Şok, sepsis, böbrek nakli, aort anevrizması gibi klinik durumlar ile kısmi
nefrektomi, kardiyopulmoner bypass benzeri bazı cerrahi girişimler sırasında
böbreğe giden kan akımının azalması veya durması böbrekte iskemi oluşumuna
sebep olur. Sonrasında böbreğin yeniden kanlanması ise reperfüzyon olarak
adlandırılır. Hem iskemi hem de reperfüzyon periyodları böbrekte ciddi bir
hasar oluşturur. Çalışmamızın amacı böbrekte iskemi/reperfüzyon ile
oluşturulmuş hasar üzerine A vitaminin morfolojik ve antioksidan etkilerini
araştırmaktır.
Çalışmada 40 Sprague- Dawley sıçan her bir grup 8’er hayvan olacak
şekilde 5 gruba ayrıldı. Kontrol grubu hariç tüm gruplarda sol böbreğe 45
dakikalık iskemi ve bunu takiben reperfüzyon uygulandı. 24 saatlik iyileşme
sürecinin ardından 1. gruba 7 gün süreyle 1.000 IU/kg/gün, 2. gruba 3.000
IU/kg/gün, dozunda 3. gruba ise 9.000 IU/kg/gün dozunda oral gavaj yoluyla A
vitamini uygulaması yapıldı. 4. gruba iskemi/reperfüzyon uygulamasından
sonra 7 gün boyunca oral gavaj yoluyla ayçiçek yağı verildi. Kontrol grubuna ise
normal değerleri elde etmek amacı ile hiçbir uygulama yapılmadı, sıçanlar
standart pelet yem ile ad libitum beslendi. 7 günlük uygulamanın sonunda
intrakardiyak kan örnekleri ve sol böbrek alındı. Kan örneklerinden süperoksit
dismutaz (SOD), malondialdehit (MDA) ve katalaz (KAT) seviyeleri belirlendi.
Doku örnekleri ise hematoksilen/eozin ile histokimyasal olarak boyandı ve
böbrek histolojisindeki değişiklikler ışık mikroskobunda incelendi.
Çalışma sonucunda böbrek histolojisinde A vitamini ile tedavi edilen
gruplarda iskemi/reperfüzyon grubuna göre böbrekte oluşan hasar azaldı. SOD,
MDA ve KAT değerlerinin ise A vitamini uygulanan gruplarda kontrol grubu
değerlerine yaklaştığı gözlendi.
Sonuç olarak, böbrekte iskemi/reperfüzyon sonrasında yapılan A vitamini
uygulaması histolojik hasarı azaltıcı ve antioksidan değerleri restore edici bir
etki ortaya çıkarmıştır
In some clinical conditions such as shock, sepsis, kidney transplantation,
aortic aneurysm, and during surgical interventions such as partial
nephrectomy, cardiopulmonary bypass decreased or ceased blood flow to the
kidney can cause renal ischemia. Then, re-supply of blood to the kidney is called
reperfusion. Both ischemia and reperfusion periods cause serious damage to the
kidney. The aim of this study is to investigate the morphological and antioxidant
effects of vitamin A on ischemia / reperfusion injury in the kidney.
In the study, 40 Sprague-Dawley rats were divided into 5 groups, each
group being 8 animals. The rats in all groups except the control group
underwent ischemia of the left kidney for 45 minutes followed by reperfusion.
After 24 hours of recovery, vitamin A was administered to the 1st group by oral
gavage at a dose of 1.000 IU/kg/d for 7 days, to the 2nd group with 3.000 IU/kg/d,
and to the 3rd group with 9.000 IU/kg/d. Group 4 received sunflower oil by oral
gavage for 7 days after ischemia / reperfusion. In the control group (5th group),
no application was performed to obtain normal values, rats were fed ad libitum
with standard pellet feed. At the end of 7 days of application, intracardiac blood
samples and left kidney were collected. Superoxide dismutase (SOD),
malondialdehyde (MDA) and catalase (CAT) levels were determined from blood
samples. Tissue samples were stained histochemically with hematoxylin/eosin
and changes in renal histology were examined under light microscope.
As a result of the study, renal damage was decreased in vitamin A treated
groups compared to ischemia/reperfusion group. It was observed that SOD,
MDA and CAT values were closer to the control group values in vitamin A
groups compared to those of ischemia/reperfusion group.
Consequently, the administration of vitamin A after renal
ischemia/reperfusion resulted in reducing the histological damage and restoring
the antioxidant values
