Etanol, stres ve nonsteroidal antiinflamatuvar ilaçların neden
olduğu mide mukozası hasarı ciddi bir tıbbi problemdir. NO, H2S ve CO gaz
otakoidlerdir ve tıbbi amaçlar için çeşitli alanlarda kullanılırlar. NO'nun
gastrik mukoza üzerinde koruyucu etkilerinin olduğu bilinmektedir. Son
zamanlarda bazı çalışmalar H2S ve CO'nun da mide koruyucu etki gösterdiğini
belirtmektedir. Bu çalışma, bu gaz otakoidlerin etanol, indometasin ve stres
kaynaklı gastrik ülser modellerinde koruyucu etkilerini değerlendirmek ve
karşılaştırmak amacıyla planlanmıştır.
Metodlar: Üç farklı otakoid prekürsör, NO için L-arginin (100 mg / kg,
ip), H2S için NaHS (5 mg / kg, ig), CO için CORM-2 (Karbonmonoksit
salgılayıcı molekül-2 = trikarboniildichlororuteniyum (II) dimer) (5 mg / kg,
ig), nin olası gastroprotektif etkileri üç farklı mide ülseri modeli olarak etanol
1 ml% 96 ig, indometasin (40 mg / kg ig) ve stres (soğuk + hareketsizlik)
üzerinde incelenmiştir. Sonuçlar ülser indeksi, gastrik mukus sekresyonu,
serbest ve total asidite, MDA GSH, protein olmayan sülfhidril grupları (NP SH), TNF alfa, PGE2, COX-1, COX-2 düzeyleri ölçülerek değerlendirilmiştir.
Bulgular: CORM-2 ve NaHS'nin etanol ile indüklenen ülserlere karşı
gastroprotektif aktivite gösterdiği bulunmuştur. L-arginin ayrıca strese bağlı
ülserlere karşı gastroprotektif olmuştur. Etanol grubunda CORM-2 ve NaHS
ile mukus sekresyonunun indüksiyonu ve MDA, TNF-α seviyelerinin azalması
gözlenmiştir. Stres grubuna L-arginin verildiğinde total asidite ve TNF-α
düzeylerinde azalma meydana gelmiştir. İndometasin, mukus seviyesini
önemli ölçüde yükseltmesine rağmen asiditeyi arttırarak belirgin olarak
gastrik ülser oluşumunu artırmıştır. Bu ajanların hiçbiri, indometasinin
neden olduğu ülserlere karşı gastroprotektif etki göstermemiştir.

Sonuçlar: CO ve H2S'nin etanol ile indüklenen ülserlere karşı
gastroprotektif etkileri olabileceği görülmektedir. Bu çalışma, yeni
antiülserojenik etki gösteren ajanların gelişimine ve bu aktivitelerin altında
yatan mekanizmalara katkıda bulunabilir
Mucosal damage induced by ethanol, stress and
nonsteroidal antiinflammatory drugs is serious medical problem. NO, H2S and
CO are gaseous autacoids and they are used in several areas of real-life for
medical purposes. It is known that NO has protective effects on gastric
mucosa. Recently some studies indicated that H2S and CO also have
gastroprotective effects. This study was planned to evaluate and compared the
protective effects of these gaseous autacoids on ethanol, indomethacin and
stress-induced gastric ulcer models.
Methods: The gastroprotective effects of three different autacoid
precursors, L-arginine (100 mg/kg, i.p.) for NO, NaHS (5 mg/kg, i.g) for H2S,
CORM-2 (Carbonmonoxide releasing molecule 2=tricarbonyldichlororuthenium (II) dimer) (5 mg/kg, i.g) for CO were
examined on three different gastric ulcer models (ethanol 1ml 96% i.g.),
indomethacin (40 mg/kg i.g.) and stress (cold+immobility). The results were
evaluated by measuring ulcer index, gastric mucus secretion, free and total
acidity, the levels of MDA, GSH, non-protein sulfhydryl groups (NP-SH), TNF
alpha, PGE2, COX-1, COX-2.
Results: It was found that CORM-2 and NaHS showed gastroprotective
activity against ethanol-induced ulcers. The significant induction of mucus
secretion and reduction of MDA, TNF-α levels were observed in the ethanol
group by CORM-2 and NaHS. The inhibition of total acidity and TNF-α levels
was determined in the stress group by L-arginine. Although significant and
increased levels of mucus were noticed by indomethacin, increases of acidity
stimulated significantly gastric ulcers. None of these agents had
gastroprotective activity against indomethacin-induced ulcers.
Conclusions: It seems that CO and H2S might have gastroprotective
effects against ethanol induced ulcers and NO against stress-induced ulcers.
