Yağ dokusunda üretilip
salınan leptinin ve adiponektinin (AN) immunolojik yolda rol aldıkları bilinmektedir.
Bu çalışmada allerjik hastalıklarda AN ve leptin düzeyleri ve bu adipokinler ile atopi
ilişkisinin değerlendirilmesi amaçlanmıştır. Yaşları 8 ay ile 18 yıl arasında değişen
13 astım, 24 allerjik rinit, 21 allerjik rinit ve astım, 13 atopik dermatitli 41’i kız
30’u erkek, toplam 71 hasta ve 9’u kız ve 8’i erkek 17 sağlıklı çocuk kontrol grubu
olarak çalışmaya alındı. Hastalardan serum AN ve leptin düzeyleri, tam kan
sayımında eozinofil yüzdesi, serum total IgE düzeyleri, Phadiotop infant ve inhalen
çalışıldı. Her hastaya deri prick testi ve 6 yaş üstü olanlara solunum fonksiyon testi
(SFT) yapıldı. Atopik dermatitli hastalarda serum leptin düzeyleri kontrol grubuna
göre yüksek saptandı (p<0,05). Hasta grupları arasında ve kontrol grubu ile
karşılaştırıldığında serum AN düzeyleri açısından farklılık saptanmadı (p>0,05).
Leptin çalışmada araştırılan hiçbir parametre ile korele değildi (p>0,05). Astımlı
hastalarda AN seviyeleri ile IgE arasında pozitif, tüm hasta grubunda FEV 1 ve
BGVA (boya göre vücut ağırlığı) ile negatif, leptin ile BGVA arasında pozitif
korelasyon saptandı (sırasıyla r=0,56, r=-0,31, r=-0,21, r=0,37, p<0,05 hepsi için ).
Atopik hastalarla non-atopik hastalar arasında serum AN ve leptin düzeyleri
açısından fark yoktu (p>0,05). Sonuç olarak atopik dermatitli hastalarda leptin
yüksek olabilir. Sebebinin araştırılması gereklidir. Adiponektin IgE düzeylerini ve
FEV 1’i etkileyebilir.
Two cytokins produced in adipose
tissue, adiponectin (AN) and leptin are candidates for studies because of their
possible role in immunopathogenesis of allergy. The aim of our study is to find out
if there is a relation between atopy and serum adipokine levels. Our study group
consisted of 30 boys and 41 girls, 13 has been followed for asthma, 24 for allergic
rhinitis, and 21 followed both allergic rhinitis and asthma, 13 for atopic dermatitis.
Ages varied between 8 months and 18 years. Control group consisted of 17 healty
children ( 9 girls and 8 boys). AN and leptin levels, percentage of eosinophils in the
complete blood count, serum total IgE levels, Phadiotop infant and inhalents scores
were studied. Skin prick test applied to all patients while pulmonary function test is
applied over 6 years of age. In atopic dermatitis patients serum leptin levels were
higher than control group (p<0,05). There was not a significant difference in serum
AN levels between study groups and control groups (p>0,05). There was not a
significant difference between serum leptin levels and all studied parameters in all
groups (p>0,05). In asthmatic patient group, there was a positive corelation between
serum AN levels and serum IgE levels (p<0,05, r=0,56). While serum AN levels
were negatively correlated with FEV 1, body weight for height (BWH) was
positively correlated with leptin and negatively correlated with in all groups (p<0,05
for all, r=-0,31,r=-0,21,r=0,37). There was not a difference about AN ve leptin
between atopic and non-atopic patients. As a result, leptin levels are high in atopic
dermatitis and the cause can be needed to research. AN can attract IgE levels and
