Bu
uzmanlık tezinde, Eskişehir Osmangazi Üniversitesi (ESOGÜ) öğrencilerinde
ve ESOGÜ’de görevli 30 yaş ve altı öğretim elemanlarında keratokonus
taraması yapılması ve hastalığın prevalansının tespit edilmesi amaçlandı.
Çalışmada orantılı/oransal tabakalı örnekleme yöntemi kullanılarak tarama
muayenesine davet edilmek üzere randomizasyonla kişiler belirlendi. Bu
kişilerden davete teşrif eden, 30 yaş ve altındaki refraktif cerrahi öyküsü
olmayan 896 kişi çalışmaya dahil edildi. Katılımcıların demografik verileri, tıbbi
özgeçmiş/soygeçmiş bilgileri ve alışkanlıkları sorgulanarak katılımcıların;
refraksiyon, oküler response analyzer ile göz içi basıncı, en iyi düzeltilmiş
görme keskinliği, biyomikroskopik muayene, optik biyometri, Scheimpflug
korneal tomografi ve ön segment optik koherens tomografi değerlendirilmeleri
yapıldı. Olgular kornea tomografisindeki verilere göre keratokonus, ektaziye
yatkın ve normal olarak sınıflandırıldı. Keratokonus prevalansı %1,9 (n=17)
%95 GA: %1,1-%3), ektaziye yatkınlık prevalansı %1,8 (n=16, %95 GA: %1-
%2,9) olarak bulundu. Cinsiyete göre keratokonus prevalansı ise kadınlarda
%1,2 (n=7/590), erkeklerde %3,3’tü (n=10/306) (p=0,03). Keratokonus tanısı
alanların %76,5’i (n=13/17) hastalığının farkında değildi. Çok değişkenli
analizlerde; keratokonus için göz kaşıma (OR:3,69, p=0,021) ve akraba evliliği
(OR:26,74, p=0,009), ektaziye yatkınlık için ise sadece yaş (OR:1,27,
p=0,011) risk faktörü olarak bulundu. Bu çalışma ülkemizde toplum temelli
randomize bir örneklem ile gerçekleştirilen ilk keratokonus prevalans çalışması
niteliği taşımaktadır
In this thesis, it is aimed to conduct keratoconus
screening in Eskişehir Osmangazi University (ESOGÜ) students and (30 years
and below) faculty members working in ESOGÜ and to determine the
prevalence of the disease. Subjects to undergo keratoconus screening were
randomly selected using a proportional stratified sampling method. 896 people
who responded to the invitation and who did not have a history of refractive
surgery at the age of 30 or below were included in the study. The demographic
data, medical/family history and habits of the participants were collected using
a standardized questionnaire. Participants were evaluated with refractometer,
intraocular pressure measurement with ocular response analyzer, best corrected visual acuity, biomicroscopic examination, optical biometry,
Scheimpflug corneal tomography and anterior segment optical coherence
tomography. According to the tomographic data, the participants were
classified as keratoconus, ectasia susceptibility and normal. The prevalence
of keratoconus was 1.9% (95% CI:1.1%-3%), and ectasia susceptibility
prevalence was 1.8% (95% CI:1%-9.9%). The prevalence of keratoconus by
gender was 1.2% in women and 3.3% in men (p=0.03). 13 (76.5%) of those
diagnosed with keratoconus were unaware of their disease. In multivariate
analysis for keratoconus, eye rubbing (OR:3.69, p=0.021) and
consanguineous marriage were found (OR:26.74, p=0.009) as a risk factor
while only age (OR:1.27, p=0.011) was the risk factor for ectasia susceptibility.
This is the first population-based keratoconus prevalence study in a
randomized sample conducted in Turkey
