Klopidogrel direnç mekanizmasını araştırdığımız çalışmamızda özellikle
CYP2C19, CYP3A4, CYP2B6, ABCB1, ITGB3 ve PON1 genlerindeki varyasyonların
bu mekanizma ile olan ilişkisini belirlemeyi amaçladık. Bu amacımızı gerçekleştirirken
klopidogrele dirençli bireylerin ve klopidogrel aktivitesi ile ilişkili varyasyonların
sıklığını Türk toplumunda ortaya çıkarmak ulaşmak istediğimiz hedeflerimiz
arasındaydı.
Çalışmamıza elektif veya acil stent implantasyonlu perkütan koroner girişim
yapılan 223 hasta dahil edildi. En az 7 gün süreyle standart doz (75 mg/gün) klopidogrel
kullanan hastalardan alınan kan örneklerinde VerifyNow P2Y12 kiti ile trombosit
reaktivitesi (PRU) ve % inhibisyon ölçüldü. Farklı tüpe alınan kan örneklerinden izole
edilen DNA’ların genetik varyasyon analizleri için Sequenom MassARRAY sistemi ile
genotipleme protokolleri yürütüldü. Hastaların genetik varyasyon genotipleri, ilaç
kullanım durumu, demografik ve klinik özellikleri trombosit reaktivitesine göre
istatistiksel testler ile analiz edildi.
PRU değeri >208 olan hastalar klopidogrel tedavisine cevap vermeyen
(klopidogrele dirençli) bireyler olarak gruplandırıldığında 69 (%30,9) hastanın
klopidogrele dirençli, 154 (%69,1) hastanın ise klopidogrele dirençsiz olduğu belirlendi.
CYP2C19*2 (G636A) tek nükleotid polimorfizmi ile klopidogrel direnci arasında
anlamlı düzeyde ilişki bulundu (χ2=26,79, p<0,001). Bu polimorfizmin A allelinin
frekansı dirençli hastalarda yüksek olmakla birlikte odds oranı G alleline göre 4,08’idi
(%95 CI 2,39-6,96). CYP2C19*17 (C806T) tek nükleotid polimorfizminin CT
genotiplerinin PRU değerleri CC genotiplerine göre daha düşük (p=0,036), %
inhibisyon değerleri ise CC genotiplerine göre daha yüksekti (p=0,025). Diğer genetik
varyasyonlardan hiçbiri istatistiksel açıdan klopidogrel direnci ve antitrombosit yanıt ile
ilişkili bulunmadı. Hasta grubumuzda CYP2C19*4, CYP2C19*7 ve CYP2C19*8

varyasyon allellerinin herhangi birini taşıyan birey bulunmaz iken, CYP2C19*3 allelini
taşıyanların oranı %0,9’du. Genetik dışı faktörler klopidogrel tedavisine cevap
durumuna göre karşılaştırıldığında ise sadece ileri yaşın klopidogrel direnci ile ilişkili
olduğu belirlendi (p<0,001).
CYP2C19*2 polimorfizminin klopidogrel direncine neden olduğu, CYP2C19*17
polimorfizminin ise klopidogrelin antitrombosit aktivitesini artırdığı çalışmamız ile
doğrulanmış oldu. Ayrıca, klopidogrel metabolizmasından yoksun olduğu bildirilen
CYP2C19*4, CYP2C19*7 ve CYP2C19*8 varyasyonlarının tespit edilmemesi, Türk
toplumuna ait yeni bir bulgu olması nedeniyle önemlidir.
In this study in which we investigated clopidogrel resistance mechanism, we
aimed to determine the association of variations in CYP2C19, CYP3A4, CYP2B6,
ABCB1, ITGB3, and PON1 genes with this mechanism. While reaching this our
objective, to reveal the prevalence of individuals resistant to clopidogrel and of
variations related with clopidogrel activity in the Turkish population were among our
targets.
223 patients who underwent elective or emergency percutaneous coronary
intervention with stent implantation were included in our study. Platelet reactivity
(PRU) and % inhibition were measured with VerifyNow P2Y12 in blood samples
collected from patients that took a standard dose of clopidogrel (75 mg/day) for at least
7 days. For genetic variation analyses of DNAs isolated from blood samples collected in
separate tubes, genotyping protocols were carried out with Sequenom MassARRAY
system. Genetic variation genotypes, drug use statuses, demographic and clinical
characteristics of patients were compared with platelet reactivity via statistical tests.
When grouped the patients with PRU values >208 as nonresponder to clopidogrel
treatment (resistant to clopidogrel), it was determined that 69 (30,9%) patients were
resistant to clopidogrel and 154 (69,1%) patients were nonresistant to clopidogrel. A
significant association was found between CYP2C19*2 (G636A) single nucleotide
polymorphism and clopidogrel resistance (χ2=26,79, p<0,001). A allele frequency of
this polymorphism was high in patients with resistance, its odds ratio was 4,08
compared to G allele (95% CI 2,39-6,96). PRU values of CYP2C19*17 (C806T) single
nucleotide polymorphism CT genotypes were lower (p=0,036) and % inhibition values
of CT genotypes were higher compared to CC genotypes (p=0,025). None of the other
genetic variations was found to be statistically associated with clopidogrel resistance
and antiplatelet response. While individuals carrying any of CYP2C19*4, CYP2C19*7,
CYP2C19*8 variation alleles were not present in our patient group, the rate of

individuals with CYP2C19*3 allele was 0,9%. When the non-genetic factors are
compared with response to clopidogrel treatment, only the higher age was determined to
be associated with clopidogrel resistance (p<0,001).
It was confirmed by our study that CYP2C19*2 polymorphism caused clopidogrel
resistance and CYP2C19*17 polymorphism increased antiplatelet activity of
clopidogrel. In addition, the fact that CYP2C19*4, CYP2C19*7, CYP2C19*8 variations
reported to lack clopidogrel metabolism unable to be detected bears importance in the
sense that it is a new finding on the Turkish population.
