Familial Mediterranean Fever is common in our country, showing autosomal
recessive transition; it is an inflammatory disease characterized by recurrent attacks
in the form of fever, abdominal pain, joint pain, chest pain. Neurological
involvements are rare in FMF patients. The aim of our study was to evaluate the
neurological findings in FMF patients. 625 patients whose MEFV gene mutation was
found positive, were included in the study and their files were scanned
retrospectively. The mean age of patients was 13.02 ± 5.18 years. Neurological
symptoms were detected in 23.2% of the patients. The most observed neurological
symptom was headache (11.8%). Neurological disease accompanying FMF was
detected in 6,19% of the patients. Epilepsy was the most common neurological
disease and was found in 2.88% of patients. The prevalence of neurological diseases
was higher than the society. It was observed that the frequency of neurological
symptoms was lower in children under the age of 5 years (p<0,05). The frequency of
neurological symptoms was higher in patients with E148Q mutation in at least one
allele; It was lower in those with M694V mutation (p<0,05). No relation was found
between the phenotype features of the disease and neurological findings.
Approximately half of the patients had a comorbid disease accompanying FMF.
Neurological symptoms were more frequent in patients with cardiac anomaly and
rhythm disorder and JIA (p<0,05). There was no correlation between laboratory
values and subclinical inflammation and neurological findings. In conclusion, early
recognition of the disease and early treatment reduce the frequency of neurological
signs and symptoms, and prevent complications that may be observed in the long term course of the disease. That‟s why we emphasize the importance of systemic
questioning, neurological history and examination in the follow-up patients
Ailevi Akdeniz AteŞi ülkemizde sık görülen otozomal resesif geçiŞ gösteren; ateŞ, karın ağrısı, eklem ağrısı, göğüs ağrısı Şeklinde tekrarlayan ataklar ile karakterize inflamatuvar bir hastalıktır. Nörolojik tutulumlar AAA hastalarında nadir görülmektedir. ÇalıŞmamızın amacı AAA hastalarında nörolojik bulguların değerlendirilmesiydi. ÇalıŞmaya MEFV gen mutasyonu pozitif saptanmıŞ 625 hasta dahil edildi ve retrospektif olarak dosyaları tarandı. Hastaların ortalama yaŞı 13,02 ± 5,18 yıl idi. Hastaların %23,2‟sinde nörolojik semptom belirlendi. En sık gözlenen nörolojik semptom baŞ ağrısı (%11,8) idi. Hastaların %6,19‟unda AAA‟ya eŞlik eden nörolojik hastalık tanısı tespit edildi. Epilepsi en sık gözlenen nörolojik hastalık idi ve hastaların %2,88‟inde bulundu. Nörolojik hastalıkların prevalansı topluma kıyasla daha yüksek bulundu. Tanı yaŞı 5 yaŞın altında olan çocuklarda nörolojik semptom görülme sıklığının daha düŞük olduğu görüldü (p<0,05). En az bir allelde E148Q mutasyonu olan hastalarda nörolojik semptom görülme sıklığı daha yüksek, M694V mutasyonu olanlarda ise daha düŞük bulundu (p<0,05). Hastalığın fenotip özellikleriyle nörolojik bulgular arasında iliŞki saptanmadı. Hastaların yaklaŞık yarısında AAA‟ya eŞlik eden bir komorbid hastalık tanısı vardı. JĠA ile kardiyak anomali ve ritim bozukluğuna sahip olan hastalarda daha sık nörolojik semptom belirlendi (p<0,05). Laboratuvar değerleri ve subklinik inflamasyon ile nörolojik bulgular arasında korelasyon saptanmadı. Sonuç olarak hastalığın erken tanınması ve tedaviye erken baŞlanması nörolojik semptom ve bulguların görülme sıklığını azaltmakta hastalığın uzun dönem seyrinde gözlenebilecek komplikasyonları önlemektedir. Bu nedenle hastaların izleminde sistemik sorgulamanın, nörolojik öykü ve muayenenin önemini vurgulamaktayız
