangazi Üniversitesi Tıp Fakültesi Halk Saglıgı Anabilim Dalı Uzmanlık Tezi,
Eskisehir, 2007. Bu çalısma, Aralık 2006-Nisan 2007 tarihleri arasında yapılan kesitsel tipte
bir arastırmadır. Çalısma grubu, Eskisehir’de kursunlu materyal kullanılan toplam 10
isyerinde çalısan 403 erkek isçi ile olusturuldu. Kursuna mesleksel maruziyeti olmayan ve
saglıklı 97 erkek, kontrol grubu olarak alındı. sçilerde kursun maruziyeti ve kursunun saglıga
etkilerinin degerlendirilmesi amacıyla hazırlanan bir anket yüz yüze görüsme yöntemiyle
uygulandı. syerlerine yönelik koruyucu önlemlerin ne durumda oldugu diger bir anket ile
degerlendirildi. sçilerden disposibl enjektör ile 4 cc venöz kan örnegi alındı. Alınan venöz
kan örnekleri EDTA içeren 2 adet vakumlu kan tüpüne aktarıldı. Bu tüplerden biri aynı gün
içerisinde tam kan sayımı için kullanıldı. Diger tüp ise kanda kursun düzeyi tayininde
kullanıldı. Kanda kursun düzeyi, LeadCare® Kanda Kursun Test Sistemi ile tayin edildi. Kan
kursun düzeyi 10 μg/dL olanlar asemptomatik kursun zehirlenmesi ve 40 μg/dL olanlar
kursun zehirlenmesi olarak kabul edildi. Elde edilen veriler, SPSS 13.0 istatistik paket
programı ile degerlendirildi. statistiksel analizler için Ki-kare testi (x2), Fisher’in Kesin Ki
kare testi, Kolmogorov-Simirnov Z testi, Student t testi ve lojistik regresyon analizi kullanıldı.
Çalısma grubunda ortalama kan kursun düzeyi, kontrol grubuna göre anlamlı bir sekilde daha
yüksek bulundu (p 0.05). sçilerde asemptomatik kursun zehirlenmesi görülme sıklıgı %10.2
olarak saptandı. Kursun zehirlenmesi olgusuna rastlanmadı. Lojistik modelde, ortaokul ve altı
ögrenim düzeyi (OR= 3.041), 1 yıldan az çalısma süresi (OR= 5.700), is bitiminde el-yüz
yıkama ve/veya dus alma alıskanlıgı olmamak (OR=2.668) ve kursun zehirlenmesi öyküsü olmak
(OR=9.290) asemptomatik kursun zehirlenmesi için önemli risk faktörleri olarak bulundu.
Çalısma sonunda, isyerlerine yönelik koruyucu önlemlerin iyi durumda oldugu görüldü.
Kisisel koruyucu araçların kullanımı ve hijyen ile ilgili alıskanlıkların tüm isçilere
kazandırılması için saglık egitimlerinin planlanması ve uygulanması gerektigi kanısına
varıldı.
The lead exposure in the workplaces worked with lead in Eskisehir.
Eskisehir Osmangazi University Faculty of Medicine, Medical Speciality Thesis in
Department of Public Health, Eskisehir, 2007. This study was done in December 2006-
April 2007 period; it is a cross-sectional type research. Study group was occurred 403 men
workers that have been worked in the total 10 workplaces that leady material used in
Eskisehir. The control group was taken total 97 men that no occupational lead exposures and
healthy. A questionnaire that is prepared to determine with the aim of lead’s effects to health
and the lead exposure among workers was used face to face method. The other questionnaire
was used the determined how the status of the protective steps. The 4 cc venous blood
samples were taken by the disposable injectors from the workers. Venous blood samples
which is taken from the workers put in 2 vacuum blood tubes that includes EDTA. One of the
tubes was used to be prepared whole blood counting in the same day. The other tube was
used to measure the blood lead level. Blood lead level was determined by LeadCare® Blood
Lead Testing System. The bloods lead level 10 μg/dL that is asymptomatic lead poisoning,
and 40 μg/dL is lead poisoning. The acquired data were determined by SPSS 13.0 statistical
packet programme. For statistical analyses, chi square test (x2), Fisher’s Exact test,
Kolmogorov-Simirnov Z test, Student t test, and logistic regression analysis were used. In
study group, the mean blood lead level was found significantly higher than control group
(p 0.05). The prevalence of asymptomatic lead poisoning was found 10.2% in the workers.
The lead poisoning event was couldn’t find. The middle school and under educational level
(OR= 3.041), under the 1 year of employment duration (OR= 5.700), no habit hand-face
washing and/or no have a shower in the end of the work day (OR=2.668), and history of lead
poisoning (OR=9.290) are important risk factors for asymptomatic lead poisoning in logistic
model. The beliefs are approached that the planning and administration of health education
programme among the use of individual protective agents and hygiene habits is going to be
being won by whole workers.
