İmplant uygulanmış hastalarda yapılan, bar ve locator tutuculu protetik
restorasyonların başarısını ve görülen komplikasyonların 2 yıl boyunca klinik ve
radyolojik olarak değerlendirmektir.
Gereç ve Yöntem: Ekim 2013 ile Ekim 2018 tarihleri arasında Eskişehir
Osmangazi Üniversitesi Protetik Diş Tedavisi Anabilim Dalına protez ihtiyacı için
başvuran ve implant uygulanmış hastalar arşivden taranıp röntgen ve kayıtları dahil
edildi. Kontrol seansları ve klinik muayene yüklemeden sonraki 12.ay ve 24. ayda yapıldı.
Hastaların demografik verileriyle birlikte; implant üstü overdenture protezlerde, protezin
lokalizasyonuna, implant sayısına, tutucu tipine ve karşıt çene durumuna göre protetik
komplikasyonlar ve implant çevresi kemik kayıplarının değerlendirilmesi yapıldı.
Radyolojik Değerlendirme ve Kemik Seviyesi Ölçümü: Ölçümler taranan ve
sayısallaştırılan ardışık radyografilerin görüntülerinden elde edildi ve Corel Draw 11.0
yazılımı kullanılarak X20 büyütmede analiz edildi. Retrospektif olarak verilerin
incelenmesinden sonra istatistiksel analizler yapıldı. İstatistiksel analiz: elde edilen
bulgular değerlendirilirken, verilerin analizi için ticari bir istatistiksel analiz programı
olan SPSS Windows 2013 yazılımı kullanıldı.
Bulgular: Çalışma 5 yıllık bir dönemde 284 implant uygulanan, 12. ve 24. ayda
kontrollerine gelen toplam 114 hasta üzerinde yapıldı. Hastaların yaşları 39 ile 91
arasında değişmekte olup, ortalama yaş 63.4'tür. Çalışmaya dahil edilen hastaların 53'ü
erkek ve 61’i kadın katılımcıdan oluşmaktadır. Hastaların 94’üne alt ve üst çene ayrı

olmak üzere locator bağlantılı, diğer 20 hastaya ise bar tutuculu overdenture protez
uygulanmıştır. İmplant destekli locator ve bar tutuculu overdenture protezlerde yükleme
yapıldıktan sonraki 2 yılın sonunda, tüm hastaların yaklaşık olarak üçte ikisinde %57.9
herhangi bir komplikasyon saptanmadı, hastaların yaklaşık üçte birinde %42.1'de bir
komplikasyon kaydedildi. İmplant yerleştirildiği bölge, sayısına, yüklemeden sonraki
güne ve tutucu türünden bağımsız olarak marginal kemik kaybını değerlendirdiğimizde
12. ve 24. ay sonunda anlamlı bulundu.
Sonuç: İmplant yüzeyinin ve bağlantı elemanlarının geliştirilmesi bu tedavi
yöntemini çok başarılı kılmıştır. Bu çalışmada analiz edilen verilere dayanarak, hareketli
protezler farklı tip ve sıklıkta komplikasyonlar göstermiştir. Bar tutuculu implant üstü
hareketli protezlerde, locator tutuculara kıyasla daha fazla komplikasyona rastlandı.
İmplant destekli overdenture protezlerde tedavi tamamlandıktan sonra protezlerin rutin
olarak kontrolü önem taşımaktadır. Bu kontrollerde oluşan gereksinimler
tamamlandığında ileride oluşabilecek komplikasyonların da önüne geçilmesi
planlanmaktadır
To evaluate the success of the prosthetic restorations with bar and locator
retainer and the complications seen clinically and radiologically for 2 years.
Material and method: Patients who applied to Eskişehir Osmangazi University
Department of Prosthodontics for prosthesis need between October 2013 and October
2018, and patients who had implants were scanned from the archive, and X-rays and
records were included. Control sessions and clinical examinations were conducted at 12
months and 24 months after loading. Along with the demographic data of the patients;
Prosthetic complications and bone loss around the implant were evaluated according to
the location of the prosthesis, the number of implants, the type of retainer, and the
opposite jaw condition in overdenture prostheses over implants. Radiological Evaluation
and Bone Level Measurement: The measurements were obtained from the images of the
scanned and digitized consecutive radiographs and analyzed at X 20 magnification using
Corel Draw 11.0 software. After analyzing the data retrospectively, statistical analyzes
were made. SPSS Windows 2013 software, a commercial statistical analysis package
program, was used for statistical analysis, while evaluating the findings obtained in the
study.
Results: Study between the years of 2013-2018, was performed on 114 patients
who underwent 284 dental implants and came to their controls at the 12th and 24th
months. Patients included in the retrospective study were 53 men and 61 women. A
locator attachment was applied to 94 patients with separate lower and upper jaws, and a

removable prosthesis with bar attachment was applied to 20 patients. Approximately two thirds of all patients had no complications in 57.9% of implant-supported locator and bar retained overdenture prostheses at the end of 2 years after loading. However, a
complication was recorded in approximately one-third of the patients and 42.1%. When
we evaluated marginal bone loss regardless of the area where the implant was placed, the
number of days after loading and the type of retainer, it was found to be significant at the
end of the 12th and 24th month.
Conclusion:The development of the implant surface and connecting elements has
made this treatment method very successful. Based on the data analyzed in this study,
removable prostheses showed different types and frequency of complications. Compared
to locator retainers, complications were more common in bar-retained implant removable
prostheses. In implant-supported overdenture prostheses, routine control of the prosthesis
is important after the treatment is completed. When the requirements arising in these
controls are completed, complications that may occur in the future are prevented
