Çalışmamızda, yüksek yağlı diyet (HFD) uygulanmış sıçanlarda, düşük
doz Streptozotosin (STZ) ile Tip 2 Diyabet (T2DM) modeli oluşturulmuş,
25(OH)D takviyesinin kan glukoz, insülin, insülin direnci (İD), lipid profili,
adipokinler ve adipositokinler üzerindeki etkileri incelenmiş ayrıca koroner ve
renal arterlerde ileri glikasyon son ürünleri (AGE) oluşumu açısından
değerlendirilmesi amaçlanmıştır.
Kapsam: Vitamin D eksikliğinin, artmış insülin rezistansı, hemoglobinA1c
(HbA1c) ve AGE oluşumu ile ilişkilendirildiğine dair çalışmaların mevcut
olması ve adipokin aracılı vasküler komplikasyonların bildirilmiş olması
nedeniyle Vitamin D takviyesinin diyabet tedavisinde olası rolü ve sitokinlerle
ilişkisini çalışmayı planladık.
Yöntem: Sıçanlar T2DM indüklenmeden önce 4 hafta süresince HFD ile
beslenmiş ardından düşük doz STZ ile diyabet indüklenmiştir. Süre sonunda;
kan glukoz, insülin seviyesi, yüksek dansiteli lipoprotein (HDL), total kolesterol
(TK), trigliserit (TG) seviyeleri, karaciğer dokusunda total antioksidan kapasite
(TAS), total oksidan kapasite (TOS), oksidatif stres indeks (OSI), serum
adiponektin ve apelin seviyeleri, böbrek dokusunda, tümör nekröz faktör-α
(TNF-α), interlökin-6 (IL-6) seviyeleri ile koroner ve renal arterlerde AGE
formasyonları belirlenmiştir.
Bulgular: T2DM grubunda, Vitamin D takviyesi ile kan glukoz seviyesinde
anlamlı bir azalma ve insulin seviyesinde anlamlı bir artış gözlenmiştir. TK
seviyeleri, HFD grubunda Vitamin D takviyesiyle anlamlı derecede azalmıştır.
HDL seviyeleri, yalnızca T2DM grubunda Vitamin D takviyesiyle anlamlı
derecede artmıştır. TG seviyelerinde, Vitamin D’nin olumlu bir etkisi
bulunamamıştır. Ayrıca artan insulin direnci (HOMA-IR) düzeyleri, Vitamin D
takviyesi ile düzelmemiştir. Adiponektin seviyeleri, kontrol grubuna göre tüm
gruplarda azalmış ve T2DM grubunda Vitamin D’nin adiponektin seviyesi
üzerine arttırıcı etkisi gözlenmiştir. Apelin seviyelerinde Vitamin D’nin etkisi

olmamıştır. Böbrek sitokin düzeylerinde, T2DM grubunda Vitamin D’nin bu
sitokinlerin düzeyini azaltıcı etkisi gözlenmiştir. OSI açısından ise, Vitamin D,
T2DM grubunda etkisiz iken, HFD grubunda azaltıcı bir etkisi gözlenmiştir.
Tedavi grubu ile diyabetik grup karşılaştırmasında, T2DM modeli açısından,
HFD grubunun, pre-T2DM dönemini yansıtması açısından değerlendirmesinin
uygun olacağını düşünmekteyiz
Sonuç: Bulgularımızın ışığında, Vitamin D takviyesinin T2DM ve buna zemin
hazırlayan metabolik sendrom tablosunda, kan glukozu, insulini ve sitokinler
aracılı olumlu katkılarının olacağını düşünmekteyiz
In our study, high-fat diet (HFD) applied rats and Type 2 Diabetes
(T2DM) model rats were created with low dose Streptozotocin (STZ), the effects
of 25 (OH) D supplements on blood glucose, insulin, insulin resistance (ID), lipid
profile, adipokines and adipocytokines were investigated and we aimed to
evaluate advanced glycation end products (AGE) in coronary and renal arteries.
Content: Vitamin D deficiency is associated with increased insulin resistance,
HbA1c and AGE formation and adipokine-mediated vascular complications
have been reported. In our study we aimed to investigate the possible role of
Vitamin D supplementation in the treatment of diabetes and its relationship
with cytokines.
Method: Rats were fed with HFD for 4 weeks and then diabetes was induced
with low dose STZ. At the end of the period; blood glucose, insulin level, high
density lipoprotein (HDL), total cholesterol (TK), triglyceride (TG) levels, tumor
necrosis factor-α (TNF-α), interleukin-6 (IL-6) levels in kidney tissue, serum
adiponectin and apelin levels, total antioxidant capacity (TAS), total oxidant
capacity (TOS), oxidative stress index (OSI) in liver tissue and AGE formations
in coronary and renal arteries were determined.
Results: In the T2DM group, a significant decrease in blood glucose level and a
significant increase in insulin level were observed with Vitamin D
supplementation. Increased insulin resistance (HOMA-IR) levels, Vitamin D
supplementation didn't improved. TK levels decreased significantly with
Vitamin D supplementation in the HFD group. HDL levels increased
significantly with Vitamin D supplementation only in the T2DM group. Vitamin
D no positive effect was found on TG levels. Adiponectin levels decreased in all
groups compared to the control group and an enhancing effect of Vitamin D on
adiponectin level was observed in the T2DM group. Vitamin D had no effect on
apelin levels. In kidney cytokine levels, Vitamin D decreased the level of these
cytokines in the T2DM group. When OSI was evaluated, Vitamin D had a

reducing effect in the HFD group. In terms of the T2DM model, we think that
the HFD group is important in terms of reflecting the pre-T2DM period and we
believe that it is important in the comparison of treatment groups in this
respect.
Conclusions: In the light of our findings, we think that Vitamin D
supplementation will have positive effects on blood glucose, insulin and
cytokines in T2DM and the metabolic syndrome table that prepares the ground
for this
