Sleeve gastrektomi, morbid
obezite tedavisinde etkili bir cerrahi prosedürdür. Anastomoz kaçağı en ölümcül
komplikasyonlarından biridir. Sildenafil sitrat, siklik guanozin monofast (cGMP) için
spesifik olan fosfodiesteraz tip 5’in (PDE-5) kompetitif ve güçlü bir inhibitörüdür. Bu
çalışma,sildenafil takviyesinin anastomoz yapılan dokuda oksidatif stres parametreleri
üzerine etkilerini belirlemek ve mikrovasküler yoğunluğu karşılaştırmayı
amaçlamaktadır. Erkek Wistar sıçanlar kontrol (s=10) ve deney (s=10) gruplarına
randomize edildi. Deneysel gruba 5 gün boyunca oral 10 mg/kg/gün sildenafil verildi.
Sleeve gastrektomi uygulanan sıçanlar 5. gün sakrifiye edildikten sonra mide
dokularına kama rezeksiyon uygulandı. Oksidatif stres için ELISA ile nitrik oksit,
malondialdehit, katalaz, süperoksit dismutaz, total antioksidan durum, total oksidan
durum, glutatyon düzeyi ölçüldü. Oksidatif stres indeksi değerleri hesaplandı.
Hematoksilin-Eozin ve Anti-CD34 ile boyanan dokuların mikrovasküler yoğunlukları
ve histopatolojik yapıları değerlendirildi. Sildenafil tedavisi ile doku TAS düzeyleri
değişmese de TOS düzeyleri anlamlı olarak azaldı. Deney grubunda doku NO
düzeyleri anlamlı olarak azalırken; katalaz ve glutatyon seviyelerinde ise anlamlı artış
izlendi. Yine deney grubunda OSI ve MDA düzeyleri azalırken; SOD düzeylerinde
artış olduğu saptandı. Histopatolojik değerlendirmelerde iki grupta normal epitel
yapısı izlendi. Ancak deneysel grupta vasküler dilatasyon, ödem, iltihap daha azdı ama
mikrovasküler yoğunluk daha yüksek görüldü. Bu çalışma sonucunda, sildenafilin
anastomotik mide dokusunda mikrosirkülatuar hemodinamikleri iyileştirerek yapılan
cerrahi işlemin başarısına katkıda bulunabileceği düşünüldü
Sleeve gastrectomy is an effective surgical procedure for treating of morbid obesity.
Cut surface leakage is one of the fatal complications. Sildenafil citrate is a competitive
and potent inhibitor of cyclic guanosine monophosphate (cGMP)-specific
phosphodiesterase type 5 (PDE-5). This study aims to determine the effects of
sildenafil supplementation on oxidative stress parameters and to compare microvessel
density in anastomotic tissue. Male Wistar rats were randomized into control (n=10)
and experiment (n=10) groups. The experimental group had oral sildenafil (10
mg/kg/day) for 5 days. Rats were sacrificed 5 days after Sleeve gastrectomy and then
wedge-resection was performed. Catalase, superoxide dismutase, glutathione, total
oxidant status, total antioxidant status, nitric oxide, and malondialdehyde levels were
measured by ELISA to assess oxidative stress. Oxidative stress index values were
calculated. The microvessel densities and histopathological structures were evaluated
with Hematoxylin-Eosin and Anti-CD34 staining. Although tissue TAS levels did not
change with sildenafil treatment, TOS levels significantly decreased. In the experiment
group, NO levels resulted in a significant decrease, whereas the tissue CAT and GSH
levels increased. In the experiment group, OSI and MDA levels decreased, while an
increase in the SOD levels was detected. In histopathological evaluation; normal
epithelial structure was observed in both groups. Vascular dilatation, edema, and
inflammation were lower in the experimental group but microvessel densities were
higher. This study showed that sildenafil supplement increased microvessel density in
tissue and it can improve wound healing by improving microcirculatory
hemodynamics and may contribute to the success of the surgical procedure
