Amaç: Degisici Epitel Karsinomu (DEK) mesane kanserinin en yaygın
türüdür. Kromozom aberasyonları DEK nun gelismesinde ve ilerlemesinde
gereklidir. Bu çalısmada, mesane degisici epitel karsinomu (DEK) tanısında
noninvaziv yaklasımla interfaz FISH tekniginin potansiyel klinik uygulamasını
belirlemek ve kromozomal anöploidilerle tümörün derecesi ve evresi arasındaki
iliskiyi arastırmak amaçlanmıstır.
Materyal ve Metot: Üroloji bölümünden degisici epitel karsinoma tanısı
almıs 30 hasta ve saglıklı 15 kontrolden, bosaltılmıs idrar ve mesane yıkama ile
hücreler elde edilmistir. Bu hücrelerde 3, 7, 8, 11 ve 17 kromozomların
sentromerlerine özgü problar ve 9p21 lokus spesifik problar kullanılarak FISH
analizi yapılmıstır.
Bulgular: FISH analizinin DEK belirlemesinde sensitivitesi mesane yıkama
ve idrar örnekleri için sırasıyla % 96.7 ve % 92.6, spesifitesi ise % 100 olarak
bulunmustur. G1-G2 (Düsük) tümörlerin mesane yıkama ve idrar örneklerinde
görülen en sık kromozom anomalisi 9p21 delesyonu (% 66-% 75) dur. Kromozom
3, 7, 8, 11 ve 17 polizomisinin T1 G2-G3(Yüksek) ve T2 G3(Yüksek) karsinomlara
spesifik oldugu bulunmustur. G1-G2 (Düsük) ve G2-G3(Yüksek) tümörleri
arasında kromozom 3, 7, 8, 11 ve 17 polizomi frekansında istatistiksel olarak
anlamlı farklılık oldugu bulunmustur (P<0,01).
Sonuç: FISH analizi, mesane yıkama ve idrar örneklerinden elde edilen
üretelyal hücrelerde kromozomal aberasyonları belirleyerek, degisici epitel
karsinomu tanısında klinige katkı saglayabilecek potansiyele sahip noninvaziv bir
tekniktir. Kromozom polizomisi ile yüksek tümör derecesi ve evresi koraledir.

Objective: Transitional Cell Carcinoma (TCC) is the most common form of
bladder cancer. Chromosomal aberrations are involved in the development and
progression of TCC of bladder. In this study, we aimed to determine the potential
for the clinical application of interphase FISH technique with noninvaziv approach
in diagnosis of transitional cell carcinoma (TCC) and to investigate the relation
between chromosomal aneuploidies with tumor grades and stage.
Methods: The cells were obtained by bladder washing and voided urine from
30 patients who had a transitional cell carcinoma (TCC) of the bladder diagnosis
and 15 healthy controls in the Urology Department. In the samples, FISH analysis
was performed by using centromere specific probes for chromosome 3, 7, 8, 11, 17
and locus specific probe of 9p21.
Results: The sensitivities of FISH to detect TCC for bladder washing and
voided urine were found 96.7 % and 92.6 %, respectively, but its specificity was
found 100%. The most frequently seen chromosomal alterations G1-G2(Low)
tumors were deletion of 9p21 (66 % and 75 %) in bladder washing and voided
urine. Polysomy was specific to T1G2-G3(High) and T2G3(High) carcinomas. A
statistically significant difference was seen in the frequency of polysomy between
G1-G2(Low) and G2-G3(High) for chromosomes 3, 7, 8, 11 and 17 (P<0.01).
Conclusions: FISH assays have potential that can provide contribution to
clinical diagnosis of transitional cell carcinoma to detect chromosomal aberrations
present in urothelial cells obtained from voided urine and bladder washing.
