Kronik lenfositik lösemi yetişkinlerde en sık gözlenen lösemi
türü olup, sıklıkla tekrar eden bazı kromozomal anomalilerin
prognostik önemleri tanımlanmıştır. En sık gözlenen anomali
delesyon 13 (del(13q)) olup, izole göründüğünde iyi prognostik
biyobelirteç olarak kabul edilmektedir. Ancak izole del(13q) saptanan
KLL olgularının klinik bulgularının oldukça heterojen olduğu
gözlenmektedir. Bizim çalışmamızda izole del(13q) saptanan
olgularda del(13q) büyüklüğünün ve genomdaki diğer kopya sayısı
değişiklikleri (KSD) ve heterozigosite kayıplarının (LOH) klinik
heterojenite üzerindeki etkisinin araştırılması amaçlanmıştır.
Çalışmamıza FISH yöntemi ile izole del(13q) saptanan otuz iki
KLL olgusu dahil edilmiş olup, otuz olgunun periferik kan
örneklerinden elde edilen DNA örneklerinde aCGH+SNP yöntemi ile
KSD ve LOH’lar analiz edilmiştir.
Çalışmamız sonucunda 28/30 olguda del(13q) saptanmıştır.
Delesyon büyüklüğü 0,34 - 28,81 Mb arasında değişkenlik göstermiştir
ve delesyon tipi (tip ve tip 2 ) ile klinik bulgular arasında bir ilişki
saptanmamıştır. Olgularımızdan 1’i izole del(13q) iken kalan olguların
birinde del(13q) dışında 1 kromozom bölgesinde, 4 olguda 2 kromozom
bölgesinde KSD saptanmış olup, diğer olgularda en az 3 KSD tespit
edilmiştir. Bir olguda saptanan en fazla KSD altmış bir adettir.
Yaptığımız çalışma sonucunda del(13q) büyüklüğünün oldukça
heterojen olduğu ve içerdiği genler açısından da heterojenite
gösterdiği saptanmıştır. Genomda diğer KSD’ler arasında nadir
KSD’ler saptanmış olup, ayrıca 1 olguda ATM gen bölgesini içeren
LOH tespit edilmiştir. Saptanan KSD’ler arasında 16p13.3, NOTCH,
COL1A1 ve 2p artışlarının ve 18p11.32 kayıplarının prognostik
önemlerinin olabileceği sonucuna varılmıştır. Sonuç olarak, kopya
sayısı değişikliklerinin, KSD büyüklüklerinin ve LOH analizlerinin
KLL patogenezinin açıklanması için önemli olması sebebiyle array
iii
yönteminin oldukça etkili olduğu savunulmuştur. Ayrıca delesyon 13q
büyüklüğünün ve saptanan diğer KSD’lerin prognostik etkisinin açığa
kavuşması için daha fazla çalışmaya ihtiyaç duyulduğu sonucuna
varılmıştır
Chronic lymphocytic leukemia (CLL) is the most common leukemia in
adults, and prognostic value of frequently recurrent chromosomal
abnormalities are discovered. Deletion of chromosome 13 [del(13q)] is the most
common chromosomal anomaly in CLL and it is considered as a good
prognostic biomarker when it is observed isolated. But it is also noticed that
clinical findings of patients with isolated del(13q) are quite heterogenous. In
our study, we aimed to investigate the relation between this clinical
heterogenity and size of del(13q) and other copy number variations (CNV) and
loss of heterozygosities (LOH) in CLL patients with del(13q).
In this study, 32 CLL patients with isolated del(13q) determined by
FISH method included and CNV & LOH analysis were performed by
aCGH+SNP method in DNA obtained by peripheral blood sample of 30
patients.
We found that 28/30 patients had del(13q). The size of deletions ranged
between 0,34 and 28,81 Mb, no correlation was found between the type of
deletions (type 1 / type 2 ) and clinical findings. Only one of our patients had
isolated del(13q), one had other CNV in a chromosomal region, 4 had in 2
chromosomal regions other patients had at least 3 CNVs except for del(13q).
The largest number of CNVs found in a patient was sixty one.
It is concluded that the size of del(13q) and the genes involved were very
heterogenous. The other CNVs found were rare CNVs, and one patient had
LOH involving ATM gene region. Out of the found CNVs; it is concluded that
gain of 16p13.3, NOTCH, COL1A1, 2p and loss of 18p11.32 may have a
prognostic value. Last of all, it is defended that array method is considerably
effective because analyses of CNVs, size of CNVs and LOHs are important to
explain the pathogenesis of CLL. Also, more studies are needed to figure out
the prognostic value of 13q deletion size and other detected CNVs
