Riskli ve risksiz gebelere ait plasentalarda patolojik bulguların
karsılastırılması ve bu patolojilerin perinatal mortalite ve morbidite ile iliskisi.

Hastanesi’nde Kadın Hastalıkları ve Dogum Anabilim Dalı’nda Kasım 2004-
Temmuz 2005 tarihleri arasında 28-42. gebelik haftalarında dogum yapan gebelere
ait 350 adet plasenta prospektif olarak incelendi. Çalısma grubunu olusturan gebeler
maternal ve fetal risk faktörlerinin varlıgına göre sınıflandırıldı. Bu çalısma 237’si
riskli, 92’si risksiz gebeye ait 329 tekiz ve 21’i ikiz gebeye ait plasentaları
içermekteydi. Tüm maternal ve neonatal bilgiler kaydedildi. 350 adet plasenta 2
patolog tarafından makroskobik ve mikroskobik olarak incelendi. Tekiz gebelerde
plasenta agırlıkları ile bebek dogum agırlıkları ve gebelik yasları arasında önemli
korelasyon saptandı (P<0.001). Bu gruplar histopatolojik bulgularına göre
karsılastırıldıgında risk faktörüne sahip gebeliklerde koriyoamnionitis (P<0.001),
akut ve kronik intervillözitis, santral infarkt ve villus maturasyonu anormallikleri
daha fazla görüldü ve bu bulgular istatiksel olarak önemli bulundu (her bir bulgu için
P<0.05). Koriyoamnionitis (P<0.001), funisitis, fibromuskuler sklerozis, sinsityal
dügümlerde artıs, periferal ve santral infarkt, perivillöz fibrin depolanımını (her bir
deger için P<0.05) içeren histopatolojik bulgular perinatal morbidite ile anlamlı
iliskili olarak izlendi. Villitis, villöz ödem, villöz damarlarda çekirdekli fetal
eritrositlerin bulunusu (her bir bulgu için P<0.001), desidüitis, hemorajik
endovaskülitis, perivillöz fibrin depolanımı ve maternal yüzey infarkt bulunması (her
bir bulgu için P<0.05) perinatal mortalite ile önemli iliski saptandı. Ayrıca bu
çalısmada gebe kadınların saglık güvence tipinin riskli gebelikler ve perinatal
morbidite ile anlamlı iliskili oldugu görüldü (P<0.001). Bu çalısma hastanemizin il
içinde yüksek riskli gebelikler için önemli bir referans merkezi oldugunu ve plasental
incelemenin perinatal tanının rutin bir parçası olması gerektigini desteklemektedir.
Comparison of pathologic features of placentas in risky and riskless
pregnancies and their relationship with perinatal mortality and morbidity.

all pregnancies at 28 to 42 weeks of gestational age, delivered at the Department of
Obstetrics and Gynaecology of Eskisehir Osmangazi University Hospital from
November 2004 to July 2005. The study groups are classified according to the
existence of maternal and fetal risk factors in pregnancies. Two hundred thirtyseven
singleton placentas of pregnancies with risk factors, 92 placentas of pregnancies
without risk factors, and 21 placentas of twin pregnancies were included in the study.
All maternal and neonatal records were reviewed. 350 placentas were examined
macroscopically and microscopically by two pathologist. Singleton pregnancies
showed significant correlation between the weight of placenta, the weight of
newborn and gestational age (P<0.001). When this study groups were compared
according to their histopathologic findings, it was found that there were more
placental chorioamnionitis (P<0.001), acute and chronic intervillositis, central infarct
and abnormality of villous maturation in the pregnancies having risk factors and
these findings were statistically significant (P<0.05, for all instance). Histopathologic
findings including chorioamnionitis (P<0.001), funisitis, fibromuscular sclerosis,
excessive syncytial knots, peripheral and central infarction, perivillous fibrin
deposition were significantly associated with perinatal morbidity in newborn
(P<0.05, for each instance). Presence or absence of villitis, villous edema, nucleated
fetal red blood cells in villous vessels (P<0.001, for each instance), deciduitis,
hemorrhagic endovasculitis, perivillous fibrin deposition, maternal floor infarct
(P<0.05, for each instance) were significantly associated with perinatal mortality in
newborn. In this study, the presence of perinatal morbidity, fetal and maternal risk
factors (P<0.001) were significantly associated with the type of health insurance of
pregnant women. This study suggests that our hospital is an important reference
center in the city for high-risk pregnancies and placental pathology should be a
routine component of perinatal diagnosis.
