Bu
çalışmaya, 01.04.2019-31.09.2019 tarihleri arasında son 6 aydır sistemik tedavi
almamış 60 psoriasis hastası ile sistemik ve dermatolojik hastalığı olmayan 60 gönüllü
dahil edildi. Psoriasis grubunda hastalık şiddetinin hesaplanması için Psoriasis Alan
Şiddet İndeksi (PAŞİ) kullanıldı. Metabolik Sendrom (MetS) varlığı NCEP III
kriterlerine göre belirlendi. Kardiyovasküler risk Framingham Risk Skorlamasına
(FRS) göre hesaplandı. Katılanların hemogram, glukoz, insülin, trigliserit, kolesterol,
düşük ve yüksek dansiteli lipoprotein, C-reaktif protein ile oksidatif stres belirteci
olarak total tiyol, native tiyol ve disülfid değerleri ölçüldü. Hasta ve kontrol grubu
cinsiyet ve yaş dağılımı açısından benzerdi. Psoriasis grubunda kontrol grubuna göre
total tiyol ve native tiyol daha düşük; disülfid daha yüksek olmakla beraber gruplar
arasında istatistiksel olarak anlamlı farklılık bulunmadı. PAŞİ ile disülfid arasında
aynı yönlü anlamlı ilişki saptandı. MetS olanlarda total tiyol ve native tiyol hem
psoriasis hem kontrol grubunda anlamlı derecede düşük bulundu. Psoriasis
hastalarında FRS kontrol grubuna göre anlamlı derecede yüksek; FRS ile total tiyol ve
native tiyol arasında da ters yönlü anlamlı ilişki saptandı. Sonuç olarak çalışmamızda
psoriasis hastalarında MetS varlığı ve FRS yüksekliği ile tiyol-disülfid homeostazının
difülfid lehine bozulmuş olduğunun görülmesi psoriasis hastalarında
kardiyometabolik komorbiditelerin gelişmesinde oksidatif stresin de bir etken
olduğunu desteklemektedir. Psoriasis hastalarında tiyol-disülfid homeostazının
değerlendirilmesi; hangi hastaların kardiyometabolik hastalıklar açısından risk altında
olduğunu tahmin edebilmek ve bu hastalar için uygun tedavi seçenekleri oluşturmak
adına yararlı ve prediktif bir yöntem olabilir
This study was conducted
between 120 people, including 60 psoriasis patients who have not received any
systemic treatment for the last 6 months and 60 volunteers without any systemic and
dermatological disease, between 01.04.2019-31.08.2019. Psoriasis Area Severity
Index (PASI) was used to calculate the severity of the disease in the psoriasis group.
The presence of Metabolic Syndrome (MetS) was determined according to the NCEP
III criteria. Cardiovascular risk was calculated using the Framingham Risk Scoring
(FRS). The hemogram, glucose, insulin, triglyceride, cholesterol, low-and -high
density lipoprotein, C-reactive protein and total thiol, native thiol and disulfide values
were measured. The patient and control groups were similar in terms of gender and
age distribution. Although total thiol and native thiol values were lower in psoriasis
group than the control group and disulfide values were higher, no statistically
significant difference was found between the groups. There was a significant positive
correlation between PASI and disulfide. In those with MetS, total thiol and native thiol
values were significantly lower in both psoriasis and control groups. FRS was
significantly higher in psoriasis patients than in the control group and a significant
negative correlation was found between total thiol and native thiol values. In
conclusion, the presence of MetS and elevation of FRS, and the fact that thiol-disulfide
homeostasis is impaired in favor of disulfide supports oxidative stress in the
development of cardiometabolic comorbidities in psoriasis patients. Evaluation of
thiol-disulfide homeostasis in psoriasis patients can be a useful and predictive method
to predict which patients are at risk for cardiometabolic diseases and to create
appropriate treatment options for these patients
