Çalışmamızın amacı trapezius kası miyofasiyal ağrı sendromu
(MAS) olan hastalara radial şok dalga tedavisi (r-ESWT) ile kuru iğneleme (Kİ)
tedavisinin boyun ile omuz ağrı ve fonksiyonları üzerine sonuçlarını karşılaştırarak,
radial şok dalga tedavisinin etkinliğini değerlendirmektir. Çalışmamıza 01.01.2018 ile
01.03.2019 tarihleri arasında kliniğimize başvuran hastaların dosyaları tarandı ve
çalışma kriterlerine uygun olan 30 r-ESWT tedavisi ve 35 Kİ tedavisi almış olan
hastaların verileri dahil edildi. Hastaların dosyalarından demografik veriler (yaş,
cinsiyet, etkilenen taraf) dışında değerlendirme ölçekleri olarak vizüel ağrı skalası
(VAS), boyun dizabilite skalası (BDS), omuz ağrı ve dizabilite indeksi (OADI) ve
omuz ile boyun eklem hareket açıklığı (EHA) ölçümleri kullanıldı. Her iki grupta
haftada bir seans toplam 3 hafta tedavi alan hastalardan oluşmaktaydı. Değerlendirme
ölçekleri tedavi öncesi, tedavi bitimi ve bir ay sonra kontrol olarak değerlendirildi.
Çalışma bulguları olarak hem Kİ hem de r-ESWT grubunda tedavi öncesine göre
tedavi sonrası ve bir ay sonraki kontrol ölçümlerinde hepsinde iyileşmeler gözlendi ve
bu iyileşmelerin seviyesi istatiksel olarak anlamlı bulundu. Gruplar arası
karşılaştırmada ise tedavi bitiminde sadece omuz fleksiyonunda r-ESWT lehine
(p:0.043), birinci ay kontrolde ise ağrılı taraf (p:0.033) ve diğer taraf boyun lateral
fleksiyonu (p:0.001) ile omuz fleksiyonu EHA ölçümlerinde (p:0.001) r-ESWT lehine
anlamlı farklılık gözlendi. Çalışmamızdan elde ettiğimiz veriler ile birlikte r-ESWT
tedavisinin trapezius kası MAS tedavisinde en az Kİ tedavisi kadar etkin olduğunu ve
omuz ile boyun EHA üzerine daha etkili olduğunu göstermektedir. Sonuç olarak r ESWT’nin klinik pratikte Kİ tedavisine iyi bir alternatif olabileceğini ve hatta daha iyi
sonuçlar alınabileceğini düşünmekteyiz
The aim of our study is to evaluate the
effectiveness of radial shock wave therapy by comparing the results of radial shock
wave therapy (r-ESWT) and dry needling (KI) therapy on neck and shoulder pain and
functions in patients with trapezius muscle myofascial pain syndrome (MAS). In our
study, the files of patients who applied to our clinic between 01.01.2018 and
01.03.2019 were scanned, and data of 30 patients who received r-ESWT treatment and
35 patients who received KI treatment were included. Apart from demographic data
(age, gender, affected side), visual pain scale (VAS), neck disability scale (BDS),
shoulder pain and disability index (OADI) and shoulder and neck joint range of motion
(EHA) measurements were used as evaluation scales. In both groups, one session per
week consisted of patients who received a total of 3 weeks of treatment. The evaluation
scales were evaluated before treatment, after the treatment and one month after, as a
control. As the results of the study, improvements were observed in both the KI and r ESWT groups in the measurements after treatment and control-one month after the
treatment, and the level of these improvements was found to be statistically significant.
In the comparison of the groups, a significant difference was observed only in the
shoulder flexion in favor of r-ESWT at the end of the treatment (p:0.043) . In the first
month control measurements, a difference was found in favor of r-ESWT in painful
side (p:0.001) and other side neck lateral flexion (p:0.001) and shoulder flexion EHA
measurements (p:0.001). The data we obtained from our study show that r-ESWT
treatment is at least as effective as the Kİ in the treatment of trapezius muscle MAS
and is more effective on shoulder and neck EHA. As a result, we think that r-ESWT
can be a good alternative to Kİ for treatment MAS and even better results can be
obtained
