Over kanseri; geli$mi$ ülkelerde kadınlarda rastlanan kanserler sıralamasında be$inci en sık kanserle ili$kili ölüm sebebidir ve kadın genital sistem kanser ölümlerinde birinci sırayı almı$tır (1). Bunun nedeni, hastalı%ın ço%unlukla ileri evrelerde tanı alması ve erken semptom vermemesi olarak söylenebilir. Epitelyal over kanserlerinde tedavi asıl olarak; cerrahi ve medikal tedavidir. Over kanseri jinekolojik kanserlerin içerisinde kemoterapiye en iyi cevap veren kanserler arasındadır. Standart tedavi platin tabanlıdır. Erken evre over kanserli hastalarda da ilk yapılan cerrahi rezeksiyona genellikle kemoterapi eklenir. Tüm evreler göz önünde bulunduruldu%unda ilk tedavi sonrasında da relaps ihtimali % 6O’lar civarında saptanmı$ olup, ileri evrelerde bu oran % 80-85’tir. Bu yüzden dikkatler karsinogenezle alakalı kritik hücre yollarını hedef alan biyolojik ajanlara çevrilmi$tir (4-8). Tez kapsamında kemoterapatiklerle kombine kullanımlarda alternatif olması amaçlı kullandı%ımız ye$il çay derivesi olan EGCG’nin (epigallocatechin 3 gallate) Ovcar-3 hücre kültüründe apoptotik etkisini tespit için Tetrazolium Testi (MTT testi) ve DAPI (4-6 diamidino- phenylindole) boyama yaptık. Ardından dört farklı gen üzerindeki etkilerini de%erlendirdik. Ovcar-3 hücre kültürü üzerinde EGCG’nin farklı dozlarının, hem tek ba$ına uygulandı%ında hem de klasik kemoterapatikler olan paklitaksel ve karboplatinin faklı dozlarıyla yapılan kombine uygulamaları sonucunda kanser hücrelerini anlamlı $ekilde yok etti%ini saptadık. Üstelik kombine kullanımda tek ba$ına olan etkileri potansiyelize ederek toplamda daha fazla hücreyi apaptozise u%rattı. Yine proliferasyondan sorumlu oldu%u bilinen VEGF ve Ki-67 üzerinde de anlamlı azalmaya sebep oldu. Klasik kemotreapatiklere direnç geli$en olgularda, kemoterapatiklerin EGCG ile kombine kullanılmasının tedavi etkinli%ini arttırdı%ını saptadık. Bu bulgumuzun etkinli%inin klinik çalı$malarla desteklenmesinin uygun oldu%u sonucuna vardık.

Ovarian cancer is the most common cause of death among women with gynecologic cancer and fifth leading cause of death after lung, breast, colorectal and pancreas cancers in women living in developed countries (1). The reason is considered to be its being diagnosed generally in its late stages and not giving any symptoms in the early stages. Primary treatment for ovarian cancer consist of surgical cytoreduction and paclitaxel/carboplatin combination chemotherapy. Supportive therapy is applied to patients in terminal stage of the disease. Ovarian cancer responds the best to the chemotherapy among the other gynecologic cancers. Taking into account the frequency of each stage of disease and its projected relapse rate, the overall likelihood of relapse after initial therapy for all stages of disease for women with ovarian cancer is 62 percent, it is 80-85 percent for women who present with stage III or IV disease. Fort this reason, a variety of emerging agents with novel targets are currently in development of ovarian cancer (4-8). EGCG; is an extract of green tea, and can inhibit tumorigenesis during the initiation, promotion and progression stages. In this thesis, we performed Tetrazolium test (MTT test) and DAPI to demonstrate the apoptotic effect of EGCG on Ovcar-3 cell culture. Then, we evaluated its effects on four different genes with real time PCR. The resulting observations are; different doses of EGCG which was used both solely and in combination with paclitaxel and carboplatin, resulted in a significant eradication in cancer cells. Furthermore, when it is used in combination, could potantiate the effects of paclitaxel and carboplatin and would induce apoptosis in more ovarian cancer cells. In addition It also caused a significant decrease on VEGF and Ki-67 which are known to be responsible for proliferation. These results show that EGCG, when used in combination with other conventional chemotherapeutics, is a promising candidate as an alternative treatment.
