To examine the effects of
erythropoietin on hepatic and renal functions, renal perfusion and clinical findings in
rats that induced septic shock, 24 Sprague Dawley rats were used. The rats were
divided into 3 groups each consisting of 8 rats, which were not discriminated
sexually. The groups were sham, control group and septic shock induced +
erythropoietin administered group. In all rats, femoral veins and arteries were
catheterized for invasive monitorization, drug injection, taking blood samples and
fluid treatment. Also in all rats urinary bladders are canulated. In sham group
catheterization and urinary bladder canulation were performed. In control group
laparotomy+to establish septic shock ceacal ligation and perforation+catheterization
+ urinary bladder canulation were performed. In septic shock induced+erythropoietin
administered (treatment) group laparotomy + to establish septic shock ceacal ligation
and perforation + catheterization + urinary bladder canulation were performed. High
dose erythropoietin (4000 IU\kg) subcutaneously administered to the rats in
treatment group 60 minutes after the ceacal ligation and perforation. Rats were
followed with intervals of 5 minutes (for arterial tension, tachycardia, fever) 14 hours
after the operations. Volume replacement was administered to the rats. Urinary
output was followed by collecting urine for 24 hours just after the beginning of
experiment. As a result renal and hepatic functions, tissue and MDA levels,
laboratory and clinical findings , histological findings of the renal and hepatic
biopsies were better significantly in treatment group. Also in the erythropoietin
administered group arterial tension levels, amount of urine and level of creatinin
clearance compared to the control group were increased significantly. Consequently,
we believe that erythropoietin may be used as a new age agent in septic shock.
eneysel septik şok modeli oluşturulan ratlarda eritropoetinin hepatik ve renal fonksiyonlar, renal perfüzyon ve klinik bulgular üzerine olan etkisini incelemek amacıyla cinsiyet farkı gözetilmeden 24 adet Sprague-Dawley cinsi rat kullanıldı. 8’er adet rattan oluşan sham grubu, kontrol grubu ve septik şok + eritropoetin (tedavi) grubu olmak üzere üç gruba ayrıldı. Tüm ratlara femoral ven ve femoral arter kateterizasyonu uygulandı. Sham grubundaki ratlara kateterizasyon ve mesane kanülasyonu uygulandı. Kontrol grubundaki ratlara laparatomi + çekal ligasyon perforasyon + kateterizasyon + mesane kanülasyonu uygulandı.Tedavi grubuna da laparatomi + çekal ligasyon perforasyon + kateterizasyon + mesane kanülasyonu uygulandı. Tedavi grubundaki ratlara çekal ligasyon-perforasyon işleminden 60 dk sonra subkutan yüksek doz (4000 IU\kg) eritropoetin uygulandı. Postoperatif septik yanıtlar için on dördüncü saatten itibaren ratlar yakın tansiyon, taşikardi ve ateş takibine alındı. Ratlara yoğun volüm replasmanı uygulandı. Ġdrar takipleri deney başlangıcından itibaren 24 saat boyunca idrar toplanarak uygulandı. Sonuç olarak eritropoetin verilen tedavi grubunda diğer gruplara göre renal ve hepatik fonksiyonlarda, doku ve kan MDA düzeylerinde, labaratuvar ve klinik bulgularda, karaciğer ve böbrek dokularından alınan örneklerin histolojik bulgularında anlamlı iyileşmeler olduğu görüldü. Ġdrar takipleri, toplanan idrar miktarları ve kreatinin klerenslerinde anlamlı derecede düzelme olduğu görüldü. Septik şok destek tedavisinde diğer ajanlarla birlikte eritropoetinin yeni nesil bir ajan olarak kullanılabileceği düşüncesindeyiz.
