Nefrotik sendromda serum ve idrar ghrelin, sCD80 ve sCTLA-4’ün rolleri ile ilgili sınırlı sayıda çalıs¸ma bulunmaktadır. Bu çalıs¸mada serum ve idrar ghrelin, sCD80 ve sCTLA-4 düzeylerinin ölçümü, bu belirteçlerin hastalı˘gın immünolojik temeli ve etyopatogenezindeki rollerininortayakonmasıamaçlanmıs¸tır.Çalıs¸mayaadriamisiniledeneyselnefrotik sendrom (NS) olus¸turulan 15 sıçan ve kontrol grubu olarak 13 sıçan alındı. Serum ve idrar ghrelin, sCD80 ve sCTLA-4 düzeyleri EL˙ISA yöntemi ile çalıs¸ıldı. Çalıs¸ma grupları arasında serum ghrelin düzeyleri açısından fark saptanmadı (p>0,05). NS’ligruptaidrarghrelindüzeylerikontrolgrubunagörebelirgindüs¸üksaptandı(sırasıyla;0,026[0,012-0,19]ng/mlve0,68[0,39-1,22]ng/ml,p=0,0001).Serumve idrar sCD80 düzeylerinin NS’li grupta kontrol grubuna göre belirgin yüksek oldu˘gu görüldü(sırasıyla;514,01±234,20ng/mlve289,05±80,90ng/ml,p=0,003),(sırasıyla; 152,48 [114,36 - 265,27] ng/ml ve 10 [9,34 - 53,94] ng/ml, p=0,0001). Serum ve idrar sCTLA-4 düzeylerinin de NS’li grupta kontrol grubuna göre belirgin yüksek oldu˘gu görüldü (sırasıyla; 7,70 [5,02 - 10,58] pg/ml ve 3,88 [2,84 - 6,18] pg/ml, p=0,019),(sırasıyla;9,64[7,87-27,34]pg/mlve2,07[0,88-6,27]pg/ml,p=0,001). Çalıs¸ma gruplarında (NS ve kontrol) serum ghrelin düzeyleri ile çalıs¸ılan herhangi bir parametre arasında korelasyon saptanmadı (p>0,05). Çalıs¸ma gruplarında idrar ghrelin düzeyleri ile serum total protein (r=0,590, p=0,002) ve serum albümin (r=0,582, p=0,002) düzeyleri arasında pozitif, idrar protein (r=-0,837, p=0,0001) ve idrar sCD80 (r=-0,867, p=0,0001) düzeyleri arasında negatif korelasyon saptandı. Çalıs¸ma gruplarında idrar sCD80 ve idrar sCTLA-4 düzeyleri (r=0,738, p=0,002) arasında pozitif korelasyon saptandı . Çalıs¸ma gruplarında serum sCTLA-4 ve serum albümin düzeyleri (r=-0,801, p=0,001) arasında negatif; idrar sCTLA-4 ve 24 saatlik idrarda proteinüri düzeyleri (r=0,591, p=0,02) arasında pozitif korelasyon oldu˘gu görüldü. Sonuç olarak ghrelinin nefrotik sendromda immünolojik süreçte rolünün oldu˘gu, böbrekte meydana gelen patoloji nedeni ile buradaki ghrelin üretiminde azalma oldu˘gu, sCD80 düzeylerindeki artıs¸ın ghrelindeki düs¸üs¸ ile ilis¸kili olabilece˘gi, sCTLA-4 düzeylerindeki artıs¸ın da membran CTLA-4’ün T hücre aktivasyonu üzerine olan inhibitör etkisini ortadan kaldırmıs¸ olabilece˘gi, tüm bu mekanizmaların neticesinde de proteinürinin meydana geldi˘gi düs¸ünülmüs¸tür.

There are limited number of studies on the roles of ghrelin, sCD80 and sCTLA-4 in nephrotic syndrome (NS). The current study aims at measuring the levels of serum and urine ghrelin, sCD80 andsCTLA-4levelsandidentifyingtherolesofthesedeterminantsonimmunological basis and etiopathogenesis of the illness. For the purposes of the study, a total of 15 rats were exposed to experimental nephrotic syndrome by injecting adriamisin and as for the control group, 13 rats, were used. Ghrelin, sCD80 and sCTLA-4 were analyzed by using ELISA method. No significant difference was found between the groups in terms of serum ghrelin levels (p>0.05). As for urine ghrelin levels, NS group was found to have significantly lower levels of ghrelin compared to control group (0.026 [0.012 - 0.19] ng/ml and 0.68 [0.39 - 1.22] ng/ml, p=0.0001). In addition, serum and urine sCD80 levels were found to be higher in NS group compared to the control group (514.01 ± 234.20 ng/ml and 289.05 ± 80.90 ng/ml, p=0.003), (152.48 [114.36 - 265.27] ng/ml and 10 [9.34 - 53.94] ng/ml, p=0.0001). Similarly, serum and urine sCTLA-4 levels were considerably higher in NS group (7.70 [5.02 - 10.58] pg/ml and 3.88 [2.84 - 6.18] pg/ml, p=0.019), (9.64 [7.87 27.34] pg/ml and 2.07 [0.88 - 6.27] pg/ml, p=0.001). No correlation was found between the parameters studied and serum ghrelin levels in study groups (p>0.05). In groups positive correlations were found between urine ghrelin and serum total protein levels (r=0.590, p=0.002) and serum albumin levels (r=0.582, p=0.002), negative correlations were found in urine protein levels (r=-0.837, p=0.0001) and urine sCD80 levels (r=-0.867, p=0.0001). A positive correlation was found in groups urine sCD80 and urine sCTLA-4 levels (r=0.738, p=0.002). In groups, a negative correlation was found between serum albumin and serum sCTLA-4 levels (r=-0.801, p=0.001), a positive correlation between urine sCTLA and proteinuria levels in 24-hour urine (r=0.591, p=0.02). The study concludes that ghrelin plays a role during immunological process in nephrotic syndrome and ghrelin levels decreaseduetothepathologythatoccursinthekidneys.Similarly,theincreaseinserum and urine sCD80 levels might be related to the decrease in ghrelin levels; and the increase in sCTLA-4 levels might have removed the inhibitor effect of CTLA-4 on T-cell activation which finally led to proteinuria.
