Kolesteatom; timpanik veya mastoid kavite içerisinde keratinize
skuamoz epitel ve subepitelyal bağ doku fragmanlarından oluşan kitle olarak
tanımlanmıştır. Kolesteatomun kesin etyopatogenezi halen bilinmemektedir.
Günümüzde kolesteatom tedavisinde kabul gören tek küratif tedavi seçeneği olan
cerrahinin yüksek maliyeti, morbiditesi ve nüks oranları nedeniyle kolesteatom
tedavisinde alternatif medikal tedavi araştırmaları hız kazanmıştır. Kolesteatom
patogenezini gözleyebilmek ve alternatif medikal tedavi seçenekleri geliştirebilmek
amacıyla kolesteatom nedeniyle opere edilen 3 hastadan alınan kolesteatom dokuları
ile kolesteatom doku kültürü oluşturuldu. HEKa hazır hücre dizileri de kültüre edildi.
Kolesteatom hücrelerine 5-FU uygulanan(n=9), siklosporin uygulanan(n=9),
takrolimus uygulanan(n=9), imiquimod uygulanan(n=12); HEKa hücrelerine 5-FU
uygulanan(n=9), siklosporin uygulanan(n=9), takrolimus uygulanan(n=9),
imiquimod uygulanan(n=12) ve 48. saatte sitokin ve canlılık düzeyi ölçümleri
yapılan gruplar oluşturuldu. Kontrol grubu olarak hiçbir ilaç uygulanmayan ve
ölçümleri hemen yapılan kolesteatom grubu(n=3), 48. saatte ölçümleri yapılan 48.
saat kolesteatom grubu(n=3), 48. saatte ölçümleri yapılan 48. saat HEKa grubu
oluşturuldu(n=3). Kolesteatom 48. saat kontrol grubunda; HEKa 48.saat kontrol
grubuna göre IL-6 ve TNF-α seviyeleri yüksek saptanmıştır. Bu sitokinlerin etki
mekanizmaları düşünüldüğünde JAK/STAT ve MAPK, PI3K/Akt(PKB) sinyal
yolağı ve NFκB proteininin etyopatogenezde önemli rol oynadığı düşünülmektedir.
Uygulanan tüm ilaçlar antiinflamatuar, sitotoksik ve antiproliferatif etki göstermiştir.
Özellikle İmiquimod 50 µg/ml dozunda güçlü etkisi ile kolesteatomda potansiyel bir
tedavi umududur
Cholesteatoma is defined as a mass consisting of keratinized
squamous epithelium and subepithelial connective tissue fragments within the
tympanic or mastoid cavity. Etiopathogenesis of cholestetaoma has still remained
unclear. Surgery is the only one acceptable treatment of cholesteatoma currently but;
investigation of alternative medical treatment options of cholesteatoma speed up
because of high cost, morbidity, recurrence rate of surgery. In order to observe the
pathogenesis of cholesteatoma and to develop alternative medical treatment options,
cholesteatoma tissue culture was formed with cholesteatoma tissues obtained from 3
patients who operated for cholesteatoma. HEKa ready cell lines were also cultured.
5-FU applicated(n=9), cyclosporine applicated(n=9), tacrolimus applicated(n=9),
imiquimod applicated(n=12) cholesteatoma cells; 5-FU applicated(n=9),
cyclosporine applicated(n=9), tacrolimus applicated(n=9), imiquimod
applicated(n=12) HEKa cells were separated groups and 48th hour cytokines and
viability measurements were established. As the control group, cholesteatoma
group(n=3) without any application and measured immediately, 48th hour
cholesteatoma group (n=3), measured at 48th hour, and 48th hour HEKa group(n=3),
measured at 48th hour, were formed. In the 48th hour control group of
cholesteatoma; IL-6 and TNF-α levels were found to be higher than the 48th hour
control group of HEKa. Considering the mechanism of action of these cytokines, the
JAK/STAT, MAPK, PI3K/Akt(PKB) signal pathway and NFκB protein are thought
to play an important role in the etiopathogenesis of cholesteatoma. All drugs
applications showed anti-inflammatory, cytotoxic and antiproliferative effects.
Especially Imiquimod is a potential treatment prospect in cholesteatoma because of
strong effect at the dose of 50 µg / ml
