Eskişehir, 2013. Çalışmamızda obezitenin, vücut kitle indeksi ile değerlendirilerek
spinal anestezi seviyesini ve süresini nasıl etkilediğini araştırmayı amaçladık.
Çalışmaya ASA I-II grubuna giren, spinal anestezi planlanan, alt ekstremite cerrahisi,
vasküler cerrahi ve ürolojik cerrahi uygulanacak hastalar dahil edildi. Yaşları, 18-65
arasında değişen 45 olgu çalışmaya alındı. Hastaların VKİ’leri, vücut ağırlığı/boyun
karesi (kg/m2
) formülü ile hesaplandı. Buna göre hastalar, Grup 1 VKİ’i 18.5-24.9
(normal kilolu), Grup 2 VKİ’i 25-29.9 (aşırı kilolu) ve Grup 3 VKİ’i 30-34.9 (obez)
olan olgular olarak 15’er kişilik 3 gruba ayrıldı. Hastaların kan basıncı, kalp atım
hızı, EKG ve SpO2 monitörizasyonları yapıldı. Hastalara sağ veya sol yan pozisyon
verilerek L4-L5 seviyesinden 25 numaralı Quincke iğnesi ile subaraknoid aralığa
girilerek 7,5 mg hiperbarik bupivakain verildi. Uygulama sonrası hastalar supin
pozisyona çevrildi. Spinal anesteziden hemen sonraki, spinal anestezi sonrası 3., 5.,
7., 10., 15. ve 20. dakikalardaki duyusal blok seviyeleri pinprick metoduyla, motor
blok dereceleri ise Bromage skalası ile değerlendirildi ve kaydedildi. Tüm hastaların
maksimum duyusal blok seviyeleri, motor blok dereceleri, T10 duyusal blok
seviyesine ulaşma süresi, maksimum duyusal blok zamanı, maksimum duyusal
bloğun 2 seviye gerileme ve motor bloğun kalkış zamanı kaydedildi. İstatistiksel
olarak Grup 3’dekilerin, Grup 1 ve 2’dekilere göre duyusal blok seviyeleri anlamlı
yüksek, T10 blok zamanı anlamlı kısa, duyusal bloğun 2 seviye gerileme zamanı ile
motor bloğun kalkış zamanı anlamlı uzun idi. Grup 3’dekilerin Grup 1’dekilere göre
maksimum duyusal blok süresi anlamlı kısa bulundu. Sonuçta standart tek doz
verilen lokal anesteziğin obezlerde daha yüksek blok seviyeleri oluşturması ve blok
seviyelerinin daha geç gerilemesi, bu hastalarda doz ayarlaması yapılması gerektiği,
doz ayarlaması yapılabilmesi için ise ek çalışmalara gerek olduğu kanaatine varıldı.
Çulha, E. Evaluation The effect of obesity on the spinal anesthesia level and
duration by regarding body mass index (BMI). Eskişehir Osmangazi University
Faculty of Medicine Anesthesiology and Reanimation Department Expertise in
Medicine Thesis, Eskişehir, 2013. We aimed to research how the obesity effect on
the spinal anesthesia level and duration by regarding BMI. The patients who were in
ASA I-II group, planned to spinal anesthesia, lower extremity surgery, vascular
surgery, and urological surgery, were included to this study. The age between 18-65
45 different cases included the study. The patients BMI calculated by dividing body
weight to body length square (kg/m2
). According to this calculation, the patients
were separated three groups of fifteen people by their BMI. Group 1 BMI:18,5-24,9
(normal weighted), Group 2 BMI:25-29,9 (over-weighted) and Group 3 BMI:30-34,9
(obese). Blood pressure, heart rate, ECG and SpO2 were monitored. By entering the
subarachnoid space, hyperbaric bupivacaine with 25 numbered Quincke needle was
given to patients who were in the left or right lateral position, at the level of L4-L5.
After application, the patients were turned the supine position. In the process
immediately after spinal anesthesia, its the 3rd, 5th, 7th, 10th, 15th and 20th minutes,
sensory block levels were assessed in the method of pinprick and the degrees of
motor block were assessed and recorded by the Bromage scale. All patients’
maximum sensory levels, motor blocks degrees, time to reach T10 sensory block
level, maximum sensory block time, two level recession of maximum sensory block
time and liftoff of motor blocks time were recorded. Statistically, group 3 patients’
sensory block levels were higher meaningfully, T10 block time meaningfully shorter
meaningfully, two level recession of sensory block time and liftoff of motor block
time longer meaningfully than the other group patients’. Group 3 patients’ maximum
sensory block time were shorter meaningfully than group 1 patients’. As a result, it
was concluded that single dose local anesthesia generated higher block levels and
made block levels degraded later in obese patients’ cases so it needed adjustment for
obese patients and for that additional study must do about this subject.
