Gabapentin santral sinir sisteminin önemli bir nörotransmitteri olan GABA’nın yapısal analoğu ve üçüncü kuĢak antiepileptik bir ilaçtır. Parsiyel epileptik nöbetler dıĢında nöropatik ağrı ve hareket bozukluklarının tedavisinde de kullanılmaktadır. Psikiyatri alanında ise bipolar bozukluk, anksiyete bozuklukları ve alkol bağımlılığının tedavisinde etkili olduğunu bildiren klinik çalıĢmalar vardır.
Bu çalıĢmada 250-300 g ağırlığında diĢi spraque-dawley sıçanlar kullanılmıĢ olup, tek doz gabapentin (5,10,20,40 mg/kg), amitriptilin 10 mg/kg, sertralin 5 mg/kg, diazepam 5 mg/kg, ketamin 10 mg/kg, i.p. olarak verildi. Ayrıca gabapentin 40 mg/kg ile diğer gruplar kombine kullanılarak her grupta 8 sıçan olacak Ģekilde 12 grup oluĢturuldu. Kontrol grubuna ise serum fizyolojik verildi. Gruplardaki sıçanlara yükseltilmiĢ + labirent testi ile ardından zorlu yüzdürme testi uygulandı. Sonuçlar ortalama ± SEM olarak verilip, istatistiksel değerlendirmede Mann-Whitney U testi kullanıldı.
Gabapentin tüm dozlarında anksiyolitik ve doza bağımlı antidepresan etki gösterirken, amitriptilin ve sertralin ile aynı oranda antidepresan etki göstermiĢtir. Ketamin ve diazepamın antidepresan etkisi tek baĢına ve gabapentin ile kombinasyonlarında daha fazladır. Sertralin ise gabapentinin bu etkisini arttırmıĢtır. Anksiyolitik etki açısından değerlendirdiğimizde ise; gabapentin amitriptilin ile aynı oranda etki yaparken, diazepam ve ketamin grupları gabapentine göre daha fazla anksiyolitik etki oluĢturmuĢlardır. Gabapentinin amitriptilin, diazepam ve ketamin ile kombinasyonlarında ise, gabapentin ve kontrole göre daha güçlü anksiyolitik etki gözlenmiĢtir. Sertralin ve sertralinin gabapentin ile kombinasyonlarında ise anksiyolitik etki gözlenmemiĢtir.
Gabapentine, a third generation antiepileptic drug, is a structural analogous of GABA which is an important mediator of central nervous system. It is used not only in the treatment of partial epileptic seizures but also in the treatment of neuropatic pain syndromes and movement disorders. Furthermore, there is clinical evidence indicating its effectiveness in the treatment of psychiatric illnesses such as bipolar disorder, anxiety disorders and alcohol addiction.
In this study, female spraque-dawley rats weighing 250-300 g were used. 12 groups each containing 8 rats were formed: Gabapentin (5,10,20,40 mg/kg), amitryptilline (10 mg/kg), sertraline (5 mg/kg), diazepam (5 mg/kg), ketamine (10 mg/kg), gabapentin 40 mg/kg was also combined with amitryptilline (10 mg/kg), sertraline (5 mg/kg), diazepam (5 mg/kg), ketamine (10 mg/kg). All the drugs were used i.p. as single dose. Saline was given to control group. Elevated plus maze and forced swimming tests were used as experimental models of anxiety and depression respectively. Results were statistically analysed with Mann-Whitney U test and given as mean +SEM.
It was observed that gabapentin showed an anxiolytic effect in all doses and a dose-dependent antidepressant effect. Its antidepressant effect was found to be the same as the antidepressant effects of amitryptilline and sertraline. The combination of gabapentin with sertraline was more antidepressant than gabapentin or sertraline alone. However, it was found that ketamine and diazepam alone and with gabapentin combination had more antidepressant effect than gabapentin alone. Gabapentin and amitryptilline showed similar anxiolytic effect while anxiolytic effect was more than gabapentin in diazepam and ketamine groups. In the combination groups of gabapentin with amitryptilline, diazepam and ketamine anxiolytic effect was more potent than gabapentin alone and control group. No anxiolytic effect was found in both sertraline alone and in combination group of sertraline with gabapentin.
