Kronik lenfositik lösemi (KLL) lenfoid seriden köken alan ve
karakteristik genetik değişikliklerin bulunduğu bir lösemi tipi olarak
tanımlanmaktadır. Kronik lenfositik lösemide genetik değişiklikler,
kromozomal aberasyonlar ve gen mutasyonlarıdır. Literatürlerde en
sık gözlenen kromozomal aberasyonlar ise 13. ve 11. kromozumun
uzun kol delesyonu (del13q, del11q), 17. kromozomun kısa kol
delesyonu (del17p) ve 12. kromozomun trizomisi olarak bildirilmiştir.
Belirtilen anomalilerden izole del13q iyi prognostik olarak kabul
edilmektedir. Fakat bazı literatürlerde 13. kromozomun uzun
kolundaki delesyonun büyüklüğü ile delesyona ek anomalilerin
varlığının prognozu etkilediği bildirilmiştir. Kronik lenfositik lösemi
belirtilen kromozomal aberasyonların yanında sıklıkla NOTCH1,
SF3B1, BIRC3, TP53 ve XPO1 genlerinde mutasyonlar saptanmıştır.
Belirtilen genler içinde NOTCH1 ve SF3B1 genlerinin hücre
siklüsünde, apoptoz ve RNA splicing gibi hücre işlevinde önemli
mekanizmalarda görevli olduğu bilinmektedir. Bu nedenle NOTCH1
ve SF3B1 genlerindeki mutasyonların hastalığın patogenezinde
önemli etkilerinin olması beklenilmektedir. Ek olarak literatürlerden
elde edinilen bilgiler doğrultusunda belirlediğimiz genlerin
hastaların tedaviye başlama süreleri ve sağkalım süresi üzerine
etkileri bulunduğu da bildirilmektedir. Biz de çalışmamızda FISH
yöntemiyle belirlediğimiz izole del13q saptanan kırk üç hastada kötü
prognostik etkisi olan NOTCH1 ve SF3B1 genlerinde mutasyon
profilini Sanger Sekanslama yöntemiyle inceledik. Belirtilen hasta
grubunda NOTCH1 genininde en sık görülen 7541_7542delCT
mutasyonunu bir hastada (%2.4) saptarken, SF3B1 geninde mutasyon
saptayamadık.

Çalışmamızda SF3B1 ve NOTCH1 genlerinde hasta sayısının ve
mutasyon oranlarının az olması nedeni ile del13q oranları, RB1 gen
delesyon varlığı, hastalık evreleri, tedaviye başlama süresi ve OS
parametreler ile ilişkisi istastiksel olarak değerlendirilememiştir.
Ancak olgu serimizde sadece 1 hastada NOTCH1 geninde mutasyon
bulunması; ilgili mutasyon ile del13q hasta grubu arasında bir
ilişkinin olmadığını düşündürmektedir. Yapılan literatür
değerlendirmesinde çalışmamız Türk populasyonunda FISH yöntemi
ile izole del13q saptanan KLL hastalarında ilgili genlerin mutasyonel
durumunu değerlendiren ilk çalışma olarak görülmekte ve bu yönü
ile literatüre katkı sağlamaktayız
Chronic lymphocytic leukemia (CLL) is defined as a type of
leukemia originating from the lymphoid series with characteristic
genetic changes. Genetic changes, chromosomal aberrations and gene
mutations in chronic lymphocytic leukemia. The most common
chromosomal aberrations in the literature have been reported as
deletion of the long arm of the 13th and 11th chromosomes (del13q,
del11q), deletion of the short arm of the 17th chromosome (del17p)
and trisomy of the 12th chromosome. Del13q isolated from the
mentioned anomalies is considered to be good prognostic. However, in
some literature, it has been reported that the size of the long arm of
chromosome 13 and the presence of additional anomalies affect the
prognosis. In addition to chromosomal aberrations with chronic
lymphocytic leukemia, mutations in the genes of NOTCH1, SF3B1,
BIRC3, TP53 and XPO1 are frequently detected. Among the mentioned
genes, NOTCH1 and SF3B1 genes are known to be involved in cell
cycle, apoptosis and important mechanisms in cell process such as
RNA splicing. Therefore, mutations in NOTCH1 and SF3B1 genes are
expected to have important effects on the pathogenesis of the disease.
In addition, it has been reported that the genes we identified in
accordance with the information obtained from the literature have
effects on the duration of treatment and survival time of the patients.
In our study, we investigated the mutation profile of NOTCH1 and
SF3B1 genes with poor prognostic effect in forty-three patients with
isolated del13q detected by FISH method by Sanger Sequencing
method. We detected 7541_7542delCT mutation in NOTCH1 gene in
one patient (2.4%), but we could not detect mutations in SF3B1 gene.

In our study, due to the low number of patients and mutation
rates in SF3B1 and NOTCH1 genes, del13q rates, presence of RB1
deletion, disease stages, duration of treatment and OS parameters
could not be evaluated statistically. However, in our case series, only
1 patient had mutations in NOTCH1 gene; suggest that there is no
relationship between the relevant mutation and the del13q patient
group. In the literature review, our study is seen as the first study
evaluating the mutational status of related genes in CLL patients with
isolated del13q detected by FISH method in Turkish population and
we contribute to the literature with this aspect
