Yoğun bakım hizmeti; bir ya da daha fazla organ
veya organ sistemlerinde oluşan yetmezliklerin tanı ve tedavisi için uygulanan hasta
bakım yöntemlerini içerir. Yoğun bakımlarda amaç, hastaların morbidite ve
mortalitelerini azaltmaktır. Çalışmamızda, yoğun bakımda takibi yapılan hastaların
İç Hastalıkları Bilim Dallarına göre yatış sıklıkları ve nedenleri, demografik
özellikleri ve yoğun bakım mortalitesini etkileyen faktörlerin değerlendirilmesi
amaçlandı. 1.Aralık. 2017- 1. Aralık 2018 tarihleri arasında Eskişehir Osmangazi
Üniversitesi Tıp Fakültesi İç Hastalıkları Yoğun Bakım Ünitesi’ne yatırılan 691
hastanın 399’u erkek (%57,7), 292’si (%42,3) kadındı. Hastaların 361’inin (%52,2)
servise devredildiği, 303’ünün (%43,8) ex, 27’sinin (%3,9) taburcu olduğu tespit
edildi. Hastaların sıklık sırasına göre gastroenteroloji, nefroloji, hematoloji, medikal
onkoloji bölümlerine yatırıldığı tespit edildi. Hastaların yatış nedenlerine göre en sık
gastrointestinal sistem kanaması, solunum yetmezliği, akut böbrek hasarı, septik şok
tanıları ile yatırıldığı belirlendi. Bölümlere göre mortalite oranları göre sırasıyla
hematoloji (%77), medikal onkoloji (%80) ve gastroentereoloji (%19,9)
saptandı(p<0,001) Yoğun bakım yatış süresi, APACHE-II (Acute Physiology and
Chronic Health Evaluation) skoru, GKS (Glasgow Koma Skalası) ile mortalite
arasında ilişki saptandı (p꞊0,07, p<0,001, p<0,001). Enfeksiyon ile ilgili
parametrelerden Laktat, CRP (C- Reaktif Protein) ve PCT (Prokalsitonin)’nin
mortaliteyi değerlendirmede anlamlı olduğu tespit edildi (p<0,001, p<0,001,
p<0,001). Elde ettiğimiz veriler; İç Hastalıkları bilim dallarına göre yoğun bakımda
hasta yatış sıklıklarının, yatış sürelerinin ve mortalitelerinin farklı olduğunu gösterdi.
Skorlama sistemlerinin etkin kullanılmasının önemi ve takipte enfeksiyöz
belirteçlerden laktat ve CRP değerlerinin mortalite ile ilişkili olduğu saptanmıştır
Intensive care service; It includes patient care methods applied for the

patients. In our study, the frequency and causes of hospitalization, demographic
characteristics and factors affecting intensive care mortality were evaluated
according to the Internal Medicine Departments of the patients who were followed
up in the intensive care unit. 1st of December. Between 2017 and 1. December 2018,

was determined that the patients were hospitalized in gastroenterology, nephrology,
hematology, and medical oncology departments in order of frequency. It was
determined that the patients were hospitalized with the diagnosis of gastrointestinal
bleeding, respiratory failure, acute kidney damage, and septic shock most frequently
according to their hospitalization reasons. According to the mortality rates according
to the departments, hematology (77%), medical oncology (80%) and
gastroentereology (19.9%) were determined respectively (p <0.001). There was an
association between duration of ICU stay, APACHE-II (Acute Physiology and
Chronic Health Evaluation), GKS (Glasgow Coma Scale) and mortality (p꞊0.07, p
<0.001, p <0.001). Lactate, CRP (C-Reactive Protein) and PCT (Procalcitonin), one
of the parameters related to infection, were found to be significant in evaluating
mortality (p <0.001, p <0.001, p <0.001). The data we obtained; It showed that the
frequency of hospitalization, duration of hospitalization and mortality are different in
intensive care unit according to the internal medicine branches. The importance of
using scoring systems effectively and lactate and CRP values, which are among
infectious markers, were associated with mortality
