Osteoporoz, kemik mineral yoğunluğunun (KMY) azalması ve kemik dokunun
mikromimari yapısının bozulması ile karakterize multifaktöriyel bir hastalıktır.
Kemik mineral yoğunluğunu etkileyen birçok çevresel faktör olmasına rağmen son
yıllarda yapılan çalışmalarda genetik yapının osteoporoz patogenezindeki etkisi
üzerinde durulmuştur. Kemik kütlesi üzerine etkisi olan birçok aday genin varlığı
rapor edilmiştir. Ancak bu genlerin kemik kütlesi üzerine olan etkisi ile birbirleri ve
çevresel etkenlerle olan etkileşimleri halen net değildir.
Bu çalışmada, 126 postmenopozal kadında (kemik mineral yoğunluğu
açısından 30 normal, 46 osteopenik ve 50 osteoporotik) lomber omurga ve femur
boynu KMY değerleri ile ERα geni PvuII, XbaI polimorfizmleri ve COL1A1 geni
Sp1 polimorfizmi arasındaki ilişki araştırılmıştır. Ayrıca çalışmamızda bu
polimorfizmlere ait genotip ve alel frekanslarının normal, osteopenik ve osteoporotik
olgulardaki dağılımı incelenmiştir.
Çalışma sonuçlarımıza göre ERα geni PvuII polimorfizmi, pp genotip frekansı
açısından normal olgular ile osteopenik ve osteoporotik olgular arasında anlamlı fark
bulundu. pp genotip frekansı, normal olgularda anlamlı düzeyde düşüktü. Yine ERα
geni PvuII polimorfizmine ait P ve p alel frekansları değerlendirildiğinde
osteoporotik olguların p alel frekansı normal olgulardan anlamlı düzeyde yüksekti.
PP genotipine sahip olguların lomber omurga ortalama KMY değeri, pp genotipli
olguların lomber omurga ortalama KMY değerinden anlamlı düzeyde yüksekti.
Diğer taraftan ERα geni XbaI polimorfizmi ile COL1A1 geni Sp1 polimorfizminde
yapılan değerlendirmelerde gruplar arasında KMY değerleri, genotip ve alel
frekansları açısından fark bulunmadı. Sonuç olarak, çalışma grubumuzdaki
postmenopozal kadınlarda lomber omurga KMY değeri üzerine ERα geni PvuII
polimorfizminin etkili olduğu gösterilmiştir.
Osteoporosis is a multifactorial disease characterized by a decrease in bone mineral
density (BMD) and micro-architectural deterioration of bone structure. Although there are
several environmental influences on BMD, a genetic contribution to the pathogenesis of
osteoroposis has recently been recognized. The existence of many candidate genes which
have effect on bone mass was reported. However, the effect of these genes' on bone mass, the
interaction among these genes and interaction among these genes and enviromental effects are
not presently clear.
In this study, the relationship among BMD values of lomber vertebra and femoral
neck, and ERα gene PvuII, XbaI polymorphisms and COL1A1 gene Sp1 polymorphism in 126
postmenopausal women (30 normal, 46 osteopenic and 50 osteoporotic in terms of bone
mineral density) was researched. Besides, the distribution of genotype and allele frequencies
belonging to these polymorphisms in normal, osteopenic and osteoporotic postmenopausal
women was also evaluated in this study.
According to our study results, significant difference was found in among normal
women, and osteopenic and osteoporotic women in terms of ERα gene PvuII polymorphism
pp genotype frequency. pp genotype frequency was significantly lower in normal women.
When P and p allele frequencies belonging to ERα gene PvuII polymorphism were evaluated,
it was observed that p allele frequency of osteoporotic women was significantly higher than
that of normal women. Lomber vertebra average BMD value of women with PP genotype was
significantly higher than that of with pp genotype. On the other hand, in the evaluations on
ERα gene XbaI polymorphism and COL1A1 gene Sp1 polymorphism, it was noted that there
was no difference in terms of BMD values, genotype and allele frequencies among groups. In
conclusion, it was designated that ERα gene PvuII polymorphism was effective on lomber
vertebra BMD value in postmenopausal women of our study group.
